Qin Hua Wan (Lozenge Pill): This prescription is highly effective for treating Mei He Qi (Plum-pit Qi, a sensation of a foreign body in the throat).
Alumen Crudum (Danfan), Borax (Pengsha), Alumen (Mingfan), Gleditsiae Sinensis Fructus Spina (Yazao), Realgar (Xionghuang) (equal parts)
Grind the above herbs into a fine powder. Use red dates boiled until soft, take the flesh, and form pills the size of Euryale Seed (Qianshi). Take one pill to be held in the mouth on an empty stomach, followed by a cup of warm yellow wine to wash it down, and internally administer Suzi Jiangqi Tang (Perilla Fruit Descending Qi Decoction).
Verified Prescription: Remove the heart and pistil of Rose Flower (Meiguihua) completely, soak in liquor and drink. The older it is stored, the better the effect.
Qin Hua Wan (Lozenge Pill): Treats Phlegm Nodules (Tan Jiehe) in the throat that cannot be expelled or swallowed.
Trichosanthis Semen (Gualouren), Armeniacae Semen Amarum (Xingren), Spongia (Haishi), Platycodonis Radix (Jiegeng), Forsythiae Fructus (Lianqiao) (each 1 liang), Mirabilite (Puxiao) (4 qian)
Grind into powder. Mix with ginger juice and honey to form pills the size of a cherry. Hold in the mouth and swallow slowly.
Pediatric hoarseness and inability to speak should be treated urgently with this prescription. If delayed until age twelve or thirteen, when mental faculties are not yet fully developed, it will be difficult to treat.
Polygalae Radix (Yuanzhi), Acori Tatarinowii Rhizoma (Shichangpu), Ziziphi Spinosae Semen (Suanzaoren, stir-fried), Xiao Chaihu Tang (Minor Bupleurum Decoction) (each 1 qian), Ricini Semen (Bimazi, with shell, one seed per year of age)
Grind the above herbs into a fine powder. Take one pig liver, slice it into several layers with a bamboo knife, sprinkle the medicine inside the liver, wrap in paper, and bake until cooked. Administer with plain boiling water according to the child's size. Take three to five times daily; speech will naturally recover within seven days.
Pediatric aphonia (Empirical Prescription): Apply Toad Gallbladder bile (Hamadan) to the tip of the tongue for immediate effect.
Wind-fire invading the Lungs, combined with the mistaken ingestion of cold and raw foods, leading to a feminine/weak voice indicates a mild condition.
Hoarseness due to Deficiency Consumption (Xu Lao) and Lung damage is difficult to treat.
Dou Hanqing stated: After illness recovery, shortness of breath and inability to produce voice are all due to the failure of Lung Qi to circulate. It is suitable to use Jiangqi Tang (Descending Qi Decoction) with added Pedanii Radix (Qianhu), and add ginger juice before administration. (Quanshu)
He also stated: Hoarseness and inability to speak after illness indicate punishment inflicted on the Lung Meridian; there is no chance of survival.
Feng Luzhan stated: Yin (aphonia) refers to having the ability to speak but being unable to produce sound, hence the classics say "unable to speak." "Unable" does not mean absolutely not speaking. Sound originates from the throat and is emitted from the root of the tongue. Due to fetal enlargement, the ascent of the Kidney Meridian is blocked. The Kidney Meridian enters the Lung, circles the throat, and connects to the root of the tongue. The throat is the domain of the Lung, and the Lung governs sound. Although the person privately wishes to speak and has thoughts, others cannot hear them, hence it is called Yin. The Lung and Kidney are mother-son organs, thus it is said that treatment is unnecessary. If Quan Daquan interprets this as mutism, it becomes a Heart disease, as the Heart governs vocalization, which has nothing to do with infantile aphonia. If Zhang Zihe proposed descending Heart Fire, and Ma Xuantai proposed tonifying Heart and Kidney, if the Kidney meridians were truly severed, the disease would be incurable, so how could there be spontaneous recovery postpartum! Therefore, the classics state "the collaterals of the uterus are severed," where the character "severed" (Jue) should be interpreted as "blocked."
