The organ restrained by the Heart is the Kidney; its enemy is the Lung; it favors the Bitter flavor; it is injured by the Salty flavor. It corresponds to the Zang organ of "Three Transformations and Eight Steaming." When in harmony, the temperament is joyful and happy; when diseased, it manifests as fright epilepsy, fear, palpitations, deficiency-induced restlessness and crying, delirious speech, mania and irritability, drooling at the corners of the mouth, and for smallpox, red macules.
The organ restrained by the Liver is the Lung; its enemy is the Spleen; it favors the Sour flavor; it is injured by pungent and spicy substances. It corresponds to the Zang organ of "Initial Transformation and Six Steaming." When in harmony, the Po (Corporeal Soul) is strong and wisdom arises; when diseased, it primarily governs Wind syndromes, evolving into convulsions and spasms, swollen, red, and painful eyes, and for smallpox, vesicles.
The organ restrained by the Lung is the Heart; the Heart's enemy is the Liver [Note: Textual anomaly]; it favors the Pungent flavor; it is injured by scorched and bitter flavors. It corresponds to the Zang organ of "Second Transformation and Seven Steaming." When in harmony, one feels pleased, with clear Qi, bright spirit, and strong Hun (Ethereal Soul); when diseased, it primarily governs wheezing and fullness, coughing, Shanghan (Cold Damage), excessive accumulation of deficient phlegm, and for smallpox, pustules.
The organ restrained by the Spleen is the Liver; its enemy is the Kidney; it favors the Sweet flavor; it is injured by the Sour flavor. It corresponds to the Zang organ of "Fourth Transformation and Nine Steaming." When in harmony, it digests grain Qi and enhances the taste of food; when diseased, it primarily governs vomiting and hiccups, Gan accumulation (malnutrition syndrome), deficiency dysentery, abdominal masses and parasites, tidal fever with loss of appetite for milk/food, and for smallpox, papules/rash.
The organ restrained by the Kidney is the Spleen; its enemy is the Heart; it favors the Salty flavor; it is injured by the Sweet flavor. It corresponds to the Zang organ of "Fifth Transformation and Ten Steaming." When in harmony, it manifests as walking, sitting, playing, laughing, and speaking; when diseased, it primarily manifests as blackened teeth resembling broken sand, hair standing on end with teeth grinding, purulent otorrhea, and for smallpox, black depressed lesions.
*Quan You Xin Jian* states: If a child's fontanelle is sunken or bulging, sweat does not flow freely but beads up like oil, the tongue extends retracting to the posterior hairline, the tongue swells causing fright, stool contains dark black blood, hair stands stiffly like hemp, and the skin lacks color, this indicates Heart Qi exhaustion, leading to death on Ren-Gui days.
If the head is heavy with crying but no tears, or if during illness there is no crying nor tearing, nails are cyanotic-black, eyes are deeply sunken, the tongue is curled and the scrotum retracted, convulsions occur with strabismus, lip and mouth twitch involuntarily, hands assume a posture as if holding the head, or the dorsum of the foot is rigidly straight. *Su Wen* states: "Its luster appears in the nails, its nourishment is in the sinews." This indicates Liver Qi exhaustion, leading to death on Geng-Xin days.
If the philtrum is full and black, lips are scorched and dry, or lips are dry, purple, and black, lips fail to cover the teeth while the face is swollen, hematuria occurs, the tongue is short or curled, nares are dilated, teeth are clenched with cold sweat like oil, the mouth puckers like a bag, the complexion is earthy, limbs are cold and reversed, feeling warm like heated stone upon touch, milk intake is uncontrolled, and stool is red-black, this indicates Spleen Qi exhaustion, leading to death on Jia-Yi days.
If there is heat, and drinking soups, water, medicines, or food causes rattling in the throat, this indicates the stomach and duodenum are directly open and cannot shelter the Lung. This syndrome is rarely recorded in other texts; I have attempted treatment but could not cure it.
If the eyes are fixed with a bright cyan hue, asthma/wheezing is unremitting, eating causes choking and coughing, phlegm and saliva block the mouth, there is rattling in the throat, nasal congestion with dryness and blackening, lung distension and diaphragmatic obstruction, sweating only on the head while limbs are cold, this indicates Lung Qi exhaustion, leading to death on Bing-Ding days.
If the face is black with hanging lips, eyes are black-cyan and swollen, gaze lacks luster, earlobes are cyan-yellow, scorched, and dry, teeth suffer from Gan disease, hair falls out sparse and yellow, skin is dry and dark, fright-wind occurs, coughing milk involves teeth grinding, qi leaks, and blackness surrounds the mouth, this indicates Kidney Qi exhaustion, leading to death on Wu-Ji days.
