[Supplementary Discussion]
Vomiting accompanied by diarrhea is termed Huoluan (Cholera-like disease). The etiology of this disease stems from deficiency-cold in the intestines and stomach, dietary irregularities, and exposure to wind-cold pathogens, resulting in disharmony between Yin and Yang.
Differentiate whether it is a heat syndrome or a cold syndrome, and observe the pulse and breath. For heat syndromes, use cooling methods; for cold syndromes, use warming methods, aiming for balance. In cases of Huoluan with vomiting and diarrhea, if heat signs are prominent with thirst, use Wuling San (Five-Ingredient Poria Pill); if cold signs are prominent without desire to drink water, use Lizhong Wan (Regulate the Middle Pill). After vomiting and diarrhea cease, if sweating occurs with cold limbs, persistent tetany of the four limbs, and a faint pulse nearly imperceptible, use Tongmai Sini Jia Zhudan Dan Decoction (Pulse-Returning Decoction with Pig Bile). In summer, if Huoluan is caused by heatstroke presenting with irritability, thirst, cold limbs, cold sweat, and calf cramps, use Xiangru San (Mosla Powder) decocted strongly, taken cold after settling, which yields immediate effect.
Regarding the pulse of Huoluan, a floating and surging pulse indicates an easier prognosis, whereas a faint and slow pulse accompanied by shortness of breath indicates a difficult prognosis.
[Formulas]
Hezi San (Terminalia Powder): Treats vomiting and diarrhea due to Huoluan in the elderly and children, effective after one dose. It also treats nine types of heart pain and unbearable cold pain in the heart and abdomen.
