The reason for postpartum lochia retention is that the zang-fu organs originally harbor deficiency-cold, or cold qi invades during labor, causing the meridians to become stagnant and obstructed, and the flow of qi to be unsmooth. Therefore, evil blood coagulates and accumulates without being discharged. If this persists for a long time, it may form masses such as "jie pi" (accumulated masses) and "shan jia" (hernias and growths), leading to abdominal distension, fullness, and pain, and generating symptoms of alternating chills and fever.
Formulas
Si Wu Tang (Four Substance Decoction) (from *Xu Yi Jian Fang*), treats postpartum lochia retention.
Dang Gui (Angelicae Sinensis Radix, sliced and baked), Chuan Xiong (Ligusticum Chuanxiong Rhizoma), Shu Gan Di Huang (Prepared Rehmanniae Radix, baked), Bai Shao (Paeoniae Radix Alba). The above herbs are crushed into coarse powder. Each dose consists of 3-4 qian (approx. 9-12g), boiled in one sheng (approx. 200ml) of water until boiling three to five times, then filtered. Take warm on an empty stomach. The original formula includes three slices of Sheng Jiang (Zingiberis Rhizoma Recens), two pieces of E Jiao (Colla Corii Asini), cooked Ai Ye (Artemisiae Argyi Folium), and a small amount of Tong Zi Xiao Bian (Child's Urine). *Rumen Shishen* states: Long Dan Cao (Gentianae Radix) and Gan Qi Wan (Dry Lacquer Pill) can be added.
Gan Qi Wan (Dry Lacquer Pill), treats postpartum evil blood retention with stabbing pain in the heart and abdomen.
Gan Qi (Dry Lacquer, crushed and stir-baked until smoke emerges), Wu Ling Zhi (Trogopteri Dung), Mo Yao (Myrrha), Mu Dan Pi (Moutan Cortex, core removed), Chen Qu (Aged Leaven, stir-fried, each half liang), Yan Hu Suo (Corydalis Rhizoma). Take twenty pills with ginger vinegar decoction; warm wine is also acceptable.
Mo Yao Wan (Myrrha Pill) (also known as Qilin Jie Pill, from *Sheng Hui Fang*), treats postpartum lochia retention, abdominal pain, and hardness below the umbilicus.
Mo Yao (Myrrha, ground), Dang Gui (Angelicae Sinensis Radix, sliced and baked), Liu Ji Nu (Viscum Coloratum Herba), Yan Hu Suo (Corydalis Rhizoma), Tao Ren (Persicae Semen, skin and tip removed, double-kernel bran-stir-baked yellow, 3 fen), Gan Qi (Dry Lacquer, crushed and stir-baked until smoke emerges), Qilin Jie (Dragon's Blood Resin), Da Huang (Rhei Radix et Rhizoma, cut and stir-fried, each 1 liang), Gui (Cinnamomi Cortex, rough bark removed), Wu Yao (Litsea Cubea Fructus, cut), Chi Shao Yao (Red Peony Root, each half liang). The above herbs are ground into fine powder, mixed with refined honey to make pills the size of paulownia seeds. Take twenty pills each time with warm wine, at any time.
Hu Po Tang (Amber Decoction), treats postpartum lochia retention with qi attacking the heart and abdomen, restlessness, and stabbing pain.
Hu Po (Amber), Jiang Huang (Curcumae Longae Rhizoma), Niu Xi (Achyranthis Bidentatae Radix, wine-soaked, sliced, and baked), Hu Zhang (Polygoni Cuspidati Rhizoma et Radix), Mu Dan Pi (Moutan Cortex, each half liang), Dang Gui (Angelicae Sinensis Radix, sliced and baked), Sheng Gan Di Huang (Raw Dried Rehmanniae Radix, baked), Gui (Cinnamomi Cortex, rough bark removed), Tao Ren (Persicae Semen, water-soaked, skin and tip removed, double-kernel bran-stir-baked, each 3 liang), Mang Xiao (Natrii Sulfas, 1 liang), Da Huang (Rhei Radix et Rhizoma, cut and baked, 1 liang), Meng Chong (Blatta, wings and feet removed, stir-baked yellow, 1 fen). The above herbs are coarsely crushed and sieved. Each dose is 2 qian (approx. 6g), boiled in one sheng of water until reduced to seven-tenths, then filtered. Take warm.
Mai Men Dong Yin (Ophiopogon Drink), treats postpartum after three days where lochia is scanty and there is restlessness below the heart.
