On the first day of febrile disease (Re Bing), the pathogen is located in the Taiyang (Greater Yang) channel, which governs the exterior. The exterior refers to the skin. With the pathogen between the skin layers, pain occurs in the head, neck, waist, and spine. Sweating therapy should be applied for recovery.

Mahuang San [From *Wanjin Huming Fang*]
Indications: Febrile epidemic toxic disease. On the first or second day: headache, high fever, whole-body heat like fire, dizziness/blurred vision, stiffness and tension of neck and back. Pulse diagnosis: Cun position floating and rapid; all three positions (Cun, Guan, Chi) floating and rapid. Sweating is indicated. If the pulse is surging, large, urgent, and has "bone strength" (firmness), sweating is contraindicated. Febrile epidemic toxic disease cannot be strictly defined by days; observe the qi of the pulse. If sweating is suitable, sweat it; if purgation is suitable, purge it. Do not err. For Yang-Du Cold Damage, after three days, the method of using medicine to resolve the disease is generally the same as treating febrile epidemic diseases. Only in the initial two days is there slight difference. Cold damage in the exterior must be released; febrile epidemic toxic disease often does not require release. If diagnosis is not careful, wait until after three days before using purgative/dispersing medicines. Formula for releasing sweating when epidemic toxin is in the exterior.
