【歌】疽发恶深其状小,浅而大者是为痈。要看部位分虚实,薄处尤当大补荣。

[Verse] Carbuncle (Ju, deep-seated sore) presents with severe pathology and a small initial appearance; Boil (Yong, superficial abscess) is shallow but extensive. One must distinguish deficiency and excess based on the location; in areas of thin flesh and blood, it is crucial to greatly tonify Ying-blood (nutritive blood).
[Discussion] The *Yuan Bing Shi* states: "Yong" refers to lesions that are superficial and large; "Ju" refers to those that are deep and malignant. Excessive heat causing blood stasis leads to Yong or Ju. Danxi stated: When treating Yong and Ju, one must observe the Six Yang meridians and Six Yin meridians. Some meridians have abundant Qi but scarce Blood, some have abundant Blood but scarce Qi, and some have both abundant Qi and Blood; they cannot be generalized. If occurring in critical areas or near weak/vulnerable regions, such as on the Shaoyin or Jueyin meridians, because these meridians have abundant Qi but scarce Blood, muscle growth may be difficult, and wound closure delayed, leading to severe harm. Therefore, one should avoid using detoxifying and purgative drugs to attack the Blood of the Yin aspect; instead, one should greatly tonify Qi and Blood to achieve success. For example, when middle-aged individuals develop Yong or Ju, upon feeling pain, if pulse and symptoms indicate weakness, one should immediately administer blood and Qi tonics to ensure a favorable outcome. Hejian stated: If swelling protrudes externally, the base is not deep, signs are superficial, and the pulse is floating, the disease is in the skin and flesh. Unless excessive Qi invades internally, urgent internal support (Tuo Li) is needed to save the interior. Use Fujian San to drain dampness and disperse stagnation, harmonizing Stomach Qi. If the condition persists, administer another half dose. If there is constipation and irritability, give a small amount of Huanglian Tang. If there is mild diarrhea, reduced irritability, and slightly subsided heat, resume administering half a dose of Fujian San. This allows Ying and Wei (defensive) Qi to circulate, preventing pathogenic factors from damaging the interior. Laypeople often use Paion Neibu Shixuan San; this is acceptable for minor sores or in winter, but if used for ulcerated wounds in summer, the acrid-hot warming properties of Guizhi (Cinnamon Twig) and Fuzi (Aconite Root), aided by Fangfeng (Siler Root) and Baizhi (Angelica Dahurica), make me fear that even with Renshen (Ginseng) and Huangqi (Astragalus), reliance on them is precarious. It is better to use Fujian San. Scholars should exercise judgment when applying these, avoiding errors.
**Tuoli Fujian San (Supporting Interior and Releasing Exterior Decoction)**
Fangfeng (3 liang, remove rhizome), Digupi (2 liang), Baifuling (2 liang), Huangqin (2 liang), Baishao Yao (2 liang), Renshen (2 liang, remove rhizome), Baizhu (2 liang, stir-fried), Huangqi (2 liang, honey-fried), Rougui (2 liang), Gancao (2 liang), Fangji (2 liang), Danggui (2 liang).
First, decoct 1 jin of Cangzhu (Atractylodes Rhizome) in 5 sheng of water until reduced to 3 sheng. Remove the Cangzhu residue, add the aforementioned herbs, and decoct until 4 zhan remain. Divide into four doses. The Cangzhu residue can be decocted again.
Back sores developing at the "Pi Du" area, below the shoulders and above the spine, caused by dietary toxins, spanning one chi wide and one cun deep. Even if ulceration reaches the bone, death does not occur unless the membrane is penetrated. Urgently treat the toxin in the Pi Du area. Internally, support the interior and protect the Heart; externally, draw out toxin and dissipate swelling, attacking from both sides. If it develops in the right Pi, fear the toxin rushing to the Heart; medication must intercept the toxin to prevent cardiac invasion. Generalized back swelling is incurable.
Honeycomb Fa (Beehive Sore) occurs exactly at the center of the spine, with the head upward. This is highly unfavorable and represents a reverse syndrome. Treatment relies entirely on supporting the interior, generating flesh, and stabilizing pain, fearing toxin invasion of the Heart which would be difficult to treat, as the Heart Fire has not yet erupted.
