The syndrome of Stone Abscess (Shiju) is identical to that of Stone Carbuncle (Shiyong), but the former is deeper. It occurs when cold pathogens invade the meridians, causing Qi and Blood to aggregate and fail to dissipate, hiding within the skin. Upon heavy pressure, it feels hard as stone, hence the name Stone Abscess. Carbuncles and abscesses are generally caused by heat pathogens; however, in this case, cold is the root cause, leading to coagulation without transformation. The toxicity adheres internally, forming a hard mass like stone. Treatment should focus on warming and unblocking the meridians, allowing heat Qi to flow freely and toxicity to vent externally, thereby rotting the heat toxin, dispersing it, and transforming it into pus and blood for discharge.
Formulas
Chenxiang San (Sinking Wood Powder) [from *Shenghui Fang*]: Treats Stone Abscess with swollen toxins, hardened masses, dry mouth, irritability and heat, limb stiffness and urgency, and inability to lie down.
Ingredients: Chen Xiang (Aquilariae Lignum Resinatum), Fang Feng (Saposhnikoviae Radix, prepared), Mu Xiang (Aucklandiae Radix, each 3 fen), Di Gu Pi (Lycii Cortex), Mai Men Dong (Ophiopogonis Radix, prepared), Dang Gui (Angelicae Sinensis Radix, prepared), Sheng Ma (Bupleuri Radix), Xuan Shen (Scrophulariae Radix), Zhi Ke (Aurantii Fructus Pericarpium, stir-fried with bran), Ling Yang Jiao (Saiga Tataricae Cornu), Du Huo (Angelicae Pubescentis Radix, prepared), Gan Cao (Glycyrrhizae Radix et Rhizoma, raw), Chi Shao Yao (Paeoniae Radix Rubra, each 1 liang), Da Huang (Rhei Radix et Rhizoma, processed, 2 liang).
Usage: Coarsely grind and sieve. Take 4 qian per dose, add 1.5 sheng of water, decoct to obtain 0.7 sheng. Strain and take warm at any time.
Zhan Si San (Stemonae Radix Powder) [from *Shenghui Fang*]: Treats Stone Abscess with hard nodules, especially if ulcerated or formed into an abscess.
Ingredients: Mu Zhan Si (Stemonae Radix), Hou Po (Magnoliae Officinalis Cortex, ginger-juice processed), Sheng Di Huang (Rehmanniae Radix, dried), Gua Lou (Trichosanthis Fructus, dried, peeled), Bai Jiang Cao (Patriniae Herba), Fang Feng (Saposhnikoviae Radix, prepared), Jie Geng (Platycodonis Radix, stir-fried), Ren Shen (Ginseng Radix), Xi Xin (Asari Radix et Rhizoma, prepared), Gui (Cinnamomi Ramulus, prepared, 0.5 liang).
Usage: Grind into powder. Take 2 qian per dose before meals, washed down with warm wine.
Dihuang Jian (Rehmannia Paste) [from *Shengji Zonglu*]: Treats Stone Abscess with hardness that does not resolve.
Usage: Take 3 jin of fresh Rehmannia Root (Sheng Di Huang), wash clean, chop finely and grind. Squeeze out the juice using cloth. Place in a copper vessel and boil over a double boiler, stirring evenly with a willow whisk until it resembles sugar. Store in a porcelain box. Take one pill the size of a bullet on an empty stomach daily, mixed with warm wine. Take at noon and evening until cured.

Bone Abscess (Fugu Ju) gets its name because pus forms attached to the bone without apparent cause. It tends to occur in large joints. In adult men and postpartum women, it often affects the thigh (Bi); in children, it often affects the spine. In acute cases in adults, pain is felt first, preventing movement; pressing elicits bone pain. After a few days, the skin and flesh become tight, swollen, and fat-like. Children cry loudly when touched near the area, indicating joint pain. In chronic cases in adults, a burning sensation in the muscles is felt first, followed by painful numbness and immobility after a few days. Children's limbs cannot move, appearing similar to paralysis. If there is a burning sensation in the joint spaces, it may be mistaken for Bone Abscess, leading to generalized swelling without ulceration and a bluish-dark complexion. Adults may also confuse it with "Thief Wind" (Zeifeng) or "Wind Swelling," not realizing it is an abscess. Generally, when the body has heat and seeks coolness in the wind, wind enters the bone fissures. Wind and heat struggle, forming Bone Abscess. Symptoms include drowsiness, heaviness, and sudden tinnitus. Exposure to cold while sleeping outdoors in autumn or summer can also cause this disease due to trapped wind-heat. Acute cases have more heat and less wind; chronic cases have less heat and more wind. Why do children get this? Their blood is abundant and muscles tender, so wind injury causes coagulation. Initially, take Loulu Tang to purge. Apply Xiaodou San to reduce swelling. Wuxiang Lianqiao Tang can be taken.
