When the pathogenic heat-toxin and noxious qi attack the Heart, it causes mental and physical restlessness. When Yang Qi is excessively hyperactive, it leads to delirious speech, red eyes, mental irritability, inability to sleep, spiritual fright and palpitations, and abnormal speech. If the toxic noxious qi is not eliminated, dryness and accumulation will form in the intestines. The pulse will be surging (Hong) and rapid (Shu); this is the clinical manifestation.
When the pathogenic heat is exuberant, patients may discard their clothes and run, climb high places and sing, or go without food for several days, climbing over walls and roofs—actions they could not perform under normal circumstances.
The ability to perform such contrary actions stems from the contention between Yin and Yang Qi, with external pathogens merging into the Yang channels. The four limbs are the root of all Yang channels; when Yang is excessive, the limbs become solid and strong, enabling one to climb and sing.
When heat is exuberant throughout the body, patients discard clothes and run; due to Yang excess, they speak wildly and curse without distinguishing kinship or strangers; widespread intense heat causes delirious speech and hallucinations (seeing and hearing things that are not there).
