Various diseases during pregnancy must be carefully understood. Ezu (pregnancy nausea and vomiting) should be inferred from phlegm-fire. When fetal qi rushes to the chest and heart causing distension and oppression, Qin-Zhu Tang (Scutellaria and Atractylodes Decoction) combined with fried Gardenia Fruit is used. Vaginal bleeding during pregnancy is named Tainlou (fetal leakage), often caused by blood-heat or qi-deficiency. For Taidong (fetal movement discomfort), Ligusticum Chuanxiong Rhizome, Angelica Sinensis Radix, and Donkey-hide Gelatin with Mugwort are used; securing the fetus and regulating qi should not be delayed. Zilin (pregnancy dysuria) requires Anrong San (Anrong Powder). Taishui (pregnancy edema with ascites) should be treated with Carp Soup. Generalized edema resembling water accumulation can be relieved by boiling and taking Stephania Tetrandra Saponaria. Edema of the dorsum pedis without other systemic symptoms is treated with Areca Peel for Zhoujiao (wrinkled foot edema). Severe irritability after the fifth month is called Zifan (pregnancy irritability), treated with Anemarrhena Rhizome and Ophiopogon Tubers. Abdominal pain in pregnancy must be differentiated as deficiency or excess; cold patterns use warming methods, heat patterns use clearing methods. Zi Xian (pregnancy eclampsia/seizures) urgently requires Antelope Horn. Er Yun (pregnancy syncope) responds remarkably to Ligusticum Chuanxiong Rhizome, Angelica Sinensis Radix, and Schizonepeta Spike. Diarrhea uses Amomum Fructus and Atractylodes Macrocephala Rhizome added to Er Bai San; for heat-type diarrhea, Scutellaria Baicalensis Root and Atractylodes Macrocephala Rhizome should be doubled. Wind-cold common cold is treated with Ginseng and Perilla Decoction; if unresolved, Huanglong Tang should be used without hesitation. Chest fullness adds Immature Bitter Orange and Platycodon Root to the base formula; heat with no sweating is driven away by Kudzu Root. Severe interior heat uses Zaotai San; extreme heat with delirium requires detoxification. Slow pulse is treated with Four Reversal Decoction and Spleen-Tonifying Decoction; heat-type diarrhea is cleared by Bupleurum Root, Scutellaria Baicalensis Root, and Pinellia Tuber. Sweating, vomiting, purging, and warming therapies must be applied cautiously, prioritizing fetal security without error. Qingpi Yin (Clear the Spleen Drink) for malaria must exclude Pinellia Tuber; Six Beast Decoction expels pathogens while tonifying spleen qi. Dysentery is effectively treated with Coptis and Cinnamon Powder; Weifeng Tang stops dysentery and tonifies deficiency. Wind-cold cough uses Ginseng and Perilla Decoction, excluding Pinellia Tuber and adding Scutellaria Baicalensis Root, Apricot Kernel, and White Bark. Constipation is treated with Areca Peel Powder; difficult urination uses Okra Seed. Palpitations, trance-like states, heart fright, qi stagnation, and shouting are dispelled by Dasheng San. Heart pain is settled by Huolong Tang; lower back pain has no rival to Tongqi San. Dasheng San can bind the fetus, but should only be taken in the ninth month.
[Dissertation]
For women during pregnancy, it is crucial that the Chong and Ren meridians are vigorous and yuan qi (original qi) is sufficient. This ensures normal appetite, robust health, un-diminished luster, and freedom from disease invasion. Thus, menstruation occurs on time, allowing conception. With abundant blood and qi, ten months can be completed safely, delivery proceeds without worry, and mother and child remain secure—what disease could there be? If blood and qi are insufficient, and the Chong and Ren meridians are weak, menstruation will be delayed, making conception impossible. Even if pregnancy occurs, the uterine gate and womb are deficient and cold, resulting in unstable conception. Or, if the Chong and Ren meridians are deficient and accompanied by heat, mild cases cause fetal movement and instability, while severe cases result in miscarriage at three, five, or seven months. Miscarriages occurring in yang months happen because fire consumes matter. Additionally, external invasion by the six climatic pathogens; internal causes from the seven emotions; excessive diet damaging the middle jiao (spleen and stomach); or excessive sexual activity damaging yuan qi internally—all such conditions rarely fail to cause disease. Therefore, for all pregnancy-related diseases, securing the fetus must come first, followed by treating the disease. Once the disease is eliminated, the fetus becomes secure; once the fetus is secured, the disease heals itself.
[Pulse Diagnosis]
Yin pulses beat vigorously below, while Yang pulses distinguish themselves above; harmonious blood and qi indicate pregnancy. The pulse of the Hand Shaoyin (Heart) meridian moves significantly; if the Chi position remains unbroken upon pressure, this indicates pregnancy.
Shaoyin belongs to the Heart, which governs blood vessels. The Kidney is the gate of the uterus, and its pulse corresponds to the Chi position. Alternatively, the Cun position may be faint, Guan slippery, and Chi rapid, flowing smoothly like a sparrow pecking. Or, examining the three positions, there may be an intermittent stop between floating and sinking, or the pulse may be level yet empty; one should inquire about menstruation. If a woman has illness but lacks pathogenic pulses, this is pregnancy rather than disease, hence the cessation of menstruation.
Differentiation of male and female fetuses is determined by left and right pulses: a rapid left pulse indicates a male, a rapid right pulse indicates a female. Deep and solid on the left, floating and large on the right—right female, left male—can be predicted in advance. A pulse deviating from the standard reaching six beats per breath cycle, deep, fine, and slippery, with paroxysmal pain connecting to the waist, indicates a viable fetus detaching. A blood mass with a string-tight and large pulse suggests life; a deficient, small, and weak pulse indicates death. Half-abortion with leakage is indicated by a Ge pulse; weakness implies blood depletion and imminent danger.
At six or seven months of pregnancy, a pulse that is substantial, large, firm, strong, wiry, and urgent indicates survival; if deep and fine, it indicates death.
Ezu (Pregnancy Nausea)
Since the second or third month of pregnancy, vomiting, inability to eat, emptiness in the heart, and irritability are named Ezu, primarily treated with Ginseng and Tangerine Peel Decoction. Danxi stated: In obese individuals, Ezu is due to phlegm; treat with Er Chen Tang plus Scutellaria Baicalensis Root, Atractylodes Macrocephala Rhizome, Bamboo Shavings, and Fritillaria Bulb, excluding Pinellia Tuber. In thin individuals, it is due to fire; add Coptis Rhizome and Gardenia Seed to the previous formula.
