[Wan] Generally, for mild cases of smallpox (Xiaochuang), the root is shallow, the pathogenic toxin is slight, and the syndrome is favorable, leading to natural scabbing and healing without residual toxins. For severe cases, the root is deep, the toxin is rampant, and the syndrome is dangerous, making it difficult to erupt and scab, resulting in residual toxins. In adverse cases, the toxin may be latent, inverted, or scab abnormally. Fortunately, if the Spleen and Stomach are inherently strong and treatment is timely, the condition will not become critically desperate, yet residual toxins may still arise. Qian Yi stated: There are three manifestations of this disease: one is scabies, the second is carbuncles, and the third is red eyes. Based on symptoms, there are more than these; even after eruption is complete, fully developed, and uniformly scabbed, residual toxins may reappear due to excessive warm-tonifying treatments or dietary indiscretion. Therefore, treating post-smallpox residual toxins requires distinguishing between exterior and interior, and assessing deficiency and excess, using tonification, dispersal, or resolution methods as appropriate, rather than blindly administering decoctions indiscriminately.
[Hai Zang] Residual toxins from sores may affect the Liver deficiency invading the eyes, or Lung deficiency manifesting as tinea and scabies. Carbuncles and furuncles occurring at bone joints indicate Kidney deficiency; those occurring in muscles indicate Spleen deficiency. Manifestations in tendons, head, face, teeth (with carious erosion), or throat swelling and pain all follow their respective meridians, arising from accumulated and unresolved toxins. This often results from delayed initial treatment or medical error, leaving toxins lingering in the meridians. Decoctions for detoxification are suitable.
[Shi Xian] If smallpox toxin attacks the Spleen, it causes diarrhea and edema; if it attacks the Liver, it causes corneal opacity; if it attacks the Heart, it causes irritability and crying; if it attacks the Kidneys, it causes earache and pus accumulation; if it attacks the Lungs, it causes cough and phlegm.
[Jie Gu] If, after eruption, residual toxins remain unresolved and body heat and sores recur, administer Jiedu Wan (Detoxification Pill) with tea.
[Dan Xi] To treat pediatric smallpox where residual toxins remain unresolved due to early consumption of grains, trapping the toxic qi:
Niu Bang Zi (Burdock Fruit, 6 qian), Gan Cao (Licorice Root, 5 fen), Xi Jiao (Rhinoceros Horn), Bai Zhu (Atractylodes Macrocephala Rhizome, 3 qian each), Jing Jie (Schizonepeta), Fang Feng (Saposhnikovia), Zhi Ke (Bitter Orange, 1 qian each).
Decoct in water and take warm.
Huoxue Jiedu Tang (Blood-Invigorating and Detoxifying Decoction) treats residual toxins.
Fang Feng, Jing Jie, Sheng Di Huang (Rehmannia Root), Chi Shao Yao (Red Peony Root), Dang Gui (Angelica Sinensis), Lian Qiao (Forsythia), Niu Bang Zi, Huang Lian (Coptis), Zi Cao (Arnebia), Gan Cao, Cang Zhu (Atractylodes Lancea), Bo He (Mint), Chuan Xiong (Ligusticum), Mu Tong (Akebia).
Equal parts, decoct in water and take.
[Carbuncles and Furuncles] These arise because toxic qi remains lodged in the meridians, causing redness and swelling into carbuncles at areas of muscle deficiency or joint mobility. Alternatively, after subsidence, failure to resolve residual toxins leads to their proliferation. Unable to vent through the skin or enter the viscera, they accumulate and form carbuncles. Mild cases present only as nodules, swellings, or furuncles. Severe cases involve widespread swelling and pain in the head, neck, chest, flanks, hands, feet, and joints. If limited to one or two sites with shallow roots, it is treatable. However, if migrating and fluctuating with deep roots and spreading extensions, minor cases may ulcerate tendons and expose bones, leading to permanent disability; major cases may persist until death. [Dan Xi] Smallpox carbuncles are mostly caused by excess toxins and blood heat. Medication should be selected based on upper or lower body location; delay is unacceptable. When suppuration occurs, clear heat and cool blood primarily using Chi Shao Yao, Gan Cao Jie (Licorice Knots), Lian Qiao, Jie Geng (Platycodon), etc. Use Sheng Ma (Cimicifuga) and Ge Gen (Pueraria) as guides for the upper body; use Bing Lang (Areca Seed) and Niu Xi (Achyranthes) for the lower body. Assist with Bei Mu (Fritillaria), Ren Dong Cao (Honeysuckle Stem), Bai Zhi (Angelica Dahurica), and Gua Lou (Trichosanthes Fruit). Use Da Huang (Rhubarb) for dry stool; use Huang Qin (Scutellaria) and Huang Bai (Phellodendron) for alternating chills and fever. This method has proven effective repeatedly. [Xue Ji] For initial stages without suppuration, use Xianfang Huoming Yin (Immortal Life Elixir) and Garlic-Separated Moxibustion to dissipate them. To induce suppuration, use Xianfang Huoming Yin to rupture them. For Qi and Blood deficiency, use Bazhen Tang (Eight Treasure Decoction) for