Scattering Flowing Injection Fa: This toxin travels with wind-heat. Urgently use wind-dispersing and heat-clearing agents to calm the Qi. Cases flowing to the hands, feet, legs, and knees are invariably fatal.
Back sores with small ends and scattered sides are caused by diet affecting Qi and Food, combined with Yin deficiency. Qi deficiency causes scattering, resulting in wide openings. Urgently take internal resolution medicines; Yang tonification is also appropriate.
Lung Yong develops between the left arm and shoulder. Initially, apply fire moxibustion to break it, then use Zhuding Tang to induce sweating and disperse it.
Right Shoulder-Draping Sore: If arising on the bone at movable joints, it is treatable. If spreading to the left shoulder, it is difficult to treat. Left Shoulder-Draping Sore: If arising on the bone at movable joints, it is treatable. If spreading to the right shoulder, it is difficult to treat. Both conditions use Xionghuang San (Realgar Powder) mixed with cotton wool, baked dry, powdered, and applied with sesame oil.
Heart-Facing Sore: Caused by excessive Heart Fire and heat convergence. The toxin is vigorous and spreading; urgently use Heart Fire-draining and guiding medications.
Shenshu Fa: Caused by dampness, anger, and hot wine consumption, injuring the inner Kidney, with toxin flowing to Shenshu. Resolve the inner Kidney toxin. If Yin-type Fa injures the Kidney membrane, it is incurable. Bilateral Shenshu Fa: Yang-type external presentation is treatable. Yin-type injuring the Kidney membrane or with thin pus indicates death.
Chronic non-healing ulcers result from deficient and exhausted Kidney Water sinking downward. Take Kushen Wan (Sophora Pill); externally apply Gezhi Gao (Paper-Separated Plaster) and Hongyu San (Red Jade Powder).
Red Thread Sore: Often unrecognized and lacking treatment methods, it kills rapidly. It arises between hands and feet, presenting with yellow blisters that suddenly turn purple-black, followed by a red thread extending upwards along the vessels. If it reaches the heart or abdomen, it causes delirium and death. If two or three red threads appear, pierce the red thread horizontally with a needle, prick along its path to release blood, and cover with a plaster to heal. This condition arises from unregulated emotions causing Qi and Blood rebellion.
Internal Gan Sore: Arises in the upper palate. Initially resembles a lotus hemorrhoid, small-rooted and hanging. When enlarged, cut the root with a hook knife and cauterize with hot iron to stop bleeding. Apply powder of Xionghuang (Realgar), Qingfen (Calomel), Fenshuang (White Lead Carbonate), Baizhi (Angelica Dahurica), and Bailian (Lily Bulb). Support the jaw with a locust wood pillow to keep the mouth open. After one to two hours, the scar fixes and the mouth opens naturally. Pus appears the next day; apply Shengji San (Generate Flesh Powder) and protect with Ruxiang Gao (Frankincense Plaster). Without jaw support, saliva interferes with medication application due to Qi exhaustion.
Bian Yong (Genital Abscess): Also known as Blood Hernia or colloquially as Bian Du (Genital Swelling), named for its inconvenient location. It involves the Foot Jueyin Liver meridian and Chong, Ren, and Du vessels, which are lateral branches of the Liver and pathways for Qi and Blood circulation. Obstruction causes abscess; purge with Yuzhu San or Taoren Chengqi Tang.
Ru Yong (Breast Abscess): Caused by anger obstruction, depression blockage, and rich food fermentation. The breast is traversed by the Yangming Meridian; the nipple belongs to the Jueyin Meridian. Jueyin Qi stagnation prevents milk flow; Yangming Heat boils, causing suppuration. It can also result from phlegm retention during lactation, warm breath from the infant blowing into the nipple, forming nodules. Initially, endure pain, massage until soft, and suckle to clear ducts for spontaneous resolution; otherwise, it becomes an abscess. Use Qingpi (Green Tangerine Peel) to disperse Jueyin stagnation; Shigao (Gypsum) to clear Yangming heat; Danggui (Angelica Sinensis), Chuanxiong (Sichuan Lovage Rhizome), and Gancao Jie (Licorice Knot) to move turbid blood; Gualou (Trichosanthes Fruit), Moyao (Myrrh), Jiuye (Tangerine Leaf), Zaojiaoci (Gleditsia Spine), and Jinyinhua (Honeysuckle) to reduce swelling and guide out toxin; add a little wine to promote drug action; moxibustion on two or three points is particularly effective. Women suffering marital or familial discord accumulate sorrow and anger, forming hidden nodules like chess pieces, painless and itchless, which after decades become excavated cavities known as Ru Ai (Breast Cancer), shaped like rocky caves, difficult to treat.
