Tong: Vomiting and diarrhea occurring simultaneously, chest and heart feeling stifled and blocked. Qi stagnation and dampness obstructing each other, clear and turbid qi failing to ascend and descend respectively, fear of internal closure.
Prescription: Zhi Banxia (Prepared Pinellia Rhizome), Xinhui Pi (Xinhui Tangerine Peel), Chao Zhuru (Stir-fried Bamboo Shavings), Chi Ling (Red Poria) and Bai Ling (White Poria), Nen Sugen (Tender Perilla Stem), Houpu (Magnolia Bark), Dafupi (Betel Nut Rind), Chen Xiangru (Aged Mosla Herb), Xian Foshou (Fresh Bergamot Fruit), Wei Muxiang (Baked Aucklandia Root), Fulonggan (Dragon's Liver Earth), Yushu Dan (Jade Pivot Pill).
Chen (Left pulse): After painful diarrhea, vomiting followed, discomfort in the Middle Jiao (Middle Burner). Pulse is deep and fine. This is due to internal obstruction by cold-dampness.
Prescription: Houpu (Magnolia Bark), Zhishi (Immature Bitter Orange), Dafupi (Betel Nut Rind), Guangpi (Broad-leaved Mandarin Peel), Nanzha Tan (Charred Hawthorn Fruit), Zhi Banxia (Prepared Pinellia Rhizome), Kourén (Cardamom Seed), Chi Ling (Red Poria) and Bai Ling (White Poria), Xian Foshou (Fresh Bergamot Fruit).
Yu (Left pulse): Entered the examination hall while ill, pathogenic factors not fully cleared; yesterday ate rice twice more and consumed three cold persimmons. Cold and food retention obstructing each other, causing rebellion and chaos of Yang qi in the chest, leading to sudden disorder of Yin and Yang qi. Watery diarrhea occurred first, followed by vomiting. Yang qi is closed and depressed, limbs are cold (Jue), pulse is hidden (Fu), sweating does not warm the body, eyes are sunken, voice is low. Frequent thirst with desire to drink, epigastric region blocked, severe pain in the chest. Middle Yang has lost its power to rotate and transform, with risk of internal closure and external collapse. Prescribed Huanglian Tang (Coptis Decoction) to harmonize Yin and Yang in the Stomach, combined with aromatic transformation to open the depression.

Second Visit: Used Zhang Zhongjing's Huanglian Tang (Coptis Decoction) to harmonize Yin and Yang in the Stomach, combined with aromatic transformation to open Qi mechanism. All six pulses rose, limb coldness turned warm, chest pain stopped, diarrhea decreased.
Although the condition shows signs of improvement, how can damp-heat be quickly resolved? Thus, dampness transforms into heat, consuming and scorching Yin fluids. Tongue coating is dry and yellow, lacking fluids entirely. Frequent thirst with desire to drink, occasional hiccups, complete absence of urination, consciousness is confused and heavy. Extremely dangerous. Tentatively prescribed a combination of pungent, salty, and cold herbs, combined with aromatic opening.
Prescription: Sheng Shigao (Raw Gypsum Fibrosum) 1 liang, Huashi (Talcum) 4 qian, Guanzi Gui (Official Cinnamon Bark) 0.6 fen, Fuling (Poria) 3 qian, Hanshuishi (Calcitum) 3 qian, Zhuling (Polyporus) 2 qian, Yuzhu (Atractylodes Macrocephala Rhizome) 1.5 qian, Zexie (Alisma Rhizome) 1.5 qian, Xian Hegeng (Fresh Lotus Stem Midrib) 1 chi, Zixue Dan (Purple Snow Pill) 0.6 fen.
Yao (Left pulse): External cold binding the interior heat, causing mutual obstruction of cold and heat, disturbing the clear and turbid qi of the Sanjiao (Triple Burner). Vomiting and diarrhea occurring simultaneously, epigastric region blocked. Suitable to use bitter-pungent methods to open the Sanjiao (Triple Burner). However, Cholera-like illness (Huoluan) during this season cannot be discussed alongside ordinary diseases.
Prescription: Zhi Banxia (Prepared Pinellia Rhizome) 2 qian, Chuan Houpu (Magnolia Bark) 1 qian, Dan Wuyu (Evodia Fruit) 0.3 qian, Chuan Yalian (Coptis Rhizome) 0.5 qian, Yun Fuling (Poria) 3 qian, Wancansha (Silkworm Frass) 3 qian, Huoxiang (Patchouli Herb) 3 qian, Chao Zhuru (Stir-fried Bamboo Shavings) 1.5 qian, Baikourén (White Cardamom Seed) 0.5 fen, Guangpi (Broad-leaved Mandarin Peel) 1 qian, Xiangru (Mosla Herb) 1 qian, Taiyi Dan (Great One Pill) 0.5 fen.
Tichen: Food retention internally, mixed sensation of cold and heat, causing interference between clear and turbid qi, blockage of the Sanjiao (Triple Burner). Initially upper vomiting and lower diarrhea, now unable to vomit out, unable to defecate smoothly, abdomen feels board-like and stagnant, full and painful upon pressure, belching smells like rotting eggs, pulse is hidden, limbs are cold, finger whorls are contracted, eyes are sunken, clear and turbid are mixed. Dampness and food retention obstructing, although vomiting and diarrhea have slightly paused, turbid accumulation is even more blocked, Qi pathways are not unobstructed, Yang qi cannot distribute. Extremely dangerous. Tentatively discussed with Brother Xianqing using bitter-pungent methods to open and unblock, descending and draining turbid accumulation. Requesting high-level experts to discuss and prescribe.
Prescription: Chuan Yalian (Coptis Rhizome stir-fried with Ginger Juice) 0.7 fen, Shang Chuan Houpu (Top-grade Magnolia Bark) 1 qian, Zhishi (Immature Bitter Orange) 1.5 qian, Muxiang (Aucklandia Root) 0.5 qian, Ju Pi (Tangerine Peel) 1 qian, Dan Wuyu (Evodia Fruit) 0.7 fen, Jiang Banxia (Ginger-processed Pinellia Rhizome) 2 qian, Baikourén (White Cardamom Seed) 0.5 fen, Dafupi (Betel Nut Rind) 2 qian, Fan Zhiqu (Medicated Leaven) 2 qian, Beijji Wan (Emergency Rescue Pill) 3 fen taken with Foshou decoction.
