All versions lack the character "Pian" (Chapter).
[The text regarding] Zi (Canthus) below is Ne [Inner Canthus]. Yun says: Zi refers to the canthus. The outer part of the eye, determined by the face, is called Rui Zi (Sharp Canthus). (Commonly known as the outer canthus). The part inside near the nose is called Nei Zi (Inner Canthus). (Commonly known as the inner canthus). The upper eyelid belongs to the outer canthus. The lower eyelid belongs to the inner canthus. Zhong Zhi-nong notes: Taiyang governs the upper lid of the eye; Yangming governs the lower lid. The upper being the outer canthus and the lower being the inner canthus means that the Qi of Taiyang and Yangming governs the upper and lower eye canthi respectively. Brief note: This section is irrelevant to Dian-Kuang (Mania-Epilepsy); it must be a fragment of an ancient classic.
Dian-Ji (Mania-Epilepsy) begins to arise... Yangming and Taiyin. Zhang Yun: First, there is a sense of unhappiness, indicating the spirit and mind are about to become disordered. Heavy pain in the head, redness when looking up or raising the eyes, indicates Jue Qi (Rebellious Qi) moving upward. When severe, extreme exhaustion leads to vexation in the heart and restless agitation. These are all omens of impending Dian-Ji. Yan refers to the forehead (Tian-Ting). Observing the Yan, pathogenic colors will necessarily appear here. One should select points such as Zhizheng (SI8), Xiaohai (SI81) of the Hand-Taiyang channel; Pianli (LI6), Wenliu (LI7) of the Hand-Yangming channel; and Taiyuan (LU9), Lieque (LU7) of the Hand-Taiyin channel, to drain pathogenic blood, stopping needling only after the blood color changes. *Jia Yi Jing*: There is a character "Zhi" (Straight) above "Shi" (Look).
When blood is excessive, leading to extreme exhaustion and then heart vexation. All versions read "stop when blood changes," but for the onset of Dian-Ji, this should be amended.
Pulling the mouth and crying out... stop when blood changes. Zhang Yun: Pulling the mouth refers to traction and deviation. It may manifest as crying out or panting and palpitations. One should observe the Hand-Yangming and Hand-Taiyang channels, locate the disease, and needle accordingly. Points are as mentioned before. "Qiang" (Strong/Stiff) means stiffness. Traction on both left and right sides suggests the disease is mostly in the collaterals; thus, if the left is stiff, attack the right; if the right is stiff, attack the left. Stop needling only after the blood color changes. This is the method of Miao-Ci (Mysterious Pricking).
Foot Taiyang, Yangming, Taiyin, Taiyang. In all versions, the character "Shou" (Hand) appears after Yin. "Xia Taiyang" (Lower Taiyang), Wu Ben reads "Hand Taiyin," which is incorrect. Zhang Yun: Points include Weiyang (BL39), Feiyang (BL58), Pucan (BL60), Jinmen (BL63) of Foot-Taiyang; Sanli (ST36), Jiexi (ST41) of Foot-Yangming; Yinbai (SP1), Gongsun (SP4) of Foot-Taiyin, etc., all govern this. Hand-Yangming channel points are the same as before.
Place its blood in a gourd vessel... Liao Bone. Zhong Zhi-nong notes: Hu-Hu refers to a gourd. Placing its blood in the gourd, if the blood moves alone when the time comes, it is due to the mutual induction of Qi. If Jue Qi attacks the Hand-Taiyin and Hand-Taiyang, the blood in the gourd moves alone, responding to the movement of Heavenly Qi (Taiyang). If it does not move, the disease has entered into Earth-Water; therefore, one should moxa the Liao-Bone for twenty moxibustion units (Zhuang). The Classic states: "If sinking, moxa it." This disease sinks into Foot-Taiyin and Taiyang; thus, one should moxa the Liao-Bone of Foot-Taiyang. Ma Yun: The point name for Liao-Bone is Changqiang (GV1).
