Postpartum spontaneous lactation is caused by the failure of Stomach Qi (Wei Qi) in the Yangming channel to secure fluids. Treatment should distinguish between cases with fire and those without. If leakage persists without signs of fire, it is due to Qi deficiency; Ba Zhen Tang (Eight Treasure Decoction) or Shi Quan Da Bu Tang (Ten Significant Tonifying Decoction) are appropriate. If overflow is caused by heat in the blood of the Yangming channel, Bao Yin Jian (Preserve the Yin Decoction) or Si Jun Zi Tang (Four Gentlemen Decoction) added with Gardenia (Zhi Zi) is suitable. If liver fire surges upward causing breast distension and overflow, Jia Jian Yi Yin Jian (Modified One-Yin Decoction) is indicated. If excessive milk causes painful distension and overflow, warm compresses should be applied to dissipate the stagnation. Lactation occurring before childbirth, known as Ru Qi (Weeping Breast), results from weak fetal origin and insufficient nourishment; children born to such mothers often fail to survive.
"Chui Ru" (Blown Breast) postpartum occurs when infant feeding is interrupted by air blown into the breast, obstructing milk flow, leading to congestion, swelling, and pain. If not treated promptly, it often develops into an abscess (Yong Zhong). Immediately administer Gua Lou San (Trichosanthes Powder) and apply Arisaematis Rhizoma (Nan Xing) powder externally, while manually massaging to disperse the blockage. For severe cases, Lian Qiao Jin Bei Jian (Forsythia and Fritillaria Decoction) is most effective.
"Du Ru" (Jealous Breast) postpartum arises when there is no infant to suckle, or the infant cannot suckle, causing residual milk to accumulate and swell. Alternatively, it may result from vigorous Qi and blood in the woman, causing breast distension, swelling, pain, aversion to cold, and fever. Failure to clear the milk through suckling will inevitably lead to an abscess. If swelling does not subside, stir-fried Malt (Mai Ya) two to three liang, decocted in water and taken orally, will rapidly resolve it. Another prescription: Chen Pi (Tangerine Peel) one liang and Gan Cao (Licorice Root) one qian, decocted in water. Another method for treating Chui Ru and breast abscess swelling/pain: use Daylily Root (Xuan Cao Gen) ground with wine for oral administration, and apply the dregs to the affected area. *Xiu Zhen Fang* recommends using Pericarpium Sophorae Flavescentis (Zhu Ya Zao Jiao), peeled, honey-roasted, powdered, and taken with wine, one qian per dose. A poem states: "How to treat blown breast? Burn soap pod to ash, mix with clam powder, blend with a cup of hot wine, and laughter will return within moments."
For severe swelling and pain with excess heat-toxin, Lian Qiao Jin Bei Jian should be used first for excellent results.
Lizhai (Xue Ji) states: Female breast abscesses belong to heat-toxins in the Gallbladder and Stomach fu-organs, causing Qi and blood stagnation. Initially, swelling and pain appear on the body surface with red skin. The patient exhibits exterior fever, alternating chills and fever, or aversion to cold with headache, irritability, thirst, and preference for cold drinks. Ren Shen Bai Du San (Ginseng Detoxification Powder), Shen Xiao Gua Lou San (Miraculous Trichanthes Powder), and Jia Xiao Yao San (Modified Free Wanderer Powder) are used to resolve the swelling. After several days, if pus forms and ruptures, thick pus flows out, and self-healing occurs once the pus is exhausted. If Qi and blood are deficient, or if detoxifying herbs were mistakenly used, preventing wound closure, and the pus is thin with a large pulse, the condition is difficult to treat. Breast cancer (Ru Yan) belongs to depression and anger affecting the Liver and Spleen zang-organs, causing Qi and blood depletion. Initially, small nodules form inside the breast with normal skin color. The patient experiences internal heat, night fevers, heat in the five centers (palms, soles, and chest), fatigue, emaciation, and irregular menstruation. Jia Xiao Yao San, Jia Gui Pi Tang (Modified Restore the Spleen Decoction), and Shen Xiao Gua Lou San are used, often resolving the condition spontaneously. If accumulated over time and growing larger, with nipple retraction resembling a rock cavity, redness, and discharge, and deep internal ulceration, it is difficult to cure. However, using Gui Pi Tang and similar formulas can prolong life. Mistaken use of attacking/draining herbs leads to critical danger. Generally, for breast conditions: if caused by anger, soothe the Liver and clear heat; if accompanied by chills and fever, release the exterior and dispel pathogens; for severe swelling and pain, clear the Liver and detoxify, combined with Garlic-separated moxibustion; if no pus forms or pus does not rupture, tonify Qi and blood; if non-converging or thin pus, tonify the Spleen and Stomach; if pain recurs after pus drainage or chills/fever appear, tonify Qi and blood; if afternoon tidal fever or internal heat occurs, nourish blood; if poor appetite or vomiting occurs, tonify the Stomach; if indigestion or diarrhea occurs, tonify the Spleen; if swelling/pain follows exertion, tonify Qi and blood; if swelling/pain follows anger, nourish Liver blood. For obstruction by infant breath, suckling and massage are required; if an abscess forms, treat as above. Tidal and evening fevers also respond to the aforementioned formulas. Generally, male cases stem from kidney and liver depletion due to sexual excess, while female cases stem from Liver and Spleen damage due to emotional depression. Practitioners must examine carefully. Pregnant women suffering from this are termed "Nei Chui" (Internal Blowing). Although the etiology is the same, medications must avoid harming the fetus.
