[Note] Diarrhea that does not cease, inability to take water and grain, deficient Qi, fine pulse, cold skin, leads to death from Yang exhaustion. Diarrhea with pure blood, mouth closed (Jinkou), vomiting and reversal, organ Qi [deficient], body heat, large pulse, leads to death from Yin exhaustion.
Canglin Tang (Granary Decoction) & Dahuang Huanglian Tang (Rhubarb and Coptis Decoction)
For initial dysentery with exterior heat, Canglin Tang is suitable; for interior heat rushing to the heart, Dahuang Huanglian Tang is used. For cold dysentery, Lizhong Tang with Hezi (Chebulae Fructus), Roukou (Arecae Semen), and Suosha (Amomi Fructus Rotundus) is indicated. If white discharge predominates, add Fuzi (Aconiti Lateralis Radix Praeparata); if red discharge predominates, add Rongui (Cinnamomi Cortex).
[Note] For initial dysentery presenting with exterior syndrome and fever, purgation should not be used; the method requires first resolving the exterior, using Canglin Tang to induce sweating. If interior heat is excessive, rushing upward to the heart causing vomiting and mouth closure (Jinkou), the method requires first purging the interior, using Rhubarb (Rhei Radix et Rhizoma), Coptis (Coptidis Rhizoma), and good wine decocted and taken to purge it. For cold dysentery, Lizhong Tang (Regulate the Middle Decoction) should be used, adding Chebulae Fructus (Hezi), Arecae Semen (Roukou), and Amomi Fructus Rotundus (Suosha). If white discharge predominates, add Prepared Aconite Root (Fuzi); if red discharge predominates, add Cinnamon Bark (Rongui).
Shaoyao Tang (Peony Decoction)
For initial dysentery without significant interior or exterior heat, use Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Zhishi (Aurantii Fructus Immaturus), Muxiang (Aucklandiae Radix), Shaoyao (Paeoniae Radix Alba/Danpi), Danggui (Angelicae Sinensis Radix), Binglang (Arecae Semen), Gucao (Glycyrrhizae Radix et Rhizoma), and a small amount of Rongui (Cinnamomi Cortex). If urination is astringent and red, add doubled Shihuo (Talcum); if defecation frequency is uncontrolled and tenesmus/pain is severe, add Dahuang (Rhei Radix et Rhizoma).
[Note] For initial dysentery without exterior heat and with mild interior heat, Shaoyao Tang is suitable. It consists of Scutellaria Root (Huangqin), Coptis Rhizome (Huanglian), Immature Bitter Orange (Zhishi), Aucklandia Root (Muxiang), White Peony Root (Shaoyao), Angelica Sinensis Root (Danggui), Areca Seed (Binglang), Licorice Root (Gucao), and a small amount of Cinnamon Bark (Rongui). If urination is astringent and red, add Talcum (Shihuo); if defecation frequency is uncontrolled and lower abdominal pain/tenesmus is severe, add Rhubarb (Dahuang).
Xianglian Hewei Tang (Costus and Coptis Stomach-Harmonizing Decoction), Shenlian Kaijin Tang (Ginseng and Coptis Mouth-Opening Decoction), & Navel-Pasting Method
After purging for dysentery, regulate Qi and Blood; Xianglian Hewei Tang is suitable. It contains Huangqin (Scutellariae Radix), Shaoyao (Paeoniae Radix), Muxiang (Aucklandiae Radix), Huanglian (Coptidis Rhizoma), Gucao (Glycyrrhizae Radix), Chenpi (Citri Reticulatae Pericarpium), Baizhu (Atractylodis Macrocephalae Rhizoma), Suosha (Amomi Fructus Rotundus), and Danggui (Angelicae Sinensis Radix). For deficiency-type red dysentery, add fried Chun Gen Bai Pi (Toonae Radix) and fried Diyu (Sanguisorbae Radix). For long-standing white dysentery with Qi deficiency, add Renshen (Ginseng Radix), Fuling (Poria), and fried Ganjiang (Zingiberis Rhizoma) to tonify. For excess-type mouth closure (Jinkou) where purging is tolerable, use Dahuang Huanglian Tang to purge. If purging is not tolerable, internally administer a decoction of Ginseng, Coptis, and Stone Lotus Seed (Shi Lianzi), taken slowly; improvement occurs upon swallowing. Externally apply Wanggua Teng San (Trichosanthes Vine Powder) to the navel: freeze-dried Trichosanthes Vine, stem, and leaves are burned to ash, mixed with sesame oil, and applied into the navel, which is effective.
