The nature of alcohol addiction is similar to other types of peculiar cravings, making it difficult to change. Among various internal peculiarities, alcohol addiction is the most severe, varying in degree and depth; some can drink one dou, others one shi, some have a limit, while others do not.
Although it is said that humans have "different intestines," this may not necessarily be due to the ability of digestive worms to metabolize food. If this were not the case, observe the state when desiring to drink: the nose smells the fragrance with joy, saliva flows uncontrollably, and upon swallowing a sip, the heart feels open and the body comfortable. The proverb states that greedy drinkers exhibit a hasty and gluttonous appearance before raising the cup, referred to as "the wine turtle worm crawling up." (Wan Yi She Cao) [Note: Yi Shuo records several cases of vomiting different worms due to hunger. Recently, Medical Officer Xi Duo Han Zi Wen wrote "Hui Zhi" (Record of Roundworms), discussing excessive hunger syndromes, mostly attributing them to roundworms. This view is insightful, as excessive hunger is likely what Lu Shi called internal peculiarity. Zi Wen's book cites historical texts with excellent analysis, which students should read.]
Liking to eat tea leaves, raw rice, paper grass, or strange items, yet feeling no fatigue or loss of appetite, maintaining normal diet, robust physique, and no thirst, all belong to worm syndromes. (Hui Bu citing Lei An)
Generally, if one has a preference for a specific food, there must be worms inside. Using the preferred food as a guide and immediately administering anthelmintic drugs will yield immediate results. (Liu Yao)
Those who like drinking tea are called "tea worm" sufferers; those who crave alcohol are called "wine worm" sufferers; those wanting to drink oil are called "oil worm" sufferers. As for children who often eat raw rice or wall mud, this is caused by worms. All should be treated with anthelmintic medicines such as lead ash, Trichosanthes kirilowii root (Leiyuan), Areca nut (Binglang), Sichuan chinaberry (Chuanlian), and Quisqualis indica (Shijunzi) made into pills, taken with the preferred food, until the medicine reaches the worms and kills them. (Yi Jing) [Note: Another method follows, recorded under "Treating with Hunger".]
Worm Drum (See Bloating Section) In the Gui Si year of the Wanli era, I was summoned by Wu Bibu, the Supervisor of Grain Transport and Judicial Review, to Huaiyin. A local gentry named Wang had a sixteen-year-old son who, after his new marriage, developed abdominal distension with palpable masses, shaped like a small melon, emaciated limbs, and unremitting day-and-night fever for half a year. Family physicians only used antipyretic and anti-distension agents, resulting in increased distension and heat, even sores on the throat and ears. Upon examination, the pulse was slippery and rapid, lips were red, abdomen painful, and the patient favored rich and sweet foods. I reasoned that abdominal pain usually presents with pale lips and aversion to food. Given this presentation, could it not be worms? I administered Awel Mass Dissolving Pills (Awei Jikuai Wan). Indeed, dozens of worms were passed, two large ones, one red and one black, over a chi long. The red line ran from head to tail; inside the worm's belly were smaller worms, several large and three or four small. After passing the worms, the fever subsided, distension decreased, and he recovered after three doses. (Chishui) [Note: Jikuai Wan contains Curcuma zedoaria (Leng), Curcuma longa (Ezhu), Scolopendra (Wugong), Aloe (Luhui), Ferula assa-foetida (Awei), Aquilaria sinensis (Chenxiang), and mineral drugs, not listed here. Another formula using Black Morning Glory Seed (Heiqiu) and six other ingredients is listed later.]
Strange Symptoms Caused by Worms If encountering bizarre diseases where treatments for phlegm and blood stasis fail, it is caused by worms. Observe the deficiency or excess of meridians and adjust treatment based on pulse and symptoms. (Liu Yao)
Strange symptoms caused by worms are diverse and hard to identify suddenly. Ancient cases mention using Trichosanthes kirilowii root (Leiyuan) for "echoing worms." Recently, a woman, after coughing and abdominal pain, suddenly liked to shout, initially making gaping sounds, then continuous humming, changing voices like a hen, frog, or boatman's call, occurring dozens of times daily. Enduring it caused chest oppression. This is called "shouting worm," similar to echoing worm. Another person, after fever and fullness, felt nauseous, vomiting several shrimp-like jumping worms. Various treatments failed. Someone suggested Copper Green (Tonglv) to induce vomiting; after two or three times, they never appeared again. (Yi Tong)
Jin Feng from Xishan, Xiuyi County, told people: Someone had severe lower abdominal pain unresponsive to medicines. A doctor moxibusted Guanyuan (CV4) for over ten moxa cones. The next day, the urethra itched, and a worm about four or five fen long emerged. It was quickly pulled out with iron tweezers. The naked worm was five or six cun long. One worm emerged daily for seven days, and pain ceased. Initially shocked, he later accepted it, pulling them out by hand. Zhang Zhongjing said: "Internal heat forces them out through the urinary opening." This person indulged in alcohol and sex, causing damp-heat steaming the bladder, combining with stagnant blood to form worms. The Classic says damp-heat generates worms. Thus, tuberculosis worms and tapeworms arise from internal damp-heat stagnation, like spring-summer damp-heat generating insects. This is also a rare disease. (Wang Shishan)
Differentiating Treatment for Deficiency and Excess For weak constitutions, warm tonification should be used first to support Yuan Qi, followed by gentle royal methods assisted by one or two anthelmintics, such as Hua Chong Wan, Shijunzi Wan, or Wuge Xiaqi Wan. Alternatively, tonify after expelling worms; otherwise, expelling worms may scatter Yuan Qi. For strong constitutions with worms attacking the upper jiao causing heart/abdominal pain, use strong decoction of Camphorwood chips (Zhangmu) to induce vomiting. After vomiting worms and pain relief, administer Licorice decoction to harmonize the stomach. For spontaneous vomiting of worms due to accumulation, use equal parts Lead Tin Ash (Heixi Hui) and Areca nut (Binglang) as powder, taken with rice water. (Ru Men)
Eliminating the Source Although treating worms requires expulsion, to cure the root and eliminate the source, one must focus on warming and nourishing Spleen and Kidney Yuan Qi. If organ Qi and Yang are strong, worms cannot survive or generate. I formulated Wenzang Wan, which is very suitable. (Jing Yue) [Note: Regarding vomiting roundworms: Anthelmintic drugs inevitably damage Stomach Qi. If Stomach Qi is further damaged, not only can worms not be expelled, but life will be lost.]
