Pediatric patients presenting with high fever, vomiting, restlessness of the heart and mind (Shen), convulsions of the hands and feet, rigidity of the body, and retraction of the eyes are suffering from Fetal Convulsion Wind Syndrome (Tai Jing Feng Zheng). "Fetal Convulsion" refers to a condition caused by the pregnant woman's improper care during gestation, such as drinking alcohol, indulging in desires, anger, or being startled and struck. When the mother is affected, the fetus inevitably senses it; alternatively, external pathogenic wind may damage the fetus. Consequently, the child inherits the maternal qi and falls ill immediately after birth. The manifestations include: within the first month, high fever, upward deviation of the eyes, clenched fists, trismus (lockjaw), rigidity of the back and waist, flooding of saliva with vomiting, convulsions with startled crying, retracted cheeks, and open fontanelles. Alternatively, the cheeks may be red, or the face blue with closed eyes. (Note: Cases of fetal wind causing eye closure must not be mistaken for Slow Spleen Wind [Man Pi], and warm tonifying medicines should not be used recklessly.) Symptoms such as eye deviation and trismus also stem from this category. Observing the color of the area between the eyebrows: if it is bright red with a hint of greenish-blue, it is treatable; if it is dark blue or black, it is untreatable. Observing the fingerprint on the tiger's mouth (the radial side of the thumb): if the curve bends inward, it is treatable; if it extends outward, it is untreatable. Initially, treatment should focus on dispersing wind pathogens, relieving convulsions, resolving phlegm, and regulating qi, along with applying paste to the fontanelle. For severe cases, Zhu Yin Wan (Cinnabar-Silver Pill) may be used to purge. If the face is blue with clenched fists and convulsions, use Bao Ming Dan (Life-Preserving Pill, which treats both acute and chronic convulsions), Gouteng San (Uncaria Powder, see Chronic Convulsion section), or Quanxie San (Scorpion Powder, see Deviated Mouth and Lockjaw section). Generally, pediatric zang-fu organs are fragile; preparations containing mercury powder (Yin Fen) for heavy sedation should not be used lightly. In such emergencies, take pig milk, finely grind small amounts of Niuhuang (Calculus Bovis) and Shexiang (Moschus), mix, and apply to the mouth. Simultaneously administer Daochi San (Guide Out Red Powder) to drain the son of the Liver (i.e., the Heart), and recovery will ensue.
The *Shibi Jing* song states: Within one month before birth, maternal fright causes illness manifesting at birth. Rigidity of the waist, lip pursing upon crying, blue face, clenched fists, retracted cheeks, closed eyes, trismus, and tight sinews/vessels. (During conception, if Yin qi is weak, the fetus is easily frightened and drops, leading to severe muscle wasting; if Yang qi is weak, the fetus is hard to frighten but results in small limbs and thin muscles, neither living out their natural lifespan. If full-term due to fright dropping, presenting with lip pursing, swollen cheeks/face, nasal congestion, and trismus, do not treat as convulsion; first induce slight sweating, then treat convulsion and regulate qi. The nursing mother takes qi-regulating herbs, and the infant applies paste to the fontanelle.) Calling a thousand times yields no eye opening; retreating wind-phlegm is the treatment key. Once phlegm is gone, wind-pathogen naturally retreats. (Failure to treat properly leads to closed eyes; treating convulsion first causes vomiting/diarrhea; in autumn/summer, Spleen Wind [Pi Feng] will surely occur. At initial onset, do not treat as Spleen Wind, lest failure to sweat produce other syndromes. Use Fengsui Wuxi Wan, followed by Yin Wan, both found in the Acute/Chronic Convulsion sections.) [Qian Yi] Convulsions occurring during the day: True cases die after two or three occurrences; false cases, though frequent, are not severe. True cases involve internal generation of convulsive epilepsy; false cases involve external injury by wind-cold. Because blood and qi are not yet solid, they cannot withstand, hence convulsions occur. To identify false cases: if breath exhaled from the mouth is hot, treatment can use dispersion methods, primarily Daqing Gao (Great Blue Ointment), and fontanelle application can be used. [Tian] For infants under one month old with convulsions, urgently take pig milk, finely grind small amounts of Chen Sha (Cinnabar) and Niuhuang (Calculus Bovis), mix, and apply to the mouth; efficacy is miraculous. The nursing mother takes Fangfeng Tongsheng San (Saposhnikovia Decoction for Unblocking the Master) for three doses, and the convulsions will subside. [Dou] For infants under one month with convulsions resembling stroke and near-death state, grind Chen Sha (Cinnabar) thickly in fresh well water, apply the juice to the five centers (palms and soles); this is most effective. [Tang] For treating fetal epilepsy and convulsive wind, Quanxie (Whole Scorpion) with head and tail intact can be used. Wrap in fresh Bohe Ye (Mentha Leaf), bind with hemp thread, roast over fire until dry, and powder. Separately grind Sheng Zhusha (Raw Cinnabar) and Shexiang (Moschus) slightly. Take with Maidong Tang (Ophiopogon Decoction). [Xue Ji] A child suffered from fetal convulsion, unresponsive to various medicines. Use Zihewche (Placenta Hominis), ground into mud-like paste. Take approximately one qian (3-5g), emulsified and taken. Additionally, have the mother take Shiquan Dabu Tang (Decoction for Tonifying All Ten Deficiencies) plus Gouteng Gou (Uncaria Ramulus cum Uncis) and Lulu (Radix Euphorbiae Humifusae). After more than two months, attacks gradually lightened; after over a year, attacks became sparse; after another year, no further attacks occurred. After eruptive disease (smallpox/chickenpox), relapse occurred. Take Zihewche (Placenta Hominis), ground into paste, mixed with Nuomifen (Glutinous Rice Flour) to make pills the size of adzuki beans. Take 100 pills each time, delivered with milk. After taking two placentas, complete recovery was achieved. After marriage, relapse occurred again. Still use the previous pills plus Shiquan Dabu Tang, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) added with Danggui (Angelicae Sinensis Radix), Huangqi (Astragali Radix), Rougui (Cinnamon Bark), and Wuweizi (Schisandrae Fructus). After a year, relieved by avoiding sexual activity, recovered. Later, due to labor fatigue, recurrence happened. Again used previous pills and Shiquan Dabu Tang etc., without effect. Used large-dose Dushen Tang (Single Ginseng Decoction), consumed several jin (hundreds of grams), then attacks slowed slightly. Then used Renshen (Ginseng Radix) 2 liang (60g) and Fuzi (Aconiti Lateralis Radix Praeparata) 1 qian (3g). After several doses, stopped abruptly. Continued previous medicine, occasionally using Dushen Tang, and recovered. Another child with fetal convulsion recovered using Zihewche Wan (Placenta Pill), Shiquan Dabu Tang, and Gouteng Gao (Uncaria Ointment). Relapsed after marriage. Used large-dose Dushen Tang, Liuwei Dihuang Wan added with Wuweizi, Huangqi, Danggui. Decocted and taken for half a year, attacks lightened slightly. No recurrence after a year. Subsequent recurrences triggered by labor or anger were cured immediately using previous medicines. Another case: relapse after recovering from Shanghan (Cold Damage), presenting with all deficiency symptoms, indescribable. Used two Zihewche (Placentas), decocted with Dushen (Single Ginseng), consumed over ten jin, and recovered. Later suffered from Shangfeng (Wind Invasion) cough, dry throat, and internal heat. Used Liuwei Dihuang Wan ingredients added with Wuweizi, decocted and taken, plus Shiquan Dabu Tang, and recovered.