[Note] After purging for dysentery, when the pathogenic factor is significantly reduced, Qi and Blood should be regulated using Xianglian Hewei Tang, consisting of Scutellaria Root (Huangqin), Peony Root (Shaoyao), Aucklandia Root (Muxiang), Coptis Rhizome (Huanglian), Licorice Root (Gucao), Tangerine Peel (Chenpi), Atractylodes Rhizome (Baizhu), Cardamom Fruit (Suosha), and Angelica Sinensis Root (Danggui). For red dysentery with excessive bleeding due to deficiency, astringents should be used, adding fried Toona Root Bark (Chun Gen Bai Pi) and fried Sanguisorba Root (Diyu). For white dysentery lasting long with Qi deficiency, add Ginseng Root (Renshen), Poria (Fuling), and fried Ginger Rhizome (Ganjiang) to tonify. For excess patterns with mouth closure (Jinkou) where purging is bearable, use Dahuang Huanglian Tang to purge. If purging is not bearable, internally decoct Ginseng, Coptis, and Stone Lotus Seed (Shi Lianzi), taking it slowly; relief comes immediately upon swallowing. Externally, apply Wanggua Teng San to the navel: this consists of Trichosanthes Vine, stems, and leaves after frost, burned to ash, mixed with sesame oil, and placed in the navel, which is effective.
Zhenren Yangzang Tang (True Man's Nourishing Viscera Decoction)
For chronic dysentery with mixed cold and heat, Wumei Wan (Mume Pill) regulates. For cold-deficiency with slipping/dysentery, Yangzang Tang warms and tonifies. It contains Renshen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Roukou (Arecae Semen), Danggui (Angelicae Sinensis Radix), Hezi (Chebulae Fructus), Rongui (Cinnamomi Cortex), Shaoyao (Paeoniae Radix), Ying Susu Ke (Papaveris Capsula), Gucao (Glycyrrhizae Radix), and Muxiang (Aucklandiae Radix).
[Note] For chronic dysentery where internal cold and heat are indistinguishable, Wumei Wan (Mume Pill) should be used to harmonize. For cold-deficiency with slipping/dysentery, Yangzang Tang (Nourishing Viscera Decoction) should be used to warm and tonify, consisting of Ginseng Root (Renshen), Atractylodes Rhizome (Baizhu), Areca Seed (Roukou), Angelica Sinensis Root (Danggui), Chebula Fruit (Hezi), Cinnamon Bark (Rongui), Peony Root (Shaoyao), Poppy Capsule (Yingsu Ke), Licorice Root (Gucao), and Aucklandia Root (Muxiang).
Xianglian Pingwei San (Costus and Coptis Stomach-Suppressing Powder), Weifeng Tang (Stomach-Wind Decoction)
For dysentery of undigested food and grain, Tiaozhong Yiqi Tang regulates the middle and boosts Qi. For damp dysentery, Xianglian Pingwei Fang (Costus-Coptis Formula) is used. For deficiency-dampness-wind dysentery, Weifeng Tang treats it. It contains Rongui (Cinnamomi Cortex), Sumi (Panicum miliacum), and Bazhen Tang (Eight-Treasure Decoction) minus Dihuang (Rehmanniae Radix).
[Note] Dysentery of undigested food and grain is due to Spleen-Stomach deficiency with insufficient transformation; Tiaozhong Yiqi Tang (Regulate Middle and Boost Qi Decoction) is suitable. For damp dysentery, Aucklandia Root (Muxiang) and Coptis Rhizome (Huanglian) combined with Pingwei San (Stomach-Suppressing Powder) are indicated. For dampness with deficiency, Weifeng Tang (Stomach-Wind Decoction) is suitable, consisting of Cinnamon Bark (Rongui), Millet (Sumi), and Eight-Treasure Decoction (Bazhen Tang) minus Rehmannia Root (Dihuang).
