[Diagram of Lock-Throat Wind]
According to this syndrome, it is caused by the accumulation and stagnation of wind-heat. It may also arise from excessive alcohol consumption, sexual indulgence, or physical labor, leading to pathogenic factors generating in the throat, resembling a lock around the throat. There are unilateral and bilateral presentations. For bilateral cases, treatment must first involve probing for and removing wind-phlegm. If there are no obvious visible traces on both sides, but only whiteness, with severe pain making eating difficult, this condition is named "Lock-Throat Wind" (Suo Hou Feng). If trismus (lockjaw) occurs, first use Tong Guan San (Passing Through Powder) insufflated into the nasal cavity to open the mouth. Subsequently, needle the Shaoshang (LU11) acupoint. Use six needles within the throat. After blowing air to extract phlegm, insufflate the secret formula. Treatment should be prompt. If phlegm-drool is obstructive, causing opisthotonos (backward arching of the body), wash the affected area with warm water and apply warmth. For severe cases, administer San Huang Tang (Three Yellows Decoction), or use Gan Jie Tang (Licorice and Platycodon Decoction) added with Jing Jie (Schizonepeta), Fang Feng (Saposhnikovia), and Yin Hua (Honeysuckle), which will lead to recovery.
For Lock-Throat with Trismus, use Tong Guan San; after Zhui Feng San (Wind-Chasing Powder), use knife-needles. After needling Shaoshang, needle six additional points. After pus and blood are removed, insufflate the Secret Divine Medicine. Take San Huang Tang plus Gan Jie Tang; adding Jing Jie, Fang Feng, and Yin Hua makes it more effective.
[White Coiling Throat Wind]
This syndrome arises from long-standing accumulated heat, damage to the stomach from fried and roasted foods, or exposure to wind and qi disturbance due to labor. Symptoms include redness in the whites of the eyes and ears, facial swelling, and purple discoloration spreading downward from the cheeks. Patients may die within hours; if delayed for one or two days, it becomes a slow wind syndrome. First, needle Shaoshang (LU11), use Zhui Feng San to extract phlegm, then insufflate the primary medicine onto the swollen area. Perform knife-needle therapy to drain pus and blood. Insufflate the secret formula multiple times, then take San Huang Tang (Three Yellows Decoction) with increased amounts of Jing Jie (Schizonepeta), Fang Feng (Saposhnikovia), and Yin Hua (Honeysuckle). If symptoms include lying supine with back arched, face down, limbs extended, delirious speech, opisthotonos, and inability to open the mouth:
First insufflate Tong Guan San. If sneezing occurs from the nose, the mouth opens. If not, wind-fire is excessive; insufflate one or two more times, wait a while until natural sneezing occurs, then the mouth opens. Use scissors to pry open, insert a ring-shaped object laterally to press the tongue, insufflate Zhui Feng San to extract phlegm, insufflate the primary medicine at the lesion, perform knife-needle therapy, then insufflate the secret formula, and take San Huang Tang.
Insufflate secret medicine for White Coiling Throat Wind with swelling, trismus, and difficulty speaking; redness and swelling on both cheeks. Limbs extended, face down, sighing leads to immediate death. First probe wind-phlegm and needle four points, then insufflate primary medicine and pierce the throat. Use secret insufflation and take San Huang San (Three Yellows Powder); internal support for pus ensures recovery.
[Yellow Coiling Throat Wind]
The etiology and treatment methods are the same as previously described. When pus forms behind the di ding (tonsillar pillar/uvula area), use a hook knife to puncture and drain the pus, then insufflate the secret formula. Take two doses of San Huang Tang (Three Yellows Decoction). Next, insufflate the primary secret formula equally, adding Bing Pian (Borneol) and She Xiang (Musk). If the tissue is ulcerated, insufflate Sheng Ji San (Flesh-Generating Powder). If pus does not drain, take Qian Jin Nei Tuo San (Thousand Gold Internal Support Powder) with added Yin Hua (Honeysuckle). Since the di ding is vital to the body, knife-needles and cautery must never be used recklessly. For ordinary carbuncles, only insufflation medicines should be used, taking San Huang Liang Ge San (Three Yellows Clearing Diaphragm Powder). If the di ding is purple and swollen, Gan Jie Tang (Licorice and Platycodon Decoction) is excellent.
