In cases of Blood Heat, menstruation often arrives early; however, one must always examine the deficiency or excess of Yin Qi. If the menstrual blood is predominantly red, or purple and thick, or if the flow is heavy, and the pulse is surging and slippery, and the organ systems and dietary preferences favor cold drinks while fearing heat, these are all indications of Fire pathogenic factors.
For treating Blood Heat accompanied by Fire, Qinghua Yin (Clearing and Transforming Drink) should be used as the primary formula. If the Fire is severe, formulas such as Chouxin Yin (Drawing Out the Firewood Drink) may also be used temporarily, but one must not mistake false Fire for true Fire, nor treat deficient Fire as excess Fire. Generally, when Heat causes blood to flow freely and excessively, leading to prolonged menstruation, herbs such as Xu Duan (Radix Dipsaci), Di Yu (Sanguisorbae Radix), Dan Shen (Salviae Miltiorrhizae Radix), Qian Gen (Rubiae Radix et Rhizoma), and Zhi Zi (Gardeniae Fructus) can be employed. In cases of mild Fire due to Yin deficiency resulting in heavy and early menstruation, treatment should focus on nourishing Yin and clearing Heat, using Bao Yin Jian (Preserving Yin Decoction) as the main formula. The term "early menstruation" refers to the general monthly pattern. The term "Blood Heat" should be determined by examining the manifestations of the entire body's Zang-Fu organs. Do not diagnose early menstruation simply because a patient who usually has irregular cycles occasionally presents with an early period. Do not diagnose Blood Heat solely based on early menstruation if the pulse and symptoms show no signs of Fire. If there are no signs of Fire in the pulse and symptoms, yet menstruation arrives early, this is due to deficiency of Heart and Spleen Qi failing to secure and control the blood. In such cases, Daying Jian (Great Camp Decoction), Dabu Wan Jian (Great Tonifying Pill Decoction), or Wufu Yin (Five Blessings Drink) supplemented with Du Zhong (Eucommiae Cortex) and Wu Wei Zi (Schisandrae Fructus) should be used. Such cases are very common; if treated as a Fire syndrome, it will inevitably lead to misdiagnosis. If menstruation occurs two or three times a month, or every half-month, or even every ten days, this indicates a disorder of chaotic Qi and Blood. Treatment should be adjusted according to Cold or Heat patterns and should not be lumped together under the category of simple early menstruation.