Honeycomb Fa on chest/breast: Due to excessive Heart Heat. Urgently drain Heart Fire; delay is fatal.
Nape Honeycomb Fa: Requires urgent rescue. Treatable if initially red, swollen, and painful. Phlegm-type Fa or those flowing to both shoulders are difficult to treat.
Abscesses under both hypochondria: Absolutely do not use Yang-tonifying drugs. Deficiency patients cannot tolerate heat-inducing agents; extreme deficiency easily damages periosteum.
Hairline Fa on both sides: Occurs at the posterior hairline on both sides of the neck, rising like nodules. Urgently remove the pathological root. Brain-Heart Fa: Heat attacks the brain periphery, causing red, swollen, hard tissue connected to ears and neck, with chills, fever, and pain. If not treated urgently, toxin enters flesh and blood, rotting into pus, which is incurable.
Posterior Head Fa: Named Tian Ju (Early Death Carbuncle). Large, purple-black. If not treated urgently, toxin enters Yuanye (deep axillary region), damages Ren Mai anteriorly, and steams Liver and Lung internally; death occurs within ten days. Urgently use Huatan Xiaozhong Tuoli San (Phlegm-Resolving, Swelling-Reducing, Interior-Supporting Powder) and Neituo Qianjin San (Interior-Supporting Thousand Gold Powder).
Behind-Ear Fa: Eruption one inch three fen behind the ear is fatal, named Rui Du (Sharp Toxin), incurable.
Chest Fa: Named Jing Ju (Well Carbuncle). Bean-sized. Within three to four days, if not treated early, toxin enters abdomen; death in ten days. Urgently take Neigu Qingxin San (Interior-Stabilizing, Heart-Clearing Powder). External eruption is treatable; internal eruption is invariably fatal.
Nine Ju types: Lung Ju in Muzhong; Liver Ju at Qimen; Kidney Ju at Zhongwan; Spleen Ju at Zhangmen; Stomach Ju at Benmen; Large Intestine Ju at Tianshu; Sanjiao Ju at Dantian; Small Intestine Ju at Guanyuan. If Qi perforates outward, it is treatable. If erupting inward, damaging membranes, with pus draining through the Large Intestine, it is incurable.
External Genital Yin Fa: Named Shen Yong or Xuanyong. Swelling and pain on the scrotum. Caused by Cold-Damp pathogens invading Bladder, Kidney, and Foot Jueyin Liver meridians, stagnating Qi and Blood in Yin meridians. Take Tuoli San (Supporting Interior Powder) plus Cheqianzi (Plantago Seed), Mutong (Akebia Stem), Danzhuye (Bamboo Leaf), Qianniu (Morning Glory Seed), and Heshouwu (Polygonum Multiflorum). Externally apply Neixiao San (Internal Resolution Powder) and Shengji Dingtong San (Generate Flesh, Stop Pain Powder). Even if the testicle is lost via external ulceration, it is harmless.
Lower Chancre: Secret One-Ingredient Thousand Gold Powder is instantly effective. Externally, stir-fry scallion white until hot and compress; reheat if cool, compress 3-5 times, then apply disinfecting and swelling-reducing medicine including Dahuang (Rhubarb), Muguaizi (Momordica Seed), Nanxing (Arisaema), and Caowu (Aconite Root). Upon ulceration, apply Shengji San.
Human-Face Sore: Caused by karmic retribution; resolve through good deeds, then take Liuqi Yin (Flowing Qi Drink). If persistent, take Kushen Wan or apply Fumu (Fritillaria) powder.