Liao Teeth and various flesh divisions... untreatable. *Jia Yi Jing*: Liao, written as Han (Chin/Jaw); Ju, written as Ju (Oblique/Angled). Ma Yun: Liao, pronounced Ku-Gan. Old interpretations took it as rising from hunger with yellow complexion. However, this chapter and the Miscellaneous Diseases chapter state "Liao pain has a fixed location," suggesting Han and Liao are interchangeable. Qu Yuan's *Li Sao* says: "What harm is there in a long chin/jaw?" It can also be a general term. Zhang Yun: The definition of Liao as hungry with a yellow face does not align with the Classic's intent. *Dong Shu* chapter says: "From the brain, Liao exits below Ke-Zhu-Ren," so this should be below the brain, before the temple, above Ke-Zhu-Ren. Its Liao is above the temple bone, between the two temples. Gu Dian-Ji (Bone Mania-Epilepsy) means the disease is deep in the bone. Between the Liao, teeth, and various flesh divisions, pathogenic Qi is congested and closed, causing distension. The body becomes emaciated, only the bones remain prominent. Sweat comes out externally, while vexation and oppression occur internally; this is already a critical condition. If there is much vomiting of frothy saliva and Qi leaks downward, it indicates failure of both Spleen and Kidney, making it untreatable. Zhong Zhi-nong notes: Liao, Ye-Kan, means face. Fen-Rou Xi-Gu (Muscle valleys) belong to the bone. Gu Ju (Bone dwelling) means flesh and bone are not close. Brief note: Liao matches Ma and *Jia Yi*, which is correct. *Kaogong Ji* states: "The straight edge of the square ruler is Ju." Gu Ju implies stiffness/straightness; one should follow *Jia Yi*.
Body fatigue, curling, spasm, and large pulse. *Jia Yi* reads "Body curling, spasm, and large pulse." Brief note: Ma and Zhang both say: "Its pulse is urgent and large"; *Jia Yi* is correct.
Sudden fall... untreatable. Zhang Yun: Bao-Pu (Sudden Fall) means sudden collapse. Zong means flaccidity. To treat this: if the pulse is full and distended, pierce all to release blood. If the pulse is not full, moxa Tianzhu (BL10) and Dazhu (BL11) of the Foot-Taiyang channel along the neck. Also moxa Daipmai (GB26) of the Foot-Shaoyang channel. This point is about three cun away from the waist. Various flesh divisions: *Ben Shu* refers to the spaces between muscles of all channels and the transport points of the limbs. Wherever there is distension and flaccidity, these should be selected. Wang Shi's *Zheng Zhi Zhun Sheng* states: Qi leaking downward causes righteous Qi from the Kidney to vacate downward, hence death. Dian manifesting like Kuang (Mania) occurs because Heart-Yang cannot overcome rebellious Yin-Qi, causing the Spirit-Mind to scatter and Yang-Qi to suddenly cease, hence resembling mania. It is like a lamp about to go out shining brightly.
Kuang (Mania) begins to arise... Yangming. *Jia Yi*: Adds "Zhi" (Of) before "Shi" (Start). Bitter anger turns into frequent anger. Zhang Yun: Lack of Spirit leads to sorrow; injury to Hun leads to madness and lack of refinement; injury to Zhi leads to forgetfulness of previous words. Liver overcoming Spleen causes bitter anger; Blood deficiency leads to fear. All result from worry and hunger, damaging organ Qi. Select Taiyuan (LU9), Lieque (LU7) of Hand-Taiyin; Pianli (LI6), Wenliu (LI7) of Hand-Yangming; Yinbai (SP1), Gongsun (SP4) of Foot-Taiyin; Sanli (ST36), Jiexi (ST41) of Foot-Yangming, etc., all can treat it. Stop needling only after the blood color changes.
Kuang starts to erupt. Ma Yun: The previous section spoke of "beginning to arise," while this says "starts to erupt," implying the disease is formed and erupting.