He also stated: The classics mention "women become aphonic in the ninth month of pregnancy" because the collaterals of the uterus are blocked. No treatment is needed; it resolves in the tenth month. Regarding conception: Month 1, Liver Meridian nourishes the fetus; Month 2, Gallbladder Meridian; Month 3, Heart Meridian; Month 4, Small Intestine Meridian; Month 5, Spleen Meridian; Month 6, Stomach Meridian; Month 7, Lung Meridian; Month 8, Large Intestine Meridian; Month 9, Kidney Meridian; Month 10, Bladder Meridian. Yin meridians precede Yang meridians, starting with Wood and ending with Water, following the generation cycle of Five Elements. However, logically deducing, the meridians of the Twelve Channels circulate day and night without interruption, nourishing fetal Qi at all times; there must be no theory of specific meridian nourishment for specific months. Now, regarding aphonia in the ninth month: by the ninth month, the fetus has grown large, and the collaterals of the uterus connected to the Kidney Meridian are blocked, hence it occasionally occurs. Since the Kidney Meridian connects to the root of the tongue, blockage of the pulse pathway leads to inability to speak. Thus, after delivery in the tenth month, speech recovers naturally without treatment. If treatment is required, it should tonify the Heart and Kidney.
He also stated: The origins of aphonia in Shanghan (Cold Damage) are diverse. Some are due to Phlegm obstruction and Qi closure; some due to blood loss and fright; some due to pathogenic heat rushing to the Heart, depleting Heart Qi; some due to fire scorching Yin, preventing dissemination. However, if throat sound remains normal but the tongue cannot move, it is called She Yin (Tongue Aphonia). If the tongue can move and articulate words but throat sound is absent, it is called Hou Yin (Throat Aphonia). She Yin is mostly due to Heart and Kidney; Hou Yin is mostly due to Lung and Stomach.
Danxi stated: Dry cough is extremely difficult to treat. This is due to severe Fire stagnation, where Phlegm and Fire pathogens reside in the Lung. Use bitter Jiegeng (Platycodonis Radix) to open upwards, and yin-tonifying and fire-descending medicines downwards. If not cured, it becomes consumption; heavy tonification and nourishment are appropriate. The Lung is the canopy; fire from the Five Zang and Six Fu organs rises internally, steaming and burning, drying up Water and injuring Metal. If chronic cough leads to hoarseness, the lung tube is ruptured, which is incurable. In the early stage, it is Cold enveloping Heat, requiring acrid dispersing methods.
Yanyi states: There was a woman suffering from prolonged heat-induced cough, body feeling like roasting, emaciated, about to develop Pulmonary Tuberculosis (Fei Lao). Using equal parts of Loquat Leaf (Pipaye), Akebia Caulis (Mutong), Farflores Flos (Kuandonghua), Asteris Radix (Ziwan), Armeniacae Semen Amarum (Xingren), and Morus Cortex (Sangpi), with half the dose of Rheum Radix et Rhizoma (Dahuang), made into honey pills the size of cherries. One pill was dissolved in the mouth after meals and before sleep. She recovered before finishing one course. Another case involved prolonged cough, severe chest pain when hungry, and white spots like rice grains on the upper lip, indicating worms biting the Lung. Administer Baibu Gao (Stemona Paste) alone with Mume Fructus (Wumei) and Arecae Semen (Binglang) to expel the worms for cure. Lung worms resemble silkworms, causing cough and hoarseness.
Those with cough and hoarseness: When Metal is solid, it does not ring; this is Phlegm-Fire stagnating in the middle. When Metal is broken, it does not ring either; this is Lung Qi damaged internally. For excess, clear it; for deficiency/breakage, tonify it; these are the ways to treat the Lung. Also, note that the collateral of Shaoyin enters the Lung, circles the throat, and clutches the root of the tongue. The Lung is the branch; if the root is deficient, the branch is weak, leading to chaotic voice, hoarse throat, flaccid tongue, and inability to project sound. Focus on rescuing the root. There are also cases of dry throat and aphonia. Ancient prescriptions used two jin of Pig Fat (Zhuzhi), rendered into oil, filtered, mixed with one jin of White Honey, refined into a paste. Take one spoonful at a time to moisten. For those who shouted loudly, rupturing the throat and losing voice, do not treat as Fire using cooling medicines. Instead, heavily tonify Bazhen Tang (Eight Treasure Decoction) with Licorice Root (Gancao) and Platycodonis Radix (Jiegeng), doubling Ginseng (Renshen), and treat as trauma. The Foot-Shaoyin Meridian clutches the root of the tongue, the Foot-Taiyin Meridian runs along the side of the tongue, and the Hand-Shaoyin Meridian ties to the root of the tongue. If these three meridians are deficient, Phlegm-Saliva takes advantage of the deficiency to block the pulse pathways, preventing the tongue from moving and articulating. If Blood is insufficient in these three meridians to nourish the tongue, it also causes aphonia. Thus, after prolonged illness with blood loss, or after mistaken sweating and purging, when Heart Qi and Blood are depleted and cannot propel the tongue, use Dushen Tang (Single Ginseng Decoction) or Bajia Tang (Eight Ingredients Decoction) with modifications. Add Bamboo Sap (Zhuli) if there is Phlegm.
