Xue Ji stated: The *Neijing* records, "The Heart stores the Shen (Spirit); when in excess, there is incessant laughter." It also states, "In the Zang organs it corresponds to the Heart; in vocalization it corresponds to laughter; in emotions it corresponds to joy."
Furthermore, excessive Fire is called "He Xi" (abundant and blazing heat). In years of He Xi, the diseases manifest as laughing, jesting, madness, and delirium. It is also said, "When the Shaoyin [Kidney/Heart] qi arrives, it manifests as joy and laughter." It is further stated, "When Jing Qi (Essence Qi) converges in the Heart, there is joy." These statements all indicate conditions belonging to Heart Fire. If laughter is incessant accompanied by groaning and abdominal pain, this is Water encroaching upon Fire, with Yin striking Yang. Yang becomes trapped and generates heat, leading to madness, delirious speech that is incomprehensible, which indicates damage to the Heart. Bian Que said: If the patient's lips and mouth are red, the condition is treatable; if they are blue-black, it is fatal. If Kidney Water is deficient and dry, failing to control Heart Fire, resulting in incessant joy and laughter, use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). If Liver Fire is blazing, generating Heart Fire, resulting in incessant joy and laughter, use Chaihu Qinggan San (Bupleurum Liver-Clearing Powder). For other accompanying syndromes, treatment should be adapted according to each specific syndrome.
A child frequently laughed and was restless, with a red face, desire for cold drinks, and hot hands and feet. Initially, Huanglian Xie Xin Tang (Coptis Decoction to Drain the Heart) powder was administered; after two doses, the condition slightly stabilized. Subsequently, Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) ingredients were decocted and taken, resulting in immediate recovery. Regular consumption of this pill ensured stability; if not taken for a month, the symptoms recurred, but resuming medication cured it again.
Another child presented with the aforementioned symptoms, showing a bluish-red complexion. This was caused by Wind-Heat affecting the Liver and Heart meridians. Chaihu Zhizi San (Bupleurum and Gardenia Powder) and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) were used, leading to gradual recovery. Additionally, the wet nurse suffered from anger-induced fever. Jiawei Chaihu Tang (Modified Bupleurum Decoction) was initially used, followed by Jiawei Xiaoyao San (Modified Free Wanderer Powder), both mother and child recovering after taking them.
A fourteen-year-old child, having overused mental effort and irregular diet, suffered from incessant laughter. The pulse was large and surging yet empty upon pressure; the complexion was red or pale. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) was used to effect a cure. In the following autumn, during the imperial examinations, due to dietary indiscretion and fatigue, the previous condition recurred, sometimes accompanied by delirious speech. The pulse was large and surging, but fine and nearly imperceptible upon deep pressure. Ren Shen (Ginseng Root) 1 liang, decocted with Sheng Jiang (Fresh Ginger Rhizome) and Da Zao (Jujube Fruit), was administered; the condition slightly stabilized, and three additional doses led to full recovery. Later, due to physical labor and mental strain, spontaneous sweating, thirst, and irritability resembling epilepsy occurred. Dang Gui Bu Xue Tang (Angelica Tonifying Blood Decoction) was initially used; after two doses, calmness was restored. Finally, Shiquan Dabu Tang (Ten Substance Great Tonic Decoction) was administered, leading to swift recovery.