Among all worms, roundworms are the most common. Expulsion methods are generally as described above, but expelling while they regenerate is not a good strategy. To eliminate the source, one must warm and nourish the Spleen and Stomach; if Spleen/Stomach Qi is strong, worms will not generate. Treatments vary between attack and tonification, requiring careful consideration of urgency and sequence, not to be ignored. (Ibid.)
If worms regenerate after removal, warm the viscera and attack gently, avoiding excessive haste or harshness. (Yi Ji)
Tonifying After Expelling Worms Even if Qi and Blood are deficient, if worms and accumulations exist, one must first use chasing and killing agents to remove worms, phlegm, fluid retention, and sour/fishy residues before administering tonics. Otherwise, efficacy will be lacking. (Yi Tong) [Note: Qian Jin Fang states: Before taking tonics or treating diseases, all worms, phlegm, and old food must be cleared. Otherwise, medicinal power will fail. Xu Shi likely based this on that saying. Also, the previous and current sections distinguish deficiency and excess.]
General Examination of Worm Medicines Kun notes: Ancient formulas for killing worms commonly used Trichosanthes kirilowii root (Leiyuan), Cyathula officinalis (Guanzhong), Dried Lacquer (Ganqi), Wax dust, Stemona sessilifolia (Baibu), and Lead ash. Adding Aconite (Fuzi) and Dried Ginger (Ganjiang) strengthens righteous Qi. Adding Sophora flavescens (Kushen) and Coptis chinensis (Huanglian) calms worms with bitterness. Adding Mume fruit (Wumei) and Chebula terminalia (Hezi) softens worms with acidity. Adding Veratrum nigrum (Lilu) and Thladiantha dubia (Gua Di) induces vomiting with worms. Adding Genkwa flos (Yuanhua) and Pharbitis nil seed (Heiqiu) promotes defecation with worms. Using Realgar (Xionghuang), Zanthoxylum bungeanum (Chuanjiao), Cnidium monnieri (Shechuangzi), Camphor (Zhangnao), Mercury (Shuiyin), and Areca nut (Binglang) treats scabies worms. Using Tamarix galericulata gum (Hutong Lei), Datura stramonium seed (Langdangzi), Allium tuberosum seed (Jiu Zi), and Toad venom (Chansu) treats dental caries worms. Using Hibiscus tiliaceus bark (Chuanjin Pi) and Peltophorum pterocarpum bark (Haitong Pi) treats wind-xuan worms. Using Semen celosiae (Qingxiang Zi) and Rubus chingii folium (Fupen Ye) treats nine orifices erosion worms. Using Decayed drum heart, Peach talisman board, Tiger bone/excrement, Dead man's pillow, Otter claw, and Stork bone drives away tuberculosis worms. Or using Southeast branches of Peach and Willow, borrowing the Qi of Spring growth and Summer expansion to assist the body. Or using East-growing roots of Evodia rutaecarpa (Wuzhuyu) or Acid Pomegranate root (Suan Shiliu) to decoct and swallow medicine, partly because these kill worms, partly because the East is the beginning of life, where worms receive Qi. East-growing roots guide medicines eastward, stealing their vital Qi, acting as an axe to cut roots. (Yi Fang Kao)
Medication Under Extreme Hunger Worms reside deep in the Spleen and Stomach. Medicine passes through the middle. Worms avoid the medicine's scent, making it hard to catch them. My method: Starve the patient severely first, then use Areca nut (Binglang) and Trichosanthes kirilowii root (Leiyuan) as guides, followed by other purgative anthelmintics. Profuse diarrhea (ten-plus times) can empty the bowels completely. Zhang Zizheng used this method to expel hundreds of worms linked together, over a zhang long. (Rumen Shinqin)
Another method: No need for medicine. Simply forbid the patient from eating their favorite foods. Endure for four to five days. Deprived of food, worms rise seeking nourishment, causing intense craving in the mouth. Bind the patient to a stool, prone position. Bring the favorite food near the mouth. The aroma enters the throat, and the worms will eagerly emerge. (Yi Jing) [Note: This follows the tea/wine worm section. This method originates from Waitai Miyao and Guangji Fang treating "fa zheng". Many similar methods exist.]
Timing of Administration According to Danxi: In the first half of the lunar month, worm heads face upward, easier to treat. In the second half, heads face downward, harder. First use meat broth or honey to guide heads upward, then medicate. These are good methods for slow treatment. However, for urgent worm syndromes, one cannot wait. Differentiating worm heads by moon phases seems vague. Using aromatic bait to guide heads shows that late-month treatment also has methods. Why worry about difficulty? (Jing Yue)
