臂取之。又曰肘尽处是穴)。小儿湿地上坐,或有蚯蚓吹其卵,肿大而垂者。以盐汤浸洗之,盖盐能杀蚯蚓毒也;

Chen Feixia stated: Infantile night crying presents with several syndromes, including those caused by cold in the Zang organs (Zang organs), heat in the Heart, unsettled Shen (Spirit/Mind), and obstinate crying. The cold and heat patterns differ among them, requiring careful differentiation. In cases of cold in the Zang organs, Yin is predominant at night; when night arrives, extreme Yin generates dryness/heat, while severe cold causes abdominal pain. Crying stops when the abdomen is pressed with the hand but resumes when the hand is removed. External symptoms include a cyan complexion, cold hands and feet, refusal to suckle milk, and incessant night crying. This is treated with Jiawei Danggui San (Modified Angelica Decoction). For crying due to Heart heat and restlessness, symptoms include a red face and red tongue, or a white and rough tongue coating. Crying subsides slightly without lamplight but worsens with it. Daochi San (Guide Out Red Powder) plus Maidong (Ophiopogon Tuber) and Dengxincao (Juncus Effusus) are indicated; for severe cases, add Huanglian (Coptis Rhizome) and Longdancao (Gentiana Root). For crying due to an unsettled Shen, symptoms include palpitations during sleep, loud crying while clinging to the mother, and a purplish-black complexion. This arises from Shen deficiency and palpitations; Anshen Wan (Calming Spirit Pill) is used to stabilize the mind. Night crying occurring after vomiting/diarrhea or major illness is also due to insufficiency of Heart Blood, treated similarly.

Tu Weisheng stated: Night belongs to Yin, so cold pain intensifies at night; how could there be "extreme Yin generating dryness"? However, to identify cold pain in infants, observe if the eyebrows wrinkle during crying; this indicates cold pain.
The *Classic of Acupuncture and Moxibustion* states: When pathogenic factors lodge in the collaterals of the Foot Jueyin (Liver) meridian, it causes sudden hernia and acute pain. It also states: Disease presenting as lower abdominal pain with inability to urinate or defecate is named Hernia, caused by Cold. Chen Feixia stated: Therefore, Qi Hernia is formed by the accumulation of Cold Pathogens. Internally, there is colicky pain around the umbilicus and abdomen; externally, the scrotal sacs swell. This belongs exclusively to the Liver Meridian and has no relation to the Kidney. Since the Liver governs anger, and children have irritable temperaments, they often contract this condition through excessive crying. This qi stagnation within is termed Qi Hernia. Treatment should promote Qi movement and resolve depression. Initially, Chai Ling Tang (Bupleurum and Poria Decoction) is used to disperse ascending pathogens; subsequently, Jiawei Erchen Tang (Modified Two-Aged Phlegm Decoction) or Muxiang Neixiao Wan (Costus Internal-Dissolving Pill) may be used. If contracted from sitting on damp ground for long periods, this is Cold Qi entering the abdomen, termed Han Hernia (Cold Hernia). Treatment should warm the middle and dispel cold, using Jiawei Danggui San or Zhuyu Neixiao Wan (Evodia Internal-Dissolving Pill). Cases with swelling but no pain are caused by Dampness. Although the testicles are swollen, there is no heat sensation, and the abdomen does not hurt; treatment should move Dampness and reduce swelling, using Jiawei Shoubing Wan (Modified Guarding Disease Pill). Infants with chronic Hernia who experience episodes every six months or year will present with visible swelling connecting externally to the testicles and internally to the lower abdomen, forming a hard, rod-like mass about five to six cun long, accompanied by obstruction of urination and defecation. This is treated with Danggui Muxiang Tang (Angelica and Costus Decoction). If the external genitalia are swollen and painless, persisting for years without resolution, early treatment is essential to prevent becoming a chronic incurable disease. Use Zhuyu Neixiao Wan added with Qianniuzi (Seed of Morning Glory), half raw and half stir-fried, using only the finest powder. Additionally, moxa the Zhangmen acupoint beside the umbilicus for significant effect. Acupoint location method: Align the transverse crease of the fifth finger's proximal interphalangeal joint of the child's palm with the center of the umbilicus (*Great Compendium of Acupuncture and Moxibustion* states: locate at the end of the costal margin, two cun above the umbilicus, six cun laterally; lie on side, flex upper leg, extend lower leg, raise arm to locate. Also states: the point is where the elbow ends). If a child sits on wet ground and earthworms blow air onto the scrotum causing swelling and垂ding, soak and wash with salt water, as salt kills earthworm toxins; or boil Cangzhu (Atractylodes Rhizome) in water with a little salt and wash, which is effective. If the infantile scrotum develops sores and ulceration, termed Tuonang (Sloughing Scrotum), apply powdered Ziye (Perilla Leaf) and wrap with Lotus Leaf, or use fresh Lotus Leaf warmed by fire and wrapped. Even if the testicles are exposed, it can still be cured.
If the external genitalia smell foul and are frequently moist and itchy, use Chaihu Longdan Tang (Bupleurum and Gentiana Decoction). For severe, unbearable itching, wash with decoction of Hujiao (Pepper).
