Accumulation and mass disorders: If the patient's stomach qi is strong and appetite is normal, attacking methods (攻法) can be used. If the patient is deficient, attacking and tonifying should be combined to ensure that the pathogenic factor is eliminated while the healthy qi remains secure. For qi stagnation, food retention, and masses, resolving stagnation is appropriate; formulas such as Wenbai Wan (Warm White Pill), Taoren Jian (Peach Kernel Decoction), or Kongxian Dan (Control Saliva Pill) may be selected.
[Note] Treatment of accumulation and mass should employ attacking methods, but only when the patient has strong stomach qi and can eat. If attacking a deficient person, tonic herbs must be combined, employing strategies such as "one attack followed by three tonifications" or "five tonifications followed by one attack." This eliminates the pathogen without damaging the healthy qi, and nourishes the healthy qi without aiding the pathogen, thereby allowing the pathogen and healthy qi to coexist peacefully. For treating qi stagnation, food retention, and masses, the secret formula Huazhi Wan (Resolve Stagnation Pill) is appropriate (recorded in the Internal Injury section). For treating accumulation and jia masses, Wenbai Wan (i.e., Wanbing Ziwan Wan, with doubled dosage of Chuanwu/Caulis Aconiti Preparatus) is appropriate. For treating blood accumulation and blood jia, Taoren Jian (Peach Kernel Decoction) is appropriate (composed of Tao Ren/Persicae Semen 1 liang, Da Huang/Rhei Radix et Rhizoma 1 liang, Meng Chong/Blattariae Periplanetae Folium stir-fried 5 qian, Piao Xiao/Natrii Sulfas 1 liang, all ground into powder. First, boil 1 jin of pure vinegar slowly in a clay pot until reduced to slightly more than half a cup, add the powder, stir for a long time, and form small pills. Do not eat dinner the day before. At dawn (wugeng), take 1 qian with warm wine to expel foul matter resembling bean juice or chicken liver. If not expelled, take again the next day; stop medication when fresh blood appears. If Meng Chong is unavailable, Zhe Chong/Mantis Oviposita may substitute, though it is less effective than Meng Chong). For treating phlegm accumulation, Kongxian Dan (Control Saliva Pill) is appropriate (recorded in the Phlegm-Fluids section).
The Classics state that disorders of the Ren Mai (Conception Vessel) result in seven types of hernia (shan). Zhang Hezi attributes the seven hernias primarily to the Liver. The Liver Channel passes through the abdomen and surrounds the genitalia, while the Ren Mai circulates within the abdominal interior. Hernia syndromes manifest as pain in the lower abdomen radiating to the genitals, qi rushing up to the heart causing difficulty in urination and defecation, alternating episodes of Jue hernia, Jia hernia, and Hu hernia, as well as ulceration with pus discharge, strangury (long), constipation, and hard, swollen scrotum.
[Note] The classics say: When the Ren Mai is diseased, men develop internal seven hernias, and women develop leukorrhea and jia-ju masses. Jia-ju refers to hernia-like conditions in women. The reason the seven hernias are governed by the Ren Mai is that the Ren Mai originates at Zhongji (CV3) and circulates within the abdomen. The reason they are governed by the Liver is that the Liver Channel passes through the abdominal interior and surrounds the genitalia. Therefore, all hernia diseases arise from these two meridians, so treatment focuses on them. Symptoms of hernia disease: Lower abdominal pain radiating to the testicles, qi rushing up to the heart, and inability to pass urine or stool is called Chong hernia. Lower abdominal pain radiating to the testicles, with rebellious Liver qi attacking the Stomach causing vomiting is called Jue hernia. Qi in the lower abdomen failing to extend, with lumps on either side causing pain is called Jia hernia. When lying down, the mass enters the abdomen; when standing, it exits the abdomen into the scrotum, resembling a fox that leaves its hole to urinate during the day and returns at night without urinating, is called Hu hernia. Lower abdominal pain radiating to the testicles, with a cucumber-shaped swelling between the ends of the Henggu bone containing pus and blood, is called [Missing Character] hernia. Lower abdominal pain radiating to the testicles, with urinary obstruction, is called Long hernia. No pain in the lower abdomen, but with a large, hard, and stiff scrotal swelling, is called [Missing Character] hernia.
