Qian Yi stated: The Liver governs Wind. In excess conditions, manifestations include eyes staring straight, loud crying, yawning, neck stiffness, and sudden chest oppression. In deficiency conditions, manifestations include teeth grinding, frequent yawning, and shortness of breath. Heat pathogen external generates warmth; internal generates Liver Heat. If symptoms include grasping at clothing collars and randomly rubbing objects, Xie Qing Wan (Drain the Green Pill) is indicated. If symptoms include high fever, thirst with desire to drink water, wheezing, and chest oppression, Xie Bai San (Drain the White Powder) is indicated.
When the Liver has Wind, the eyes move continuously without convulsions; if Heart Heat is also present, convulsions occur. Treat Liver Wind with Xie Qing Wan (Drain the Green Pill); treat Heart Heat with Dao Chi San (Direct Outward the Red Powder). If Wind is severe causing the body to arch backward and become rigid without convulsions, it indicates the Heart does not accept heat; one should tonify the Kidney to treat the Liver (treating the mother organ).
When the Liver has Heat, the eyes stare straight without convulsions; if Heart Heat is also present, convulsions occur. The treatment method is the same as for Wind conditions. Any disease, whether new or chronic, can draw out Liver Wind. When Wind moves, it stops at the head and eyes because the eyes belong to the Liver. When Wind enters the eyes, moving up, down, left, and right like wind blowing, neither light nor heavy, the child cannot bear it, resulting in dizziness and eye tremors. If Heat enters the eyes, pulling on the sinews, making both eyes tight so they cannot turn to look, the eyes stare straight. If accompanied by Heart Heat, convulsions occur because both the mother (Liver) and child (Heart) organs have excess heat, with Wind and Fire battling each other.
Zhang Yuansu (Jiegu) stated: The Liver governs planning and courage. When Heat is excessive, there is seeking of clothes, rubbing objects, continuous eye movement, inability to turn the eyes, or extreme cases of body rigidity and bending, all due to Wind-Heat. The eyes are the opening of the Liver, belonging to Wood; Wood nature is urgent, hence such symptoms.
Qian Yi stated: The Heart governs Fright. In excess conditions, manifestations include crying and wailing, fever, drinking water, and convulsions. In deficiency conditions, manifestations include lethargy, palpitations, and restlessness. Heart diseases often present with crying, shaking of hands and feet, fright, and palpitations. If Heart Qi is hot, the chest and heart area will also be hot; the patient wants to speak but cannot, and has a desire for coolness, thus preferring to sleep face down. To observe Heart Heat, check if breath from the mouth is warm during sleep, or if the child sleeps face down, looks upward, shakes the head, or grinds teeth; these are signs of Heart Heat, treated with Dao Chi San (Direct Outward the Red Powder). In Heart Excess, Qi flow is obstructed up and down; lying on the side prevents Qi from flowing freely, so the patient prefers lying on the back to allow Qi to circulate freely, treated with Xie Xin Tang (Drain the Heart Decoction).
Qian Yi stated: The Spleen governs fatigue. Disease manifests as lethargy, diarrhea, and loss of appetite. In excess conditions, there is lethargy, body heat, and thirst; in deficiency conditions, vomiting and diarrhea occur, generating Wind.
The Lung governs panting. In excess conditions, there is chest stuffiness, confusion, rapid breathing; some patients desire water, others do not. In deficiency conditions, there is belching and long sighs. Lung Heat manifests as picking at the eyebrows, eyes, nose, and face. If Lung is excessive and complicated by Wind-Cold, there is chest fullness, shortness of breath, urgency, coughing, and rebellious Qi; one should first disperse Lung Qi, then disperse Wind-Cold. If the Lung is only affected by Cold Damage, there is no chest fullness. Lung deficiency heat manifests as deep red lips; treat by dispersing deficiency heat. Lung weakness manifests as white lips; tonify. If there is chest stuffiness, confusion, coarse breathing, panting, and belching, it is difficult to treat due to Lung deficiency and damage. Chronic Spleen and Lung diseases lead to deficiency and white lips. The Spleen is the mother of the Lung; when both mother and child are deficient and cannot nourish each other, it is called "Qie" (Timidity/Weakness). The Lung governs lip color; white and lustrous is auspicious; dry as dead bone is fatal.
Zhang Yuansu (Jiegu) stated: The Lung governs Qi. Dry-Heat manifests as high fever, thirst, wheezing, chest oppression, dry nose, and hand-picking at the brows and face, treated with Xie Bai San (Drain the White Powder). Chest fullness, shortness of breath, urgency, coughing, and rebellious Qi are all due to excess Lung Qi compounded by Wind pathogen injury, termed Minor Evil; first drain the Lung, then disperse.
Qian Yi stated: The Kidney governs deficiency, not excess; only in carbuncle-rash eruptions turning black and depressed is it considered excess. Children with Kidney deficiency are originally timid and weak. Due to incomplete fetal Qi, Spirit is insufficient, there is excessive white sclera, open fontanelles, and pale complexion; these are hard to nurture, and even if they survive, rarely exceed sixty-four years of age. If parents indulge excessively in sexual desires, children may die before forty. It is incorrect to attribute this solely to post-natal illness causing Kidney deficiency. Furthermore, insufficient Kidney Qi causes the feet to curl downward/crawl, because bones are heavy and desire to sink and shrink. Kidney Water is Yin; Kidney deficiency leads to fear of light, suitable for tonification.
Zhang Yuansu (Jiegu) stated: What is called "crawling downward" is due to insufficient Kidney Qi, with hot feet that dislike being covered by clothes. However, this area below the navel is governed by the Kidney, originating from Heart Qi descending into the Kidney region. This is actually Kidney deficiency with Heart excess, suitable for Di Huang Wan (Rehmannia Pill).
