Empirical Prescription (from *Xindu Medical Cases*)
Gualou (Trichosanthis Fructus), Gancao Jie (Licorice Joint), and Jinyinhua (Lonicerae Japonicae Flos) (each 5 qian); Lianqiao (Forsythiae Fructus), Chaihu (Bupleuri Radix), and Qingpi (Citri Reticulatae Pericarpium Viride) (each 2 qian). Decoct in water and take orally.
After the rupture of Nangyong (Scrotal Abscess):
Bangke (Concha Arcae, calcined), Huanglian (Coptidis Rhizoma), and Qingdai (Indigo Naturalis) (equal parts). Grind into fine powder and apply externally.
This prescription treats cases where the scrotal abscess has ruptured, resulting in hardening of the scrotal skin with pus discharge and exudation.
Cangzhu (Atractylodis Rhizoma), Wuzhuyu (Evodiae Fructus, stir-fried), Chuanxiong (Ligustici Chuanxiong Rhizoma), and Danggui Shen (Angelicae Sinensis Radix, root portion) (each 1 qian); Baizhu (Atractylodis Macrocephalae Rhizoma), Tianhuafen (Trichosanthis Radix), Guanzi Gui (Cinnamomi Cortex), and Mutong (Akebiae Caulis) (each 8 fen); Qingmuxiang (Tutcheri Aromatis Radix) (1.5 qian); Sheng Huangqi (Astragali Radix) (2 qian); Longdancao (Gentianae Radix) (5 fen); Shepan Guo (2 qian; if unavailable, substitute with another herb).
Chop the above herbs as one dose, decoct with one bowl of white wine, and take on an empty stomach.
This prescription is indicated for Nangyong accompanied by systemic fever, bitter taste in the mouth, dryness of the throat, and redness, swelling, and pain of the scrotum.
Duzhong (Eucommiae Cortex, mixed with salt water, stir-fried until fibers break, ground into fine powder, sifted to remove fibrous material, retaining only the pure powder, 1 liang).
Decoct with one jin (approx. 500g or 16 oz depending on historical context, here likely referring to volume/weight ratio for decoction) of raw wine until half remains. Take orally to induce sweating, which will reduce swelling and relieve pain. If not cured, administer another dose to achieve cure. For external application, use Ming Xionghuang (Realgar, 1 liang) and Baicaoshuang (Soot from burning various herbs, 1 qian), grind uniformly, mix with honey water for topical application. If it dries, moisten with chicken feather dipped in water. After healing, take a root-removing prescription. Take Sheng Nanxing (Arisaematis Rhizoma), Sumu (Sappanwood), and Zisuye (Perillae Folium) (each 5 qian), and Chen Xiangyuan (Citri Sarcodactylis Fructus, aged) (one piece). Grind all into fine powder. Take one bladder from a castrated pig that has never held water, boil in wine until soft and mushy, mash together with the herbal powder, form into cakes, sun-dry and bake until crisp, then grind into powder. Use steamed cake paste as a binder to make pills. Take 80-90 pills on an empty stomach with salt-water broth. One course of this treatment ensures permanent cure without recurrence.
Nangyong (Scrotal Abscess):
Haipiaoxiao (Sepiae Endoconcha), Gafen (Margarita), and Ercha (Katechu) (equal parts).
Grind extremely finely and sprinkle on the affected area.
Another Prescription:
Gan Heye (Loti Folium, dried and roasted) and Zisuye (Perillae Folium) (equal parts, dried and roasted).
Grind extremely finely, mix with wine to form a thick paste, apply to erythematous skin, and cover externally with oiled paper.
Wang Kentang stated: "Shenzang Feng" (Kidney Wind) is also known as Shennang Feng Chuang (Scrotal Wind Sores). It arises in hidden areas, causing itching leading to sores. Accompanying symptoms may include tinnitus and ear itching, nasal redness, loose teeth, and whitish discoloration in the nail beds (from *Yixue Zhunsheng*).
Imperial Physician Dai stated: In folk custom, this is called Shenzang Feng Chuang. It results from seminal fluid deficiency combined with external impurity, invaded by wind-damp pathogens. It begins with damp sweat and itching of the scrotum, spreading to the limbs, presenting as white spots on fingers and generalized sores. (Treatment: Wash with decoction of Shechuangzi (Cnidii Fructus), Wuzhuyu, or Gancao Jie. Externally, burn Xi Cai Gen/Jing/Ye (Amaranthus root/stem/leaf) to preserve its nature, dry, and sprinkle. This also treats Yin Han (scrotal dampness) causing sores.)
Chen Shigong stated: Shennang Feng is caused by wind-dampness in the Liver Meridian. The primary symptom is itching, with a preference for bathing in hot, humid conditions. Morphologically, it presents as nodules and numbness of the scrotum, breaking down to exude fatty fluid (from *Waike Zhengzong*).
