平复而风冷客之,冷气乘腰,故令腰痛也。若寒冷邪气连滞背脊,则痛久未已,后忽有娠,
必至损动,盖胞络属肾,肾主腰故也。

The *Daquan* states: The Kidney governs the waist and feet. Postpartum lower back pain occurs because a woman's Kidney is anatomically connected to the uterus (Bao Gong). Childbirth labors and injures Kidney Qi, damaging the uterine collaterals. If wind-cold pathogens invade before the deficiency has recovered and stabilized, cold qi attacks the waist, causing lower back pain. If cold pathogenic factors linger in the spine, the pain persists for a long time. If pregnancy occurs subsequently, it will inevitably cause damage, because the uterine collaterals belong to the Kidney, and the Kidney governs the waist.
Xue notes: For the aforementioned symptoms caused by deficiency of True Qi with pathogenic invasion, use Danggui Huangqi Tang (Angelica and Astragalus Decoction) or Shiquan Dabu Tang (Ten Complete Tonifying Decoction) as the principal formula, assisted by Sangjisheng Tang (Mulberry Mistletoe Decoction). If there is no response, add Fuzi (Prepared Aconite Root) to Shiquan Dapu Tang.
A postpartum patient presented with lower back pain, abdominal distension, frequent belching, and vomiting after taking various medicines. I diagnosed this as Spleen deficiency and Blood weakness. I used only Baizhu (Atractylodes Macrocephala Rhizome), stir-fried until yellow, one liang per dose, decocted in rice washing water. She drank one spoonful at a time. After four doses, she gradually stabilized; after more than one hundred doses, she was cured. (It is ingenious that when all medicines induce vomiting, only Baizhu is used, with specific dosage, decoction method, and administration instructions.)
Danggui Huangqi Tang: Treats excessive blood loss after childbirth, lower back pain, body heat, and spontaneous sweating.
Danggui (Angelica Sinensis Radix, two liang), Huangqi (Astragali Membranacei Radix), Baishao (Paeoniae Radix Alba, stir-fried, each two liang).
Chop the herbs. Take six qian per dose, add five slices of Shengjiang (Zingiberis Rhizoma Recens), decoct one and a half cups of water to one cup. Take warm at any time.
Xuanhu Siwu Tang: Treats abdominal pain due to blood stasis and stabbing lower back pain due to blood stagnation.
Danggui (Angelica Sinensis Radix), Chuanxiong (Ligusticum Chuanxiong Rhizoma), Baishao (Paeoniae Radix Alba), Shudi (Rehmanniae Radix Preparata, each seven and a half qian), Xuanhusuo (Corydalis Rhizoma, wine-boiled, two liang).
Grind into fine powder. Take three qian per dose, mix with wine and swallow.
Rushen Tang: Treats lower back pain due to blood stasis after childbirth.
Xuanhusuo (Corydalis Rhizoma), Danggui (Angelica Sinensis Radix), Guixin (Cinnamomi Cortex, equal parts).
Decoct in water and take orally.
You Rushen Tang (Shengliao Wuji San modified with Taoren): Expels retained blood, dispels wind-dampness.
See the formula for Wuji San (Five Accumulations Powder) under the previous section on abdominal pain.
Guangji Fang: Treats postpartum deficiency-cold, where blood and qi flow into the waist and legs, causing severe pain preventing movement.
Baijiang (Patriniae Herba, suitable for blood coagulation, though difficult to source), Danggui (Angelica Sinensis Radix, each eight fen), Chuanxiong (Ligusticum Chuanxiong Rhizoma), Shaoyao (Paeoniae Radix), Guixin (Cinnamomi Cortex, each six fen).
Chop the herbs. Decoct in two sheng of water to obtain eight ge. Divide into two warm doses. Avoid scallions.
Sangjisheng Fangfeng Tang: Treats postpartum dizziness due to wind-pathogen, lower back pain preventing turning or side-bending, heaviness in the four limbs, and difficulty walking. (This formula treats chronic pain caused by external contraction.)
Duhuo (Angelicae Pubescentis Radix), Chuanxiong (Ligusticum Chuanxiong Rhizoma), Shaoyao (Paeoniae Radix, stir-fried until yellow), Guixin (Cinnamomi Cortex), Xuduan (Dipsaci Radix), Shengjiang (Zingiberis Rhizoma Recens), Sangjisheng (Taxilli Cacumen, each six fen), Danggui (Angelica Sinensis Radix), Fangfeng (Saposhnikoviae Radix, each eight fen).
Cut the herbs. Decoct in water and take orally.
Taoren Tang: Treats cases where lochia (E Lu) initially flows but suddenly decreases, stops intermittently, accompanied by heavy stabbing pain in the waist or radiating to the thighs. This is due to blood stagnation in the collaterals. If not treated immediately, abscesses may form. Use Taoren Tang. To prevent abscess formation, pre-administer Wuxiang Lianqiao Tang.
Taoren (Pruni Persicae Semen, peeled and removed of tips), Sumu (Sappan Lignum), Shengdi (Rehmanniae Radix, each half liang), Mengchong (Blaberidae Insect, stir-fried, wings and feet removed), Shizhi (Hirudo, stir-fried, thirty each).
Take three qian per dose, decoct in water, take hot on an empty stomach. Stop taking once lochia is discharged.
Wuxiang Lianqiao Tang: Treats postpartum blood stasis causing lower back pain and abscess formation.
Muxiang (Aucklandiae Radix), Dingxiang (Caryophylli Flos), Chenxiang (Aquilariae Lignum Resinatum), Ruxiang (Olibanum), Shexiang (Moschus), Shengma (Cimicifugae Rhizoma), Duhuo (Angelicae Pubescentis Radix), Sangjisheng (Taxilli Cacumen), Lianqiao (Forsythiae Fructus), Mutong (Akebiae Caulis, each two liang).
Grind into coarse powder. Take five qian per dose, decoct in water, add a small amount of Zhuli (Bambusa Textilis Culmis Succus) and take. (Since this is blood stasis, why use only five aromatic herbs to move Qi? It is likely because the pathology involves Qi affecting Blood. The dosage of Shexiang may be erroneous.)
