(Attached: Jue Ni Syndrome)
Da Quan states: Postpartum blood syncope (Xue Yun, 血晕) occurs due to failed blood flowing into the Liver Meridian, causing blacking out of eyes and dizziness, inability to sit or stand, and even coma and unconsciousness. This is called blood syncope. Mixing Hei Shen San (Black Divine Powder, 黑神散) with fine wine is best. Treating it as An Feng (Dark Wind) or Zhong Feng (Wind Stroke) is a mistake. When blood-heat rises counterflow due to deficiency and attacks the Heart, causing coma and unconsciousness, and Qi closure approaching extinction, drinking Child's Urine (Tong Bian, 童便) is most excellent. However, there are three causes: fainting due to excessive mental or physical exertion; fainting due to excessive bleeding; and fainting due to insufficient bleeding. Although the symptoms of fainting are the same, the treatments differ, and careful examination is required. For fainting due to excessive bleeding, characterized by stupor and restlessness, use tonifying blood and clearing Heart medicines. For fainting due to insufficient bleeding, where lochia does not descend and rushes up to attack the Heart, causing fullness and urgency below the heart, coma, clenched jaw, and complete unconsciousness, use blood-breaking and blood-moving medicines. (This section lacks the treatment method for fainting caused by excessive mental/physical exertion; generally, the focus is on clearing the Heart, cooling blood, and tonifying, such as in the Bao Ming Ji Fang. Using Child's Urine or Ophiopogon (Maimen Dong, 麦门冬), Smoked Plum (Wumei, 乌梅) etc. is acceptable. Xue uses Bu Zhong Yi Qi Tang because it is due to labor; if due to mental exertion, Zhu Sha An Shen Wan is also excellent. Some doubt that Coptis (Huang Lian, 黄连) in the formula is unsuitable postpartum, which depends on the practitioner's judgment.) Ancient methods state: After delivery, prepare a weight stone, yellow stone, or hard charcoal, burn until red-hot, place in a vessel, and urgently pour vinegar over it near the bed; the vinegar vapor can eliminate blood syncope. Doing this occasionally within one month postpartum is beneficial.
Cui states: All fainting cases are due to deficiency-heat, surging blood and Qi, and emptiness in the abdomen. For those about to deliver, first apply strong vinegar to the mouth and nose, keep vinegar nearby for inhalation, and drink it slowly; this is the superior method. If feeling faint, wipe the face with vinegar; once recovered, drink vinegar, and give a little water to quench thirst. (Another version says give a little water to quench thirst.) Another method: Burn Dry Lacquer (Gan Qi, 干漆) to produce thick smoke and fumigate the mother's face to awaken her. (If dry lacquer is unavailable, burn an old lacquerware vessel to produce smoke, which is also excellent.) (Vinegar method consolidates spirit, water method clears heat, burning dry lacquer disperses blood confusion in the upper body; all are excellent.)
Guo Jizhong asks: What is postpartum blood syncope? Answer: Postpartum sudden deficiency of Qi and Blood, failure to settle, blood follows Qi upward, confusing the Heart Spirit, causing visual disturbances. In extreme cases, it causes suffocation, unconsciousness, clenched jaw, coma, and cold breath. Physicians who do not recognize this call it An Feng; treating it as such makes recovery difficult. Simply administering Qing Hun San (Clear Soul Powder, 清魂散) will restore consciousness.
Xue states: Postparture depletion of Yuan Qi, lochia attacking upward due to deficiency, causing blurred vision and dizziness, or chest fullness, coma, and clenched jaw, or phlegm obstruction, should be urgently treated with Shi Xiao San (Pain-Stopping Powder, 失笑散). If fainting due to excessive bleeding, or coma and restlessness, use a large dose of Xiong Gui Tang (Cyathula and Angelica Decoction, 芎归汤) to tonify, or Yun Tai Zi San, or Child's Urine. Add Er Chen Tang (Two-Cured Decoction, 二陈汤) if there is phlegm. If caused by mental/physical labor, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction, 补中益气汤) with Cyperus (Xiang Fu, 香附). If due to extreme Qi and Blood deficiency leading to unconsciousness, use Qing Hun San, or only Xiong Gui Tang and other major Qi and Blood tonics. For any delivery, fumigation with vinegar or lacquerware, or inserting pills made of Pinellia (Ban Xia, 半夏) powder mixed with cold water into the nostrils, prevents these ailments. Danxi states: Blood syncope is due to dual deficiency of Qi and Blood, with Phlegm-Fire rising. Use Er Chen Tang to guide out phlegm, or Modified Zhu Sha An Shen Wan (Cinnabar Calming the Spirit Pill, 朱砂安神丸), taken with Ophiopogon Decoction (Maimen Dong Tang, 麦门冬汤). Generally, postpartum facial paralysis involves major tonification of Qi and Blood, combined with phlegm treatment. If Spleen and Stomach deficiency fails to consolidate, use Liu Jun Zi Tang (Six-Gentlemen Decoction, 六君子汤). At five months, take An Tai Yin (Fetus-Calming Drink, 安胎饮). At eight to nine months, add Belamcanda Rhizome (Da Fu Pi, 大腹皮) and Buxus Seed (Huang Yang Nao, 黄杨脑). Near delivery, take Bao Sheng Wu You San (Protect Life and Ensure Safety Powder, 保生无忧散) (see previous Lin Yue Shu Tai Fang); prior ailments will not occur.