Under the tongue, Shaoyin. Zhang Yun: Hand-Yangming, Taiyang, and Taiyin channel points are as before. Under the tongue is Lianquan (CV23) of the Ren Mai. Shaoyin refers to Shenmen (HT7) and Shaochong (HT9) of the Heart Channel. Brief note: Hand-Taiyang was not mentioned earlier for selection; Zhang likely made an error. "Under the tongue, Shaoyin" likely refers to Lianquan (KI23? Note: Text says Zu Shao Yin Lianquan, but Lianquan is CV23. The text might imply a connection or error in attribution in the commentary, but I translate faithfully to the source text's claim): Likely refers to Zu Shao Yin (Foot-Shaoyin) Lianquan point. (Note: Anatomically Lianquan is Ren Mai, but commentary links it here).
Bu Sheng (Not Excessive) means releasing it. *Jia Yi*: Adds "Zhe" (One who) after "Sheng". Ma Yun: If not excessive, release it without selecting. Zhang Yun: Should be released regardless.
Kuang Yan (Mad speech) Jing (Fright). *Jia Yi*: Yan, written as Xi (Joy). Correct.
Generated by Shao Qi (Deficient Qi). Zhang Yun: Qi decline leads to timid spirit, hence hallucinations and startled cries.
Two Liao. *Jia Yi*: Liao, written as Han (Chin/Jaw). Brief note: Commentators do not annotate acupoints.
Kuang patients eat much... those not manifesting outwardly. Zhang Yun: Eating much and seeing ghosts, smiling secretly, caused by excessive joy damaging the Spirit. *Nan Jing* states: "Those losing Yang see ghosts; those losing Yin go blind." Zhong Zhi-nong notes: Not manifesting outwardly means cold laughter without sound. Heart-Qi deficiency causes cold laughter; Heart-Qi excess causes loud laughter.
Wei Ying Ru Ci Zhe (Not yet like this). Zhang Yun: Refers to newly onset Kuang disease, lacking the manifestations described in the previous five sections.
Ququan. Brief note: This point belongs to the Jueyin Liver Channel, seen in *Ben Shu* chapter. *Jia Yi* and other books do not mention arterial movement. Only *Wai Tai Mi Yao* states: Two cun lateral to the tibia, at the pulse.
Yi Fa Qu Zhi (Select according to method). Ma Yun: As previously placing blood in the gourd to verify. Zhang Yun: If it does not resolve, seek methods from the previous five sections to select points.
Moxa Liao Bone twenty Zhuang. *Jia Yi* reads "Moxa Liao Bone twenty Zhuang." Liao Bone refers to the tail curvature. Zhong Zhi-nong notes: Liao Bone is where the Du Mai circulates. Du Mai meets Liver Mai at the head and neck; thus, moxa Liao Bone to draw Jueyin Meridian Qi and let it dissipate from below.
Feng Ni (Wind Reversal)... select Jing and Well points. Zhang Yun: Wind sensed externally, Jue Qi reverses internally, this is Feng Ni. Shen Le-Le means cold shivering of skin and hair. Xi-Ran means occasional cold air swallowing, gasping and clenching teeth. Hungry leads to vexation; full leads to restlessness, wind pathogen reversing internally. Hand-Taiyin interior-exterior are Lung and Large Intestine. Foot-Shaoyin is Kidney. Foot-Yangming is Stomach. Qing means cold. Selecting Ying, Jing, and Jing-Stream points refers to points of these four channels. Le, pronunciation Lei. Xi, pronunciation Xi. Zhong Zhi-nong notes: Wind excess without disease causes sudden swelling. Limbs Le-Le, cold-dampness. Xi-Ran, appearance of cold trembling. This is wind moving water-cold Qi showing these symptoms. Brief note: Le-Le implies gathering of water, seen in Mu Hua's *Hai Fu*. This describes sudden swelling of limbs. Zhang's note on miscellaneous diseases: "Lips Le-Le" means swollen appearance. Same for body Le-Le below. Xi, likely sighing Xi. Sighing also implies fear, hence describing cold shivering.