[Swelling Cheek Carbuncle Diagram]
This syndrome is caused by cold-dampness and wind-toxicity, generating swelling below the cheeks, either on one or both sides. If the inside of the mouth is swollen, insufflate the secret formula, use a knife to release blood, and drain pus over time. In the early stage, with headache, fever, and aversion to cold, take Jing Jie Fang Feng Bai Du San (Schizonepeta and Saposhnikovia Detoxifying Powder), followed by Xue Shi Tang (Scholar's Decoction). If it persists without resolving, take Qian Jin Nei Tuo San (Thousand Gold Internal Support Powder), externally apply Jin Gu San (Golden Hoop Powder), or use fire-needles to drain pus; applying ointment leads to self-healing.
Swelling cheek wind-toxicity arises from wind-heat; swelling and nodules on both cheeks. Severe intraoral swelling requires needle-bloodletting. Early headache and chills-fever make Jing Jie Fang Feng Bai Du San indispensable. Ying Zhou Xue Shi Tang and Qian Jin Nei Tuo, along with the wonderful secret of applying Jin Gu Powder.
[Coiling Tongue Throat Wind]
This syndrome results from wind-heat-dampness toxicity, or arises from wind-labor and excessive alcohol/sex. The chin swells, trismus occurs, the tongue curls and swells, with tendons visible like earthworms, and yellow spiky lesions with white coating appear. If teeth cannot bite open, needle Shaoshang (LU11) to bleed, which is treatable. However, if white-yellow fluid drains, it is untreatable. Treatment involves first probing for wind-phlegm, scraping the tongue coating, insufflating the primary medicine, using a knife to pierce blue veins to bleed, and needling Yu Ye (Jiacong EX-HN12) and Jin Jin (Jiacong EX-HN13) acupoints. Bleed, rinse mouth thoroughly, insufflate the secret formula, and take San Huang Tang (Three Yellows Decoction) and Liang Ge San (Clearing Diaphragm Powder); recoverable in one to two days. If pus forms after many days, take Qian Jin Nei Tuo San (Thousand Gold Internal Support Powder). For wooden tongue that is short and large, insufflate Zhui Feng San (Wind-Chasing Powder). (Head shaking indicates untreatability; black hard masses are difficult to treat; mortality is high. First needle four points...)
Coiling Tongue Throat Wind with chin swelling begins; scrape tongue coating and pierce blue veins. Probe wind-phlegm and insufflate secret medicine, then needle Yu Ye and Jin Jin. Trismus and yellow-black tongue; bleeding from Shaoshang lightens the condition. San Huang Liang Ge San is suitable initially; use Qian Jin Nei Tuo when pus forms.
[Running Horse Throat Wind]
This syndrome arises from eating hot foods, drinking alcohol, or wind exposure. First needle four points to determine life or death, insufflate Zhui Feng San (Wind-Chasing Powder) to extract phlegm, and insufflate the secret formula into the throat and rotten tooth areas. Rub Zhui Feng San on the curled tongue, or insufflate the primary medicine on the tongue. Needle three points under the tongue; do not needle if the tongue is not curled. Take Er Chen Tang (Two Aged Medicaments Decoction), San Huang Tang (Three Yellows Decoction), and Liang Ge San (Clearing Diaphragm Powder), with added Jing Jie (Schizonepeta) and Fang Feng (Saposhnikovia). If head shaking, tooth loss, head swelling, and carbuncle rupture occur, all are untreatable. If speech is clear and the patient is young and strong, treatment is possible. Use [specific substance] processed by frying to ash, insufflate to stop pain. Then take juice from [same substance], mix with Zhui Feng San for gargling, and take San Huang Tang with Jing Jie and Fang Feng. The first dose can be used; subsequent doses cannot. Remember this.
Running Horse Throat Wind generates under the tongue, swift as a running horse entering the throat. First needle four points, then insufflate secret medicine. Gargle with [substance] juice and Zhui Feng San. Needle three points under the heart-tongue; expectoration clears qi flow. Er Chen and Liang Ge San are still suitable; head shaking and black carbuncles result in emptiness/death.
[Double Tonsil Abscess]
This syndrome is externally caused by wind-heat and internally by qi... Yuan Ming Fen (Ammonium Chloride) taken with vinegar extracts phlegm. Insufflate the primary medicine, knife-bleed, insufflate the secret formula and primary medicine. Take San Huang Liang Ge San (Three Yellows Clearing Diaphragm Powder). Remove pus if present, take Qian Jin Nei Tuo San (Thousand Gold Internal Support Powder), insufflate Sheng Ji San (Flesh-Generating Powder), and take Jie Geng Tang (Platycodon Decoction) for greater stability.