A family member's wife had postpartum lower abdominal pain, suddenly developed clenched jaw, was灌ed with Shi Xiao San, gave birth after a long time, and healed with Si Wu Tang (Four Substances Decoction, 四物汤) plus Pao Jiang (Roasted Ginger, 炮姜), Bai Zhu (Atractylodes Macrocephala, 白术), and Chen Pi (Tangerine Peel, 陈皮).
A parturient drank alcohol during childbirth (alcohol is not suitable for parturients), had excessive lochia, suffered from blood syncope, and exhaled alcohol odor. This is blood moving recklessly due to alcohol-heat, causing fainting due to deficiency. Use Fo Shou San (Buddha's Hand Powder, 佛手散) plus Ge Gen (Kudzu Root, 干葛) two qian; cured in one dose. Alcohol is fierce; it is unsuitable during pregnancy and postpartum, lest it cause the aforementioned syndrome. A crowded, noisy, and hot delivery room can also cause this syndrome.
Lu Yan from Fenghua treated Mrs. Xu from Xinchang who died suddenly postpartum, with only slight warmth in the chest and diaphragm. Lu diagnosed: This is blood stagnation/muffling. Use dozens of jin of Hong Hua (Safflower, 红花), boil in a large pot. When boiling, fill a wooden barrel. Place the patient on top to fumigate. When the vapor diminishes, replenish. After a while, the woman's fingers moved, and she revived the next day. This aligns with Xu Yinzong's treatment of Empress Dowager Wang.
Jingyue (Zhongjing) states: Postpartum Yu Mao (Depression and Vertigo), pulse is weak, vomiting, inability to eat, stool is hard, but only head sweats. The reason is Blood deficiency leading to Jue (Cold Extremities/Reversal), Jue leads to Mao (Vertigo/Clouding). To resolve Mao, profuse sweating must occur. Because Blood deficiency causes lower Jue, solitary Yang rises outward, hence head sweat. Therefore, postpartum women favor sweating; loss of Yin Blood leads to excess Yang, so sweating restores Yin-Yang balance. Due to hard stool and vomiting/inability to eat, Xiao Chai Hu Tang (Minor Bupleurum Decoction, 小柴胡汤) is primary; upon resolution, appetite returns. If fever recurs after seven to eight days, this is Stomach Excess, Da Cheng Qi Tang (Major Order the Qi Decoction, 大承气汤) is primary.
Current commentary: Yu Mao is synonymous with Yun (Syncope). From this, postpartum blood syncope has three methods: Sweating, Purging, and Harmonizing. One must distinguish between Exterior/Interior and Deficiency/Excess precisely.
Qing Hun San (Clear Soul Powder): Postpartum blood syncope, sudden Qi and Blood deficiency, unrest, blood follows Qi upward, confusing Heart Spirit, causing visual disturbances. In extreme cases, suffocation, unconsciousness, clenched jaw, coma, cold breath. First, fumigate with Dry Lacquer (Gan Qi, 干漆) or lacquerware smoke via the nose, frequently stay in a room with vinegar and hot coals, then administer this medicine to awaken.
Ze Lan Ye (Eupatorium Leaf), Ren Shen (Ginseng) (each 2.5 qian), Chuan Xiong (Ligusticum) (0.5 liang), Jing Jie Sui (Schizonepeta Spike) (1 liang), Gan Cao (Licorice) (2 qian).
Grind to fine powder. Take 2 qian per dose, mix with half a zhan of warm wine and half a zhan of hot water, or mix with Child's Urine and force-feed rapidly. Upon swallowing, eyes open, Qi settles, and consciousness returns.
Xiong Gui Tang (Cyathula and Angelica Decoction): Treats postpartum excessive blood loss, coma/unconsciousness.
Chuan Xiong, Dang Gui (Angelica Sinensis, 当归) (remove radicle, wash in wine, dry, equal parts).
Chop, take 4 qian per dose, decoct in water, take warm, anytime. If stabbing pain in abdomen, add wine-fried Bai Shao (White Peony Root, 白芍药); severe cases add Gui Xin (Cinnamon Heart, 桂心). If dry mouth and irritability/thirst, add Wu Mei (Smoked Plum, 乌梅), Mai Men Dong (Ophiopogon, 麦门冬). If chills and fever, add Gan Jiang (Dried Ginger, 干姜), Bai Shao. If fluid retention below heart with slight nausea, add Fu Ling (Poria, 茯苓), Sheng Jiang (Fresh Ginger, 生姜). If deficiency-irritability and insomnia, add Ren Shen, Zhu Ye (Bamboo Leaf, 竹叶).