Zu Bao Qing (Feet suddenly cold)... warm then drain it. *Jia Yi*: Fan, written as Ao. No small character. All versions read Huan as Nuan (Warm), should amend. Zhang Yun: Zu Bao Qing means sudden cold. Chest feels like splitting, intestines like knife cutting, distress and pain. Vexation prevents eating, Qi rebels internally. Pulse large and small are all choppy, pathogen rebels in channels. If body is warm, select Foot-Shaoyin to drain it. If body is cold, select Foot-Yangming to tonify it. Note: Foot-Shaoyin points Yongquan (KI1), RanGu (KI2); Foot-Yangming points LiDui (ST45), NeiTing (ST44), Jiexi (ST41), FengLong (ST40). All govern Jue Ni. Brief note: Chest like splitting, intestines like cutting, is severe abdominal distension. *Jia Yi* is correct.
Below chest two ribs... immediate relief is it. *Jia Yi*: Er Xie (Two Ribs), written as San Lei Jian (Three Rib Spaces). Movement between hands has response character. Zhang Yun: Below chest two ribs refers to between the lower chest and left/right second ribs. Likely Zhangmen (LR13) and Qimen (LR14) of Foot-Jueyin. Ask patient to cough; if pulse moves and responds to hand, these are the points. Also select Bei Lu (Back Spine). Press with hand; if disease immediately relieves, this is the puncture site. Foot-Taiyang channel is between Lung and Diaphragm. Zhong Zhi-nong notes: Chest below two ribs are arterial sites of Zhongfu (LU1) and Yunmen (LU2) of Hand-Taiyin. Brief note: If "Chest below" refers to Zhongfu/Yunmen, it is unstable.
Nei Bi Bu De Sou (Internal closure unable to urinate)... use long needle. Zhang Yun: The following four sections discuss Jue Ni complications. Internal closure unable to urinate means disease in Water Organ; thus, puncture Yongquan (KI1), Zubin (KI9) of Foot-Shaoyin; Weiyang (BL39), Feiyang (BL58), Pucan (BL60), Jinmen (BL63) of Foot-Taiyang. Liao Shang is above the coccygeal bone of Du Mai, point name Changqiang (GV1). Puncture with long needle, the eighth needle.
Qi Ni (Qi Reversal)... meridian of movement. Zhang Yun: Taiyin Spleen channel, select Yinbai (SP1), Gongsun (SP4). Yangming Stomach channel, select Sanli (ST36), Jiexi (ST41). Jueyin Liver channel, select Zhangmen (LR13), Qimen (LR14). Severe cases additionally select Shaoyin and Yangming. "Meridian of movement" means observing the diseased meridian and puncturing it. The above two sections are recorded in *Jia Yi* under the chapter "San Jiao Constraint Internal Closure Manifesting Unable to Pass Urine and Feces."
Shao Qi Shen Le-Le (Deficient Qi Body Le-Le)... tonify Foot-Shaoyin. Ma Yun: Body Le-Le means unconfined; Yan Xi-Xi means unconnected breath. Zhang Yun: Shen Le-Le means cold shivering. Yan Xi-Xi means timid Qi. These indicate Essence deficiency failing to transform Qi; thus, tonify Foot-Shaoyin Kidney channel. Zhong Zhi-nong notes: Qi not moistening leads to heavy body.
Duan Qi (Shortness of Breath)... remove blood collaterals. Zhong Zhi-nong notes: Shortness of breath means Qi rises and shortens, so breath is short and disconnected. With movement, Qi depletes further. Zhang Yun: This is also Qi deficiency, suitable for Kidney tonification. But if blood collaterals are observed, they should be removed. Note: These two sections belong to Qi deficiency. Not tonifying Hand-Taiyin but tonifying Foot-Shaoyin shows that Yang roots in Yin, Qi transforms from Essence. Treatment must seek the root, visible here. Needling and medication follow the same principle. Brief note: The above six sections, Ma and Zhong attribute to Dian-Kuang, which is incorrect. Feng Ni and the following three sections, Zhang considers Jue Ni complications. However, inferring from *Jia Yi*, chapters have different meanings, nor must they be Dian-Kuang or Jue Ni.
