Xu Donggao stated: When phlegm-fire disturbs the mind and shen (the spirit, governed by the Heart), causing mental restlessness, excessive overthinking damages the body. This leads to blazing fire and stagnant phlegm, resulting in many cases of insomnia. There are cases where insufficient Kidney water fails to raise true yin, while Heart Yang remains hyperactive alone, also leading to inability to sleep. There are cases of Spleen fatigue with stagnant fire that cannot disperse; every time at dawn (wugeng hour), as qi rises upward, irritability ensues, making sleep impossible. In such cases, methods to invigorate the Spleen, resolve depression, clear phlegm-heat, and descend fire are appropriate. For those with robust constitutions who occasionally suffer from insomnia due to phlegm-fire, Gun Tan Wan should be used first, followed by formulas such as An Shen Wan or Qing Xin Liang Ge San. For those with weak constitutions, whether due to overexertion or post-illness recovery, this is a deficiency pattern; formulas for nourishing blood and calming the mind are appropriate. For insomnia occurring after illness or postpartum in women, this is due to deficiency of qi and blood and insufficiency of the Heart and Spleen organs. Even if phlegm-fire is present, aggressive treatment should be avoided; tonification should be the primary approach, possibly assisted by herbs that clear phlegm-heat and descend fire. For insomnia not caused by post-illness conditions, although treated based on phlegm-fire patterns, it is essential to assist with herbs that nourish blood and tonify deficiency; otherwise, the treatment is inappropriate.
