Tian Diao (Heavenly Hook) disease, upon onset, presents with the head and eyes looking upward. There is palpitation and high fever. The eyes are retracted backward (opisthotonos). Tears flow but do not move freely. The hands and feet undergo clonic spasms. The child cries sorrowfully at times, as if possessed by spirits.
In severe cases, the fingernails and toenails turn cyanotic. This is generally caused by the wet nurse consuming rich and greasy foods, leading to accumulated toxins in the stomach, which generates heat in the infant's Heart and Lungs. Heat-phlegm stagnates and obstructs, causing the pathology, exacerbated by external wind pathogen invasion. The treatment principle should be to resolve and disperse the pathogenic factors, using Gouteng Yin (Uncaria Decoction). If the child has a sallow complexion, Qi deficiency, and panting, use Sijunzi Tang (Four Gentlemen Decoction) added with Quanye (Whole Scorpion Tail).
Nei Diao (Inner Hook) disease manifests as abdominal pain often accompanied by panting. The lips are dark, and the scrotum is swollen. The body is curved or bent backward. The corners of the eyes are red. If there is a sensation of internal spasming in the five viscera with severe pain and wild screaming, or if diarrhea occurs with legs curled up, once the internal syndrome appears, the external syndrome will also manifest. It is extremely difficult to regulate. For the internal syndrome, use Wuxie Liujunzi Wan (Scorpion and Six Gentlemen Pill). For the external syndrome, use Gouteng Yin (Uncaria Decoction). Treatment is feasible if the child can take milk and food. If the cause is the wet nurse drinking strong alcohol or eating rich/greasy foods, leading to accumulated toxins in the stomach, use Jiawei Qingwei San (Modified Stomach-Clearing Powder). If the cause is the wet nurse's depression and anger, leading to accumulated heat in the Liver, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder), with a slight addition of Loulu (Radix Euphorbiae Humifusae). Both mother and child should take the medication.
Xiao'er Panchang Qi (Infant's Intestinal Coil Qi) involves pain where the waist bends forward, the child cries without tears, and sweat appears on the forehead. This is all due to wind pathogen attacking the Liver Meridian. Since the Liver and Kidney reside in the lower jiao, pain causes the waist to bend. Crying without tears occurs because wind-dryness consumes body fluids, hence no tears. Sweat on the forehead occurs because wind-water assists Heart fire. Mouth closure and cold feet indicate that Spleen Qi fails to nourish the limbs. Diarrhea with undigested food indicates Liver Wood overacting on Spleen Earth. Use Dingxiang Lianshi Wan (Clove and Melia Pill). For those bending their waist and crying without tears, administer with salt water. For those with sweat on the forehead, administer with Congchi Tang (Scallion and Fermented Soybean Decoction). For those with mouth closure and cold feet, administer with Renshen Tang (Ginseng Decoction). If the face is red, lips are cracked, urination is obstructed, and the lower abdomen is distended and painful, this is Heat in the Small Intestine. Use Jiawei Tongxin San (Modified Heart-Unblocking Powder). If pain persists, decoct scallion water to wash and rub the abdomen. Apply hot scallions to moxa/press the umbilical and abdominal area. After a while, urine is discharged, and pain ceases. If caused by the wet nurse's dietary stagnation, use Baohe Wan (Preserve Harmony Pill). If caused by anger stirring Liver Fire, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder). Both mother and child should take the medication.
Infant Wind-Strike presents with nasal congestion and fever. Or phlegm obstruction inducing spasms. This is often caused by the wet nurse blowing air from her nose onto the fontanelle. Simply administer Xingxing San (Awakening Powder). Alternatively, take seven scallion heads and one slice of ginger, finely chop and mash them, spread on paper, hold between palms. When warm, apply to the fontanelle. The pathogen will then dissipate. Remove the scallion, then use a piece of silk about one cun long, coated with flour paste, and reapply to the fontanelle. This prevents future wind-strike. Other methods for dispersing pathogens with herbs are no different from adults. However, because the infant's Spleen and Stomach are weak, symptoms of cold hands and feet are common. Therefore, the treatment method focuses primarily on protecting the Spleen and Stomach.
