Mr. Tang stated: In pediatric hot diarrhea (Hot Diarrhea), the stool is yellow with a reddish hue, sometimes containing froth. This condition arises from accumulation within the zang-fu organs. Causes may include the nursing mother's preference for alcohol or heat-inducing foods, or the infant contracting dampness at birth which transforms into heat. If physicians fail to understand the pathology and merely administer cardamom (Doukou) and chebula (Hezi), it is as ineffective as pouring water on stone. Since they do not recognize the syndrome, they cannot differentiate between cold and heat, and their therapeutic methods are inappropriate; how can they expect efficacy? This syndrome should be verified by signs such as scanty dark urine, dry mouth, and irritability. The treatment should employ Qian Yi's Baizhu San (Atractylodes Macrocephala Decoction) with Xiangru removed. Alternatively, Wuling San (Five-Ingredient Poria Decoction) may be used (if accompanied by fright, Guizhi may be omitted). For severe heat, Sini San (Frigid Extremities Powder) or Daxiehu Tang (Major Bupleurum Decoction) [both from Shanghan Lun] with Dahuang removed are effective. Additionally, Huanglian Wan (Coptis Pill) [for Gan malnutrition] is also suitable. Tiaozhong Tang (Regulate Middle Decoction) [for dysentery] with Dahuang removed, plus Huanglian and Zhike, is indicated. For diarrhea complicated by heat, specifically Taiyang and Shaoyang combined disease presenting with spontaneous diarrhea, Huangqin Tang (Scutellaria Decoction) should be given; if vomiting occurs, Banxia is added. For dysentery with heat complication characterized by difficult, dark red urination, Jiajian Sishun Qingliang Yin (Modified Four-Compliance Cooling Drink) [for interior heat] is used to stop the symptoms.
Mr. Xue stated: Redness of the right cheek with a desire for cold drinks indicates excess heat in the Stomach meridian, treated with Xiehuang San (Drain Yellow Powder) [for Spleen heat]. Aversion to cold and preference for warmth indicates deficiency heat in the Stomach meridian, treated with Baizhu San [for thirst]. Redness of both the right cheek and forehead indicates heat in the Heart and Spleen, treated with Xiehuang San plus stir-fried black Huanglian. Redness of both the left cheek and right cheek indicates Liver fire invading the Spleen earth, treated with Sijunzi Tang (Four Gentlemen Decoction) [for vomiting and diarrhea] plus Chaihu. If an infant suffers sudden injury from milk or food, Baohe Wan (Preserve Harmony Pill) [for retained food] is used. The nursing mother must strictly avoid rich flavors and regulate her diet. If the mother suffers from food stagnation causing infant vomiting and diarrhea, the mother should be treated. Generally, initial illness with heat indicates excess due to pathogenic qi dominance; eventual transformation into cold indicates deficiency due to depletion of true qi; prolonged illness with heat indicates true interior cold with false exterior heat. Prolonged diarrhea leading to deficiency-cold of Yuan Qi should be treated according to the aforementioned syndromes.