Bao Ming Ji Fang: Treats critical postpartum blood syncope due to excessive bleeding and Blood deficiency; tonify.
Dang Gui (Stir-fried), Chi Shao Yao (Red Peony Root, 赤芍药) (stir-fried, each 1.5 qian), Sheng Di Huang Zhi (Fresh Rehmannia Juice, 生地黄汁) (one large zhan).
Cut, decoct in water for three to five boils, take warm. If feeling irritability/heat, remove Dang Gui (Dang Gui is acrid-warm, unsuitable for heat/irritability), add half a zhan of Child's Urine, take.
The above treat diseases of fainting due to excessive blood loss.
Cu Mo (Vinegar-Quenched Ink): Take immediately after birth to prevent fainting.
Song Yan Mo (Pine Soot Ink) or Jing Mo (Capital Ink), amount unspecified, burn red with charcoal, quench in Rice Vinegar (Mi Cu, 米醋), repeat quenching seven times, grind extremely fine. Immediately after birth, take 1-2 qian, mix with Child's Urine. Light vinegar soup or warm wine is also acceptable. (Wine is inferior to vinegar, vinegar is inferior to Child's Urine).
Du Xing San (Single Traveler Powder): Treats postpartum blood syncope, coma/unconsciousness, rushing to heart causing suffocation.
Wu Ling Zhi (Trogopterus Dung, 五灵脂) half raw half stir-fried, grind to powder. Take 2 qian per dose, mix with warm wine and force-feed. Cures upon entering throat. If not cured, add equal parts of stir-fried Pu Huang (Lygodium Spore, 蒲黄), named Shi Xiao San (Pain-Stopping Powder, 失笑散). One prescription adds equal parts Jing Jie, mix with Child's Urine.
Duo Ming San (Life-Seizing Powder): Treats postpartum blood syncope, blood entering Heart Meridian, incoherent speech, forgetfulness, loss of will, and hundred diseases postpartum.
Xue Jie (Dragon's Blood, 血竭), Mo Yao (Myrrh, 没药) (equal parts).
Grind to powder. Immediately after birth, mix half a zhan of Child's Urine and half a zhan of fine wine, boil for one or two boils, mix and take 2 qian. Wait a while, then take again. The failed blood descends naturally, no longer rushing upward. Or simply mix with plain water.
Si Wei San (Four-Ingredient Powder): Treats all postpartum diseases, especially effective if taken immediately after delivery.
Dang Gui, Yan Hu Suo (Corydalis, 玄胡索), Xue Jie, Mo Yao (equal parts).
Grind to fine powder. Take 2 qian per dose, decoct one zhan of Child's Urine to six parts, take orally. If Cold in Heart/Diaphragm, double Dang Gui. If Qi oppression and panting, double Yan Hu Suo. If lochia is slow, double Xue Jie. If cramping pain in Heart/Abdomen, double Mo Yao.
Hei Shen San (Black Divine Powder): Treats incomplete lochia expulsion, retained placenta, Qi/Blood attacking upward, chest/abdominal fullness, or firm/swollen/cramping pain in navel/abdomen, as well as blood syncoma with coma, blacking out, clenched jaw, and various postpartum blood stasis diseases.
Dang Gui, Shao Yao (Peony), Shu Di Huang (Prepared Rehmannia, 熟地黄), Gan Jiang (Roasted Dried Ginger, 干姜), Gui Xin, Pu Huang, Gan Cao (Roasted, each 4 liang), Hei Dou (Stir-fried, peeled, half sheng).
Grind to fine powder. Take 2 qian per dose, mix with half a zhan of wine and half a zhan of Child's Urine, decoct together and take. Lou states: Hei Shen San is suitable for Cold patterns and Autumn/Winter seasons. For impatient, thin constitutions and Summer months, use with caution.
Hong Hua San (Safflower Powder): Treats postpartum blood clouding, blood syncope, blood flooding, irregular menstruation, and old dry blood Qi.
Gan He Ye (Dried Lotus Leaf, 干荷叶 - good at breaking blood), Mu Dan Pi (Moutan Cortex, 牡丹皮), Chuan Gui (Angelica), Hong Hua, Pu Huang (Stir-fried, equal parts).
Grind to fine powder. Take half liang per dose, decoct in wine with dregs, take warm. If retained placenta, mix half liang with Yu Bai Pi (Mulberry Root Bark, 榆白皮) soup; immediate effect.
Lu Jiao San (Deer Horn Powder): Treats postpartum blood syncope, caused by deficiency-fire carrying blood upward gradually. Burn Deer Horn (Lu Jiao, 鹿角) to ash to remove fire toxicity, grind extremely fine. Mix with good wine and Child's Urine, force-feed. One sip awakens. This herb moves blood. (Tonifying formula, excellence lies in administration method).
Yu Jin San (Curcuma Powder): Treats postpartum blood rushing to heart causing death, and expels fetus.
Yu Jin (Curcuma Longa, 郁金) burned to retain nature, grind to powder. Take 2 qian, mix with strong vinegar one he, force-feed; immediate revival.
