The disease of vomiting blood is generally a chronic condition; it may arise from acute consumption (Ji Lao, acute deficiency syndrome) damaging the Ying and Wei (Nutritive and Defensive Qi), causing Qi stagnation and upward rebellion, leading to reckless bleeding. If treated with cooling agents such as Lotus Root Juice (Ou Zhi) or Rehmannia Root (Sheng Di Huang), death will inevitably be hastened. In treating such patients, I use very simple prescriptions: equal parts of Licorice Root (Gan Cao) roasted and Dried Ginger (Gan Jiang) processed. These are ground into powder. Each dose consists of two qian, boiled in one zhong of water until boiling five to seven times. It should be sipped warm, taken on an empty stomach and at noon. By harmonizing the Qi and Blood, and the Ying and Wei, recovery occurs naturally. This must not be overlooked. (From "Zhu Shi")
Jianzhong Decoction (Cordialising the Middle Decoction) treats internal injury from chronic consumption with alternating cold and heat, vomiting and regurgitation, and hematemesis.
Ingredients: Sugar (three jin), Paeonia Root (Shaoyao), Pinellia Tuber (Banxia), Fresh Ginger (Shengjiang), Licorice Root (Gancao) (each three liang), Cinnamon Twig (Guixin) (two liang), Jujube Date (Dazao) (fifty pieces).
Chop the above seven ingredients, boil in two dou of water, reduce to seven sheng, divide into seven doses. Take five times during the day and twice at night. (Note: "Qianjin Yifang" lacks Gancao and Guixin, but includes Rehmannia Root Shengdihuang.) (From "Qianjin Fang")
Roupi Decoction (Softening the Spleen Decoction) primarily treats insufficiency of Spleen Qi, deficiency-cold in the Lower Jiao, fullness and obstruction in the chest and diaphragm, sweating, distension and fullness under the hypochondrium, hematemesis, and lower bleeding.
Ingredients: Prepared Rehmannia Root (Shu Dihuang) (three liang), Astragalus Root (Huangqi), Cinnamon Twig (Guizhi), Paeonia Root (Shaoyao), Licorice Root (Gancao) (each one liang).
Grind into coarse powder. Each dose is two qian, boiled with a small amount of water and wine to six parts remaining. Strain and take warm. (From "Jifeng Puji Fang"). "Furen Liangfang" cites "Yangsheng Biyong Fang" for treating chronic consumption with hematemesis, epistaxis, lower bleeding, and spontaneous sweating of white fluid. (Annotation: The character "xia" [lower] is missing the character "xue" [blood].)
Chen Yuepo's Formula treats vomiting blood or uterine bleeding (Xue Beng) with massive hemorrhage filling basins and buckets, cyanotic and pale complexion, pulse faint and near collapse, cold limbs, or spontaneous sweating, in critical cases. Use Processed Aconite Root (Zhi Fuzi) five qian, Ginseng Root (Renshen) one liang, Charred Ginger (Jiangtan) eight qian. Boil in two large bowls of water to eight parts remaining. Take warm. (From "Huiyan"). Another prescription by Longtan for uterine bleeding causing spirit dispersion and mental confusion, with impending cold limbs: use Aconite Root processed with child's urine in the base formula, plus Angelica Sinensis Root (Danggui) and Licorice Root (Gancao) roasted. (Refer to treatment examples for comparison.)
Guipi Decoction (Restore the Spleen Decoction) treats spleen injury from overthinking, frequent forgetfulness due to Heart disturbance, where the Spleen fails to control Heart Blood, leading to reckless bleeding, manifesting as hematemesis or lower bleeding. (From "Degui Fang")
Regulation Method Hemoptysis cannot be cured without quiet rest and abstinence from sexual desire; this applies to all diseases, but is especially crucial here. (From "Liu Yao")
For hematemesis, avoid salt, vinegar, fermented foods, seafood, and hot-natured foods. Sleep and sit with high pillows to prevent Qi rebellion. After seven days of this regimen, the major symptoms stabilize. Full recovery is defined as no recurrence for three years post-healing. If one cannot maintain such caution, any lapse will cause frequent recurrences, inevitably worsening cough and other symptoms; this is considered incurable. (From "Bai Wen")
