The *San Yin Fang* records: When women exert themselves excessively during labor, it may cause prolapse of the vagina resembling rectal prolapse, or protrusion of the genitalia accompanied by a sensation of tightness, swelling, and pain. Sexual intercourse or physical fatigue can also trigger this condition, presenting with continuous discharge of clear fluid and dribbling urination.
Zhu Danxi recorded: A woman in her thirties, two days after delivering a daughter, had an object resembling a handkerchief with a pointed tip protruding from the vagina, weighing approximately one *jin*. I reasoned that this was caused by Qi deficiency due to pre-natal overexertion or liver flaccidity (*Gan Wei*), fortunately with minimal blood deficiency. As it was late and cold, fearing cold damage, I urgently prescribed half a *qian* of Honey-fried Huangqi (Astragali Radix), one *qian* of Renshen (Ginseng Radix), five *fen* of Baizhu (Atractylodis Macrocephalae Rhizoma), one and a half *qian* of Danggui (Angelicae Sinensis Radix), and five *fen* of Shengma (Cimicifugae Rhizoma). After three doses, the prolapsed part retracted, and she felt secure after sweating throughout her body. However, a piece of tissue, weighing about five or six *liang*, had dried and detached where it touched the mat; this was adipose membrane. Subsequently, her appetite improved and she slept well. Her pulse was choppy (*Se*) generally, slightly wiry on the left, yet her constitution appeared robust. I prescribed half a *qian* each of Baizhu (Atractylodis Macrocephalae Rhizoma) and Shaoyao (Paeoniae Radix Alba), one *qian* of Chenpi (Citri Reticulatae Pericarpium), and one slice of Shengjiang (Zingiberis Rhizoma Recens), decocted and taken for two to three doses to nourish her.
Another woman, after childbirth, had an object resembling a covered bowl with two branches protruding from the vagina. Her husband sought treatment. I reasoned this was uterine prolapse due to weak Qi and Blood causing sinking. I prescribed a large dose of Shengma (Cimicifugae Rhizoma), Danggui (Angelicae Sinensis Radix), and Huangqi (Astragali Radix) in one dose. Half a day later, the husband returned, saying, "After taking two doses, there was a sound, and upon inspection, it had retracted into the vagina. However, because it had been dry and stuck to the mat overnight, a piece the size of a palm broke off. My wife is crying at home, fearing that if it is broken, it cannot regenerate." I reasoned that this was not intestine but adipose tissue; muscle tears can heal, and if Qi and Blood become abundant, it will surely regenerate. I then prescribed Siwu Tang (Four Substance Decoction) added with Renshen (Ginseng Radix) for one hundred doses. Three years later, she gave birth again.
For treating uterine prolapse, use one and a half *qian* of Huangqi (Astragali Radix), one *qian* of Renshen (Ginseng Radix), seven *fen* of Danggui (Angelicae Sinensis Radix), three *fen* of Shengma (Cimicifugae Rhizoma), and two *fen* of Gancao (Glycyrrhizae Radix et Rhizoma). Prepare as one dose, decoct with one cup of water until reduced to half a cup, remove dregs, and take before meals. Additionally, wash with a solution of Wubeizi (Terminalia Chebulae Fructus) powder and apply the powder topically. Repeat several times, and continue oral medication to prevent recurrence permanently.
A postpartum woman expelled a fleshy cord from the urethra, three to four chi long. Touching it caused excruciating pain. She first took Shixiao San (Pain-Dispersing Powder) several times. Two *jin* of ginger with skin were mashed and mixed with two *jin* of refined sesame oil, boiled until the oil was absorbed/dry. The fleshy cord was wrapped in silk cloth, bent, and placed beside the urethra. It was fumigated with the ginger previously mentioned, and when cool, warmed by moxibustion or hot compress. After six nights, it shrank by more than half; after twelve days, it fully retracted. She then took Shixiao San (Pain-Dispersing Powder) and Xiong Gui Tang (Ligusticum and Angelica Decoction) for regulation. If the fleshy cord breaks, it is incurable.
Danggui Huangqi San (Angelica and Astragalus Powder) treats postpartum vaginal prolapse (retraction of the vaginal opening). (Tonifies and lifts.)
Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), Huangqi (Astragali Radix), Renshen (Ginseng Radix) (each two *qian*); Shengma (Cimicifugae Rhizoma) (five *fen*).
One formula contains Gancao (Glycyrrhizae Radix et Rhizoma) instead of Baishao (Paeoniae Radix Alba).
Crush into one dose, decoct in water, and take warm before meals. Externally, wash with Wubeizi (Terminalia Chebulae Fructus) infusion and apply powder. (Wubeizi secures prolapse.)
Jiawei Bazhen Tang (Modified Eight Treasures Decoction) treats postpartum rectal prolapse. (Tonifies Qi and Blood and lifts.)
Bazhen Tang (Eight Treasures Decoction) (eight *qian*); Huangqi (Astragali Radix) (one *qian*); Fangfeng (Saposhnikoviae Radix), Shengma (Cimicifugae Rhizoma) (each five *fen*).
Crush for one dose, decoct in water, and take. Externally, fumigate and wash with Jingjie (Schizonepetae Spica), Huoxiang (Agastaches Herba), and Chupi (Toosendan Fructus) decoction.
Jiawei Siwu Tang (Modified Four Substance Decoction) treats vaginal protrusion due to excessive bearing-down efforts during delivery. (Nourishes Blood and secures prolapse.)
Siwu Tang (Four Substance Decoction) (four *qian*); Longgu (Fossilia Ossis Mastii) (lightly ground separately, added just before administration).
Crush, decoct in water, and take. For vaginal pain, add Gaoben (Ligustici Chuanxiong Rhizoma) and Fangfeng (Saposhnikoviae Radix), removing Longgu (Fossilia Ossis Mastii). (The brilliance lies in adding and removing one herb, utilizing Gaoben's lifting property.)
Danggui San (Angelica Powder) treats vaginal prolapse, also known as *Han* hernia.
Danggui (Angelicae Sinensis Radix), Huangqin (Scutellariae Radix) (each two *liang*); Baishao (Paeoniae Radix Alba) (one *liang*); Chupi (Toosendan Fructus) (burned to retain char, half *liang*); Muli (Concha Ostreae) (calcined, two and a half *liang*).
Grind into powder. Take two *qian* per dose, mixed with warm wine or rice broth as desired. Avoid climbing heights or lifting heavy objects. (This refers to uterine prolapse; Ciji (Tribuli Fructus), Muli (Concha Ostreae), Huangqin (Scutellariae Radix), and Baishao (Paeoniae Radix Alba) cool and secure prolapse.)
Liuhuang San (Sulfur Powder) treats severe bearing-down efforts during childbirth causing vaginal prolapse resembling rectal prolapse, or protrusion of the genitalia with tightness, swelling, and pain (prolapse due to Liver Heat), triggered by movement or sexual activity, with continuous clear discharge and dribbling urination.
Liuhuang (Sulfur), Haiheke (Sepiae Endoconcha) (each half *liang*) (Liuhuang and Haiheke warm and secure prolapse); Wuweizi (Schisandrae Chinensis Fructus) (two and a half *qian*).
Grind into powder, apply to the affected area, changing three times daily.
Wujiao Tang (Pepper and Aconite Decoction) treats vaginal protrusion. (Warming method.)
Shujiao (Zanthoxyli Fructus) (omitted in one formula), Wutou (Aconiti Radix), Baiji (Bletillae Rhizoma) (each half *liang*).
Grind into powder. Wrap a *fangcun bi* (square-inch measure) in cotton, insert three *cun* into the vagina. Replace when warmth is felt in the abdomen, once daily. Reapply the next morning. Cure in seven days.
Cunjin San (Inch Gold Powder) treats female rectal prolapse. (Warming, astringent, and cooling; ingenious.)
Shechuangzi (Cnidii Fructus), Longnao (Borneolum Syntheticum), Huluba (Trigonellae Semen), Zixiaohua (Calostomatis Caulis) (equal parts).
Grind into powder. Take five to seven *qian* per dose, decoct with half a bowl of water, and pour over the affected area. Effective after two or three applications.
Fuyuan Tang (Restoring Primordial Decoction) treats postpartum uterine non-retraction.
