To treat infants and young children with slow spleen wind syndrome characterized by surging and undigested phlegm-saliva, Bai Jiang Can Wan (White Silkworm Pill) is indicated.
[Formula Composition]
Zhi Niu Dan Wu Wei Zi (Prepared Bull Gallbladder and Schisandrae Fructus, each 1 fen), Bai Jiang Can (White Silkworm, remove silk glands and stir-fry), Qian Zi (Mantis Egg Case), Di Bie Chong (Ground Beetle), Wu Ling Zhi (Trogopteri Dung, from Sichuan), Quan Xie (Whole Scorpion, stir-fried), Ban Xia Mo (Pinelliae Rhizoma Powder, each 1 fen, soaked in fresh ginger juice).
[Preparation and Administration]
Grind the above ingredients into fine powder. Use the cooked Pinelliae Rhizoma Powder paste as a binder to form pills the size of hemp seeds. Take 30 pills per dose, administered with a decoction of Yin Jin Hua (Honeysuckle Flower) and Bo He (Menthae Haplocalycis Herba).
[Commentary]
Wind pathogen affecting the Spleen is caused by deficiency; this is the origin of fright and convulsions, leading to the production of phlegm-saliva. Yin epilepsy does not manifest violently or suddenly, but empirically, if one attempts aggressive treatment hastily, it causes greater harm. Therefore, harsh drugs that open orifices and unblock collaterals are prohibited as they offer no benefit. The appropriate therapeutic method is to select an excellent formula now to save lives with a single dose. Bai Jiang Can Wan (White Silkworm Pill) alone can resolve phlegm. When wind has transmitted to the "slow spleen" stage, wind-phlegm mixes, and fright-heat converges, physicians must weigh the pros and cons when prescribing, hesitating to decide definitively. If the child's Yang Qi has not yet collapsed, direct administration of this formula is suitable. However, if Yang deficiency and Yin excess prevail, presenting with critical urgency and unpredictable changes, where spirit, complexion, and pulse appear terrifyingly abnormal, the following excellent formulas should be administered.
For treating infants and young children with slow spleen wind syndrome accompanied by cold limbs and cold extremities, Fu Liu Wan San (Sulfur and Aconite Pill and Powder) is particularly effective.
[Formula Composition]
Hei Fu Zi Jian (Tip of Black Aconite Root, two pieces, peeled, raw use), Xie Shao (Scorpion Tail, seven pieces), Shu Liu Huang Mo (Prepared Sulfur Powder, 1 liang).
[Preparation and Administration]
Grind the above ingredients into an extremely fine powder. Mix with fresh ginger juice to form pills the size of mung beans. Administer 20 pills per year of age, taken with rice water.
Black Fu Tang (Black Aconite Decoction) for treating slow spleen wind with abundant phlegm and cold extremities.
[Formula Composition]
Hei Fu Zi (Black Aconite Root, processed and powdered, weight 2 liang), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 1 liang), Nan Xing (Arisaematis Rhizoma, processed, 1 liang), Gan Cao (Licorice Root, honey-fried, 1 liang), Ban Xia (Pinelliae Rhizoma, 1 liang, washed seven times in boiling water).
[Preparation and Administration]
Cut the herbs into small pieces. For each dose, take 2 liang of the mixture, add 1.5 cups of water, three small slices of fresh ginger, and one jujube. Decoct until half the liquid remains, strain, and administer orally using a spoon. Stop medication once the hands and feet feel warm and symptoms gradually improve.
For treating infants and young children whose acute convulsive disease has transmitted to slow spleen wind syndrome, with cold phlegm stagnating in the diaphragm causing recurrent attacks without dissipation. This condition involves long-standing deficiency and should not be treated with sudden purgation. If the child exhibits slow convulsions and severe lethargy/coma, this formula is indicated.
[Formula Composition]
Da Hei Fu Zi (Large Black Aconite Root, one piece weighing 8-9 liang, peel skin and navel, carve a hole at the neck, insert half a liang of Ge Shuang Peng Sha (Borneolum Syntheticum) and half a liang of Borax into the hole, seal with Aconite and Atractylodes powder, burn to ash while preserving nature), Tian Nan Xing (Arisaematis Rhizoma, processed, half a liang), Xie Shao (Scorpion Tail), Qiang Huo (Notopterygii Rhizoma et Radix, each 1 liang), Zhu Sha (Cinnabar).
[Preparation and Administration]
Grind into fine powder, mix evenly with Cinnabar, and bind with refined honey to form pills the size of a cockle seed. Take 1 to 2 pills per dose. Decoct Yin Jin Hua (Honeysuckle Flower) and Bo He (Menthae Haplocalycis Herba), add 3-5 drops of wine to dissolve the pill, and administer.
[Commentary]
All three formulas utilize Hei Fu Zi (Black Aconite Root). One must not rigidly adhere to its hot nature and fear administering it to young children due to their tender constitution. In cases of spleen wind with cold hands and feet, severity varies from mild to acute. Assess the severity: use decoctions for mild cases, and pills or pastes for severe cases; all are effective. Treatment should continue until the hands and feet become warm and Yang Qi recovers, bringing comfort. Once warmth is restored, combine with formulas that awaken the Spleen and rectify the Stomach. These formulas are specifically indicated only when acute convulsions transmit to the slow spleen wind syndrome. Liu Huang (Sulfur) acts rapidly, expelling wind and resolving phlegm, awakening the Spleen, rectifying the Stomach, warming the zang-fu organs, tonifying the intestines and stomach, rescuing life, restoring Yang Qi, and saving critical conditions. It is highly effective. Do not act in panic and indiscriminately administer pills or powders, nor use doses that are too large or too small, lest errors occur. Study and accept the subsequent formulas to avoid absurdity and misleading practice.