debility; for excess conditions rupturing, use Tuoli San (Support Interior Powder) to tonify and promote closure. Generally, for fever, swelling, and pain with unconstipated stools, use Xianfang Huoming Yin and garlic-separated moxibustion. For constipation, add Da Huang to Xianfang Huoming Yin. Once bowels move but pain persists, administer another dose of Xianfang Huoming Yin followed by Tuoli San to tonify Yuan Qi. For fever and fatigue with normal bowels, use Buzhong Yiqi Tang (Middle-Boosting and Qi-Enriching Decoction); if ineffective, use garlic-separated moxibustion. For post-ulcer fever, dry mouth, and limb fatigue, use Dongyuan Shengyu Tang (Dongyuan's Holy Salvation Decoction). For dripping pus and intermittent fever, use Sijunzi Tang (Four Gentlemen Decoction) plus Renshen (Ginseng) and Huang Qi (Astragalus). If the nursing mother has Liver channel blood deficiency causing fever, use Jiawei Xiaochaihu Tang (Modified Minor Bupleurum Decoction). For Liver-Spleen depression and anger causing fever, use Jiawei Guipi Tang (Modified Restore the Spleen Decoction). For heat accumulation from rich foods, use Jiawei Qingwei San (Modified Clear Stomach Powder). Using only cooling-blood herbs like Baidu agents damages Yuan Qi, preventing ulceration or closure. [Wan Quan] When treating smallpox carbuncles, first identify the meridian involved and adjust guiding herbs accordingly: Qiang Huo (Notopterygium) and Fang Feng for Taiyang; Sheng Ma and Bai Zhi for Yangming; Chai Hu (Bupleurum) for Shaoyang; Du Huo (Angelica Pubescens) for Shaoyin; Fang Feng for Taiyin; Chai Hu for Jueyin. For initial red, swollen, hard, and painful lesions, prick with a needle, suck out bad blood with the mouth, and apply Badu Gao (Toxin-Pulling Ointment). For robust patients with solid stools, use Paidu San (Toxin-Expelling Powder) to regulate. For Qi-deficient patients with poor appetite, use Shixuan San (Ten-Accumulation Powder) or Liuqi Yin (Flowing-Qi Decoction). For mild, small toxins, internally administer Xiaochaihu Tang plus Sheng Di Huang, externally applying Badu Gao. This treats pre-ulcerative stage. For suppurated but unruptured lesions, use a Pi Zhen (scalpel) to drain pus to prevent internal spread. For ruptured lesions, use Shiquan Dabu Tang (Complete Great Tonifying Decoction). This treats the ulcerative stage.
Xianfang Huoming Yin (Immortal Life Elixir Preserved in the Field) treats smallpox carbuncles, toxins, and all sores; it dissipates unformed ones and ruptures formed ones. It is a holy medicine for disinfection, pus elimination, and pain relief.
Jin Yin Hua (Honeysuckle Flower), Chen Pi (Tangerine Peel, 3 qian each), Zao Jiao Ci (Gleditsia Spine, stir-fried), Chuan Shan Jia (Pangolin Scale, stir-fried with clam powder), Ru Xiang (Frankincense), Mo Yao (Myrrh), Bai Zhi, Fang Feng, Dang Gui (1 qian each), Bei Mu (Fritillaria), Tian Hua Fen (Trichosanthes Root), Gan Cao Jie (Licorice Knots, 0.7 qian each).
Take 5 qian per dose, decoct in wine. Infants and mothers take together. Can also be powdered and taken with wine. If severe with exterior pathogens, add Ma Huang (Ephedra) to disperse; if toxin is interior, add Da Huang to purge. Adapt to clinical presentation; this is a life-saving agent, but contraindicated in Yuan Qi collapse.
Renshen Baidu San (Ginseng Detoxification Powder) treats residual toxin carbuncles.
Renshen, Chi Fu Ling (Red Poria), Qiang Huo, Du Huo, Qian Hu (Peucedanum), Bo He, Chai Hu, Zhi Ke, Chuan Xiong, Jie Geng (equal parts), Gan Cao (half dose), Niu Bang Zi, Fang Feng, Jing Jie, Lian Qiao, Jin Yin Hua (this is Jingfang Baidu San).
For head: add Bai Zhi, Sheng Ma. For upper body: double Jie Geng. For hands: add Hou Gui (Thin Cinnamon). For waist: add Du Zhong (Eucommia). For legs/feet: add Niu Xi, Mu Gua (Chaenomeles).
Hua Pi San (Birch Bark Powder) treats smallpox, mastitis, and all swellings.

Xi Jiao Xiaodu Wan (Rhinoceros Horn Detoxification Pill) treats post-smallpox residual toxins and all sores.
Sheng Di Huang, Jing Jie, Dang Gui, Xi Jiao (powder), Fang Feng, Niu Bang Zi (crushed, fried), Chi Shao Yao, Lian Qiao, Jie Geng (7 qian each), Bo He, Huang Qin, Gan Cao.
Powder, honey-pellet size of Eccarya seed. Take one pellet dissolved in warm water.
Note: Huadu Dan lowers fire, cools blood, and clears cold-toxins. Xiaodu Wan clears heat, detoxifies, and breaks blood stasis. Children’s organs are delicate, and Yuan Qi is easily damaged. Post-smallpox Qi and Blood are deficient; such potent formulas may harm Stomach Qi. If Stomach Qi is damaged, unformed toxins won't dissipate, formed ones won't suppurate, and ruptured ones won't granulate. Smallpox originates from viscera, turning blood into pus. Recurrence after healing indicates Spleen-Stomach deficiency, muscle wasting, and shortness of Ying-Wei. Physicians must exercise caution.
Shixuan San (see Appearance section).
Tuoli San (Support Interior Powder) treats weak Yuan Qi from smallpox toxin unable to suppurate/disperse; prevents formation or induces rupture.
