I have discussed this condition, which arises from the accumulation of wind-phlegm and retained fluid in the chest and heart region, compounded by an external contraction of wind-heat. The Lung is the canopy (the highest organ covering all other viscera) among the five zang organs, situated above the diaphragm. When water qi overflows, the Lung becomes hyperfluent; after a long period, it condenses into phlegm and stagnates in the chest and heart. Coupled with the internal emergence of wind-heat, the external manifestations include red lips, flushed face, coughing, and rapid breathing. This leads to the sternum protruding high like an inverted palm, a condition named Turtle Chest (Guixiong). The treatment method involves relaxing the Lung, resolving phlegm, and unblocking the diaphragm to eliminate phlegm-fluid in the Lung meridian. First, administer Wuling San (Five-Ingredient Poria Decoction) combined with Kuanqiang Qi Yin (Harmonizing and Broadening Qi Decoction), mixed with ginger juice and scallion decoction for oral administration. Subsequently, treat with Qingfei Yin (Lung-Clearing Drink), Xionghuang San (Realgar Powder), Biyu Wan (Jasper Pill), and Ruyi Gao (Ruyi Ointment). If, after administering these medicines, the disease recurs and transforms, presenting with fixed staring eyes, upward congestion of phlegm-saliva, and convulsions, it becomes difficult to treat.
As for Turtle Back (Guibei), it occurs when infants under half a year old are forced to sit too early. External wind attacks the spine, transmitting pathogenic factors into the marrow, causing the back to rise like a turtle shell, hence named Turtle Back. It ultimately becomes a chronic, stubborn disease; how should it be treated?
Within one hundred days after birth, or after six months of age, if an infant suddenly presents with slightly raised, reddish eyelids, a bluish-dark complexion, and nocturnal irritability and crying, or if the face resembles cinnabar, this indicates latent heat accumulating internally. In cases where the face is not red, the origin lies in severe fright suffered by the mother during pregnancy. Fright-pathogens damage the fetus and transmit sequentially. After birth, exposure to heat-toxins or further fright may occur. Fright generates heat; this heat accumulates internally and manifests externally. Initially, it appears scattered across the face, resembling water-filled vesicles, with slightly red but non-turgid bases, appearing and disappearing irregularly. It then spreads to the neck, becoming as red as cinnabar, named Jingdan (Fright Erythema). First, wash the eyes with Sisheng San (Four-Saints Powder). Then, decoct Bairen San (Hundred-Resolving Powder) together with Wuhe Tang (Five-Harmonies Decoction) to resolve fright-heat and erysipelas-like toxins. Niubang Tang (Arctium Decoction), Danggui San (Angelica Powder), Sansan Jie San (Three-Dispersal Resolving Powder), and Huangqin Siwu Tang (Scutellaria Four-Substance Decoction) can all be used for treatment. If Jingdan erupts between the chest and breasts, accompanied by slight phlegm-wheeze and convulsions, urgent measures to vent heat and extract toxins are required to prevent internal invasion, which would cause significant harm. Add Shengma (Cimicifugae Rhizoma) and Sheng Dihuang (Rehmanniae Radix Recens) to Wuhe Tang, decoct with water, fresh ginger, and Jiedixin (Centellae Herba), and administer orally; the condition will naturally subside. Continue调理 with the aforementioned medicines to prevent other complications, or alternatively, administer Wan'an Yin (Ten-Thousand-Peace Drink).