Regarding Thief Wind (Zeifeng): The patient suddenly has no fever but suffers from sudden exposure to cold wind, causing deep bone pain without restricting movement. Pressing causes bone pain. Over time, it forms painful nodules or scrofula (Luoli). Bone Abscess, however, eventually swells and suppurates, which distinguishes it. Treating Bone Abscess as Thief Wind worsens the condition and increases pus. Treating Thief Wind as Bone Abscess exacerbates wind-cold, leading to scrofula, hemiplegia, and contractures. Accurate diagnosis at the onset is crucial. Differentiation: Bone Abscess starts with pain before swelling; Thief Wind is painful but not hot. Bone Abscess features high fever, alternating chills and heat in limbs, red urine, constipation, and anhidrosis. Purging heat and opening pores can resolve it. Even if not fully resolved, it becomes superficial. Thief Wind causes night pain, extreme tenderness to touch, inability to turn over, no high fever or alternating chills, feeling cold and shivering, slight relief with heat application, and occasional sweating. It responds to acupuncture, moxibustion, heat therapy, and wind-dispelling herbs.
Deep abscesses resulting from old ulcers often appear on the shins (Jing), worsening with water exposure and cold, becoming bone abscesses (Gu Ju) over years. They are dark green/black, hard around the edges, with purulent discharge in the center, resistant to treatment. The surrounding healthy tissue swells vacuously, and bone fragments may be expelled. Soak in Wen Chilong Pi Tang. Wash daily in summer, every four days in winter. Apply Bai Ru San for necrotic tissue; apply every three days. Long-term application of Jia Zhushi San cures it. Make a paste from burned pig manure powder and fill the wound. Suck away white discharge and reapply until healed. If necrotic tissue returns, use Bai Ru San again. For stubborn bone abscesses, moxa application on the wound for three days and nights cures it. If bones protrude from recurrent ulcers, it is called Bone Abscess. Burn two fresh female black chickens, keep the bones, char them, and mix with scraped stems from three families' oxen and bamboo steamers from three families (burned to ash). Use as a suppository/paste; bone fragments should expel.
Bone Abscess resembles White Tiger Disease (Baihu), Flying Corpse Disease (Feishi), and Dripping Joint Wind (Lijie Feng). Dripping Joint Wind migrates unpredictably; White Tiger and Flying Corpse diseases have shallow pain relieved by pressure. Bone Abscess has deep pain unaffected by pressure. Some say White Tiger and Flying Corpse diseases also form pus on bones and heal only after bone fragments expel. If so, they are the same disease differing only in depth. White Tiger/Flying Corpse (vulgarly "Wind Sha") has shallow pain; Bone Abscess rarely expels bone. Clear heat-toxin; do not rely solely on Wuxiang Lianqiao or Loulu formulas. First warm the Kidney (Shen), e.g., Lingbao Gao or Shenyao. Only then use needle/cautery based on depth to extract the toxin root. Otherwise, toxins spread via meridians, causing generalized edema and sudden death.
Treatment requires heat-warming to dispel toxins and warm the Kidneys; avoid pure cooling agents. When using needle/cautery, determine depth to extract the toxin root. Otherwise, toxins spread, causing generalized edema and death. During illness, symptoms vary: thirst before ulceration indicates organ dryness; excessive drinking during diarrhea indicates fluid loss; thirst after ulceration indicates fluid depletion from pus/blood loss. Sudden spleen/stomach deficiency causes diarrhea. Cold invasion causes cough. Qi rebellion with heat causes chest phlegm. Unvented heat-toxin causes constipation. Toxic fumes attacking the heart cause vomiting, hiccups, palpitations. Yin-Yang conflict and Qi-Blood disharmony cause alternating chills/fever, organ damage, bloody regurgitation. External toxin attack causes extreme restlessness. Large-dose multi-herb formulas are needed.
