A scholar in Zhejiang experienced sudden, unbearable pain in his finger. Between the fingernails, a coral-like swelling grew two cun high, presenting a mirage-like appearance with blood-colored vapor streams forming scenes of warriors on horseback, city walls, and spears, which he could see himself. He had a fondness for drinking distilled liquor. Physicians attributed these hallucinations to fire pathogenic factors and prescribed *Radix et Rhizoma Rhei* (Dahuang), which provided slight relief. Long-term use led to the cessation of symptoms. However, how can fire pathogenic factors create such anomalies? (From *Yishi Zixie*)
Wang Kentang stated: Swelling and toxic accumulation at the web spaces of the five fingers are commonly known as "Yacha" or "Bingxiecha." (*Zhunsheng*)
Shen Douyuan stated: Web space toxins of the hand all involve the Hand meridians. The dorsal web spaces correspond to the Three Yang Meridians of the Hand, while the palmar web spaces correspond to the Three Yin Meridians of the Hand. In diagnosis, it is necessary to distinguish the affected meridian and administer guiding herbs accordingly. (*Qixuan*)
Wang Shengzhi stated: The Three Yin and Three Yang Meridians of the Hand differ in their abundance and deficiency of Qi and Blood. Toxems arising in the web spaces should be treated according to the specific meridians involved. (*Lili*)
Zhu Danxi stated: Web space sores occur in the interosseous spaces between the phalanges. Except for the sore at the Hegu acupoint of the thumb, which is called Hegu Ju, sores in other web spaces are collectively called Hand Web Space Fa. Initially, they resemble millet grains, red in color, gradually enlarging to the size of a bean, accompanied by heat. Pain persists after ulceration until the pus is drained and cleared.
Wang Kentang stated: Tiger Mouth Ju, also known as Tiger Web Space Du, Hegu Ju, Yaci Du, Bi Xie Du, Hand Web Space Fa, and Bing Xie Cha. This condition is caused by the accumulation of wind-heat toxicity in the Large Intestine Meridian of Hand-Yangming. (*Zhunsheng*)
Chen Shigong stated: Hegu Ding, commonly known as "Tiger Mouth Baiya," often originates from nodular blisters or spreading red threads, hence the name "Ding." This itching is caused by damp-toxicity attacking from the Stomach Meridian of Foot-Yangming. Upon onset, puncture the lesion, cover with Chansu Paste, and apply Jinhual San. Treatment must focus on internal and external resolution and expulsion. (*Zhengzong*)
Hu Gongbi stated: Tiger Mouth Du, also known as Yaci Du, is highly destructive to the hand tissue. If the toxicity enters the Liver and Spleen, it can be fatal. (*Qingnang*)
He further stated: Symptoms include redness, heat, and pain, with possible cracking and swelling of the tiger mouth area.
Xue stated: This condition often arises at the junction of the thumb and index finger, presenting as sudden swelling and toxicity with unbearable pain. If not treated early, it will cause severe tissue necrosis of the hand, known as Pai Xie Du.
Zhu Danxi stated: Tiger Mouth Ju occurs in the interosseous space between the thumb and index finger (at the Hegu acupoint), resulting from the coagulation of damp-heat in the Large Intestine Meridian. Also known as Yacha Du. Initially, it resembles a bean, with diffuse swelling, cyanotic color, numbness, pain, and hardness. If small yellow millet-like blisters appear initially with itching, heat, and pain, and red lines spread upward toward the axilla, it is named Hegu Ding.
