Facial carbuncles arise from internal heat combined with external deficiency, allowing wind-dampness to invade and generate sores. This is because the Lung governs Qi and manifests in the skin and hair; the Spleen governs the muscles.
Shengma Jia Huanglian Tang (Coptis Decoction with Cimicifuga) treats facial heat. Shengma Jia Fuzi Tang (Aconite Decoction with Cimicifuga) treats facial cold. What is the basis for this? The answer is: it originates from Zhang Zhongjing. Yunqizi's commentary on *Shanghan Lun* (Treatise on Cold Damage) states: regarding body heat, if the pulse is floating and long, it indicates the root pulse is deep and excess. Yangming channel disease primarily presents as steaming body heat without aversion to cold. Body heat is a branch symptom; when Yangming root is excess, stomach dryness occurs, prohibiting purgation. Misusing sweating methods may transform into jaundice. Taiyang channel governs the exterior, where Ying and Wei (Nutritive and Defensive Qi) reside. Below Ying and Wei, muscles belong to Yangming. In cases of simultaneous disease of two Yang channels, Gegen Tang (Kudzu Decoction) is indicated. Wei aspect corresponds to Guizhi Tang (Cinnamon Twig Decoction), Ying aspect corresponds to Mahuang Tang (Ephedra Decoction); within Ying and Wei, Guizhi Mahuang Gemi Tang (Half-and-Half Decoction of Cinnamon Twig and Ephedra) is used. Below Ying and Wei, in the muscle layer, Gegen Tang is used, also known as Jiemo Tang (Muscle-Relieving Decoction). Thus, Yangming is the root of muscles, not solely for inducing or stopping sweating. Combining Guizhi Tang and Mahuang Tang, adding Gegen creates Gegen Tang, illustrating the sequence of Ying, Wei, and muscles. Guizhi, Shaoyao, Shengjiang, Gancao, Dazao stop sweating; adding Mahuang, Guizhi, Gancao, Shengjiang induces sweating. Gegen has a thin flavor; adding it alone is not for sweating or stopping sweating, but to relieve the muscle layer via Gegen, hence the name Gegen Tang. Qian Boyuan (Qian Zhongyang) formulated Shengma Tang to treat Shanghan warm epidemic, severe wind-heat fever, headache, limb pain, and eruptive diseases before or after eruption, using Gegen as the sovereign herb, Shengma as the assistant, and Gancao, Shaoyao to harmonize the middle Qi. Zhu Gongyi compiled *Luren Shu* (Book of Living People), listing Shengma Tang as a resolving medicine for the Yangming channel. I once treated Lady Yang, whose Yangming channel exhibited both root and branch excess. First, the interior was attacked, then wind-heat in the channels was drained, thus using Shengma Jia Huanglian Tang to treat heat with cold herbs.