Jingjiesui (Schizonepetae Spica), Huoxiangye (Agastaches Folium), Choukunpi (Ailanthi Cortex) (equal parts).
Chop, decoct in water, and fumigate/wash. The uterus will retract.
One source states: If the rectum protrudes before delivery and fails to retract afterward, it is called "Pan Chang Xian Hua Chan" (Coiled Intestine Offering Flower Birth).
Treatment: Have the mother lie supine. Mix half a cup of good rice vinegar with seven *ge* of fresh spring water. Stir well, then suddenly splash it onto the mother's face or back. Each splash induces contraction. After three splashes and three contractions, the intestine fully retracts. This is an excellent method.
Another source states: For prolapsed intestine, folk custom uses splashing cold water on the mother's face to induce natural retraction. This method must never be used for weak individuals, as fear may cause illness or complete collapse. Use this formula instead.
Take forty seeds of Bamianzi (Ricini Semen) (one source says fourteen), remove shells, mash, and apply to the crown of the mother's head. It will naturally retract. Once retracted, wash the Bamianzi (Ricini Semen) off the head with water.
Another method: For prolonged prolapse dried by wind, unable to retract.
Warm a small amount of knife-sharpening water over fire, moisten the prolapsed intestine, then brew a cup of good Magnetitum (Ci Shi) decoction for the mother to drink. It will naturally retract. (Symbolic intent.)
Danxi Formula: Treats postpartum intestinal non-retraction.
Heat five *jin* of Sesamum indicum Oil (Xiangyou), pour into a basin, wait until warm, and have the patient sit in the oil basin. After about the time it takes to eat one meal, blow Sijiao (Gleditsiae Sinensis Spina) powder into the nose. Sneezing immediately causes retraction. Ingenious. (Has methodology.)
Miscellaneous Formulas: Treat "Pan Chang Chan".
Banxia (Pinelliae Rhizoma) powder insufflated into the nose causes the intestine to retract upwards. (Principle of inducing sneezing.)
One source: Scorpion (Quanxie) powder, any amount. Hold water in mouth, insufflate into nose. Immediate effect.
One source: Dip a large paper wick in sesame oil, light it, blow out, and fumigate the mother's nose with the smoke. The intestine rises.
Miscellaneous Formulas: Treat postpartum vulvar swelling and prolapse.
Mix Tiejing (Ferrum) and Yangzhi (Adeps Sus) until thick. Wrap in cloth, heat, apply as hot compress, and push inward.
One source: Use two *liang* of Zhike (Aurantii Fructus), remove pith, chop, decoct in water, soak warmly. After a while, it retracts.
One source: Human urine mixed with Shaojiu (Fermented Rice Wine) taken orally, one *fangcun bi* per dose.
One source: Place the prolapsed intestine in a clean lacquerware vessel, immerse in strong decoction of Huangqi (Astragali Radix). The intestine rises.
One source: Heat Shed of Snake (Shetui) and Shechuangzi (Cnidii Fructus), wrap in cloth, apply as hot compress to the affected area. Also treats postpartum vaginal pain.
One source: Use one *sheng* of Shechuangzi (Cnidii Fructus) alone, heat, wrap in silk, apply as hot compress. Also treats postpartum vaginal pain.
One source: Burn rabbit head to ash, apply topically.
One source: Wash soft with warm water, then fumigate with smoke from burning male mouse feces (Xiongshufen).
Moxibustion Method: Treats postpartum vaginal prolapse. Moxa on the transverse crease below the umbilicus, 27 moxa cones.
One method: For women with vaginal prolapse, lethargy in limbs, and chest oppression, govern with Shaohai (HT3). (Alternatively, Zhaohai (KI6)).
One method: For women with prolapsed womb, constant dampness, and failure to retract, moxa Shenque (CV8), Yuquan (EX-CA1), and Yinjiao (CV7). (One finger-width below the umbilicus, 50 moxa cones, repeated three times).
Another method: Three *cun* lateral to Yuquan (EX-CA1). (Moxa according to age, repeated three times).
One method: For women with cold swelling of the vaginal opening, moxa Guilai (ST29). (30 moxa cones).
