Patient Wang: Cold phlegm stagnates and obstructs, causing dysfunction of the Jiache (St6) acupoint area, which presents as elevated, swollen, and hard, with a white but not red color. This belongs to Yin-cold bone groove wind (Gu Cao Feng), differing from cases presenting with redness and body heat.

Patient Zhao: Spleen deficiency allows damp-heat to enter the collaterals, resulting in numbness and swelling of both hands' finger joints and wrists. This is Drumstick Liutan (a chronic suppurative inflammation), which is not easy to heal quickly.

Patient Huang: Umbilical wind (Qi Feng) arises from damp-heat accumulation in the Spleen and Kidney. Currently, there is abdominal pain and diarrhea; one should first regulate the Middle Jiao.

Certain Patient: Ying Qi (Nutrient Qi) circulates within the vessels, while Wei Qi (Defensive Qi) circulates outside the vessels. In individuals who are obese and have excessive dampness, Wei Qi is often deficient and cold, while Ying Qi is frequently in a state of excess heat. Therefore, obese people during summer often have cold skin yet are prone to developing external sores. In this case, the sore erupted on the back and spine. After three treatment phases, internal pus has formed, but external necrotic tissue has not yet penetrated. The fire in the Ying level is extremely intense. Due to weak Wei Qi failing to distribute properly, it cannot guide blood to transform necrosis nor carry toxins outwards, gradually showing signs of toxin sinking inward, which is quite concerning. Without warming methods, Wei Yang cannot be assisted to encourage vitality; without clearing heat, Ying heat cannot be resolved to transform toxins. The Classic states: "When blood is excess, it should be drained; when Qi is deficient, it should be drawn up." This method is appropriate.

Additionally, use San Jiao Feng for fumigation.
Yuan notes: It is unclear if San Jiao Feng refers to San Jiao Hu Ma.

Patient Zhao: Swelling of the throat extends to the upper Hang Sang (nasopharynx), indeed constituting Hou Yong (Throat Abscess). Soup and water are difficult to swallow, with excessive phlegm and constipation. The disease has persisted for four days, with fierce evil fire blazing. Xiuweng's main approach of clearing transformation and washing away phlegm is very correct. My opinion is to directly use Liang Ge San to penetrate both exterior and interior, which is even more concise. Still awaiting correction.
Liang Ge San added with Niu Bang Zi (Arctii Fructus), Jie Geng (Platycodonis Radix), Lu Gen (Phragmitis Rhizoma).

Ren Yuan said: To become a master of surgery (Wai Ke), one must read many internal medicine formula books. Generally, the difficulty in treating surgical cases lies in internal injury Yin syndromes. However, it does not go beyond the eight characters: Exterior/Interior, Yin/Yang, Excess/Deficiency, Cold/Heat. If these eight characters are understood, life and death, ease and difficulty, will naturally be clear in the mind. The human body's Ying and Wei Qi circulate endlessly; once there is stagnation and reversal, swelling, hardness, and pain occur, generating external sores. Surgical books divide conditions into Five Good Signs and Seven Bad Signs to determine auspiciousness or inauspiciousness, which essentially boils down to seeking answers in Yin and Yang. It is said that among the Five Good Signs, at least four should be present; among the Seven Bad Signs, no more than three should appear. More Yang indicates auspiciousness; more Yin indicates inauspiciousness. If good and bad signs coexist, death or survival is possible; it depends on whether the skilled practitioner treats correctly (survival) or incorrectly (death). Even for strange syndromes not covered by standard texts or lineage teachings, as long as one's learning is profound, determining the Six Channels locations and examining Yin-Yang and Excess-Deficiency, one will naturally have confidence regarding life, death, auspiciousness, and inauspiciousness. As for ointments, pills, powders, and applications, one should not hesitate to spend money, sincerely and devoutly preparing them, and they will naturally yield immediate effects. Because such diseases have tangible traces to seek, they are more graspable than internal medicine. However, manual techniques and knife techniques must have lineage transmission; otherwise, mistakes may be made upon initiation, and by the time one realizes the error, the patient has already suffered significantly.
