Insects are generated in the anus, causing itching of extreme intensity, as if comfort is derived from their movement in and out with the force of the human body. This is due to playful teasing in childhood where wind pathogen entered through the opening, thus presenting this strange manifestation. (Shishi Milu)
Mi Jian Zhi Yang Dan (Honey-Fried Itch-Relieving Pill) (from Shishi Milu)
Cnidii Fructus (Shechuangzi), Melia Radix (Lianshugen) (each 3 qian), Raw Glycyrrhizae Radix et Rhizoma (Sheng Gancao) (1 qian).
Grind all into fine powder. Boil together with honey to form a suppository shaped like the male genitalia, insert into the anus, allow it to melt naturally. One dose stops the itching and cures the condition.
Xinfa states: Feng Gan (Wind-related Parasitic Disease) occurs when wind-dampness invades the Gu Dao (anus). The appearance resembles wind-xuan (wind-induced eczema/psoriasis) with intense itching; upon breaking, yellow fluid flows and spreads, accompanied by mild pain throughout the body. It should be treated with Ru Sheng San.
Feng Luzhan stated: Itching and pain in the Gu Dao are mostly caused by damp-heat generating insects, potentially developing into hemorrhoids. It is advisable to smoke with realgar mixed with mugwort, or take one jin of peach leaves, steam them until extremely hot, place them in a small-mouthed bottle, and sit over it to fumigate for immediate relief.
Ru Sheng Gao (Ru Sheng Ointment)
Angelicae Sinensis Radix (Danggui) (5 qian), Crotonis Semen (Badouren) (3 qian).
Boil eight liang of Sesame Oil (Mahu You) until the herbs are scorched, add three liang of Beeswax (Huang La) until fully melted, filter through silk cloth, add two qian of Calomel (Qingfen), stir evenly, and apply externally.
Xinfa states: Guyang Ju (Abscess on the Yang Aspect of the Thigh) arises on the lateral aspect of the thigh, behind the tip of the hip joint. The toxin binds inwardly to the bone joints, and the pus reaches deep to the bone, hence presenting as diffuse swelling without color change.
Xinfa states: Guyin Ju (Abscess on the Yin Aspect of the Thigh), also known as Chi Shi, occurs below the medial seam of the thigh, near the side of the scrotum. Because it lies偏 (biased/tending towards) the Foot Jueyin Liver Meridian, it is named Guyin Ju.
Qima Yong (Riding Horse Carbuncle) is caused by young people failing to protect their health, such as frequenting brothels, indulging in sexual pleasures, retaining semen during intercourse, enduring hunger while maintaining yang energy, forcibly holding back qi at the moment of ejaculation, or re-stimulating yang after ejaculation. It can also result from intercourse immediately after the partner has ejaculated without cleaning. All these factors are sufficient to generate this malignant toxicity. Treatment is primarily with Qi Wei Sheng Shen Tang. This toxicity enters via deficiency; therefore, one must greatly tonify blood and assist with products that expel pathogens. Then the disease will depart as easily as lost items returning. Otherwise, the condition will linger and tarry, and disaster will follow swiftly. Rather than waiting for the toxic momentum to spread widely before attempting to disperse it, is it not better to act while the righteous qi is still strong at the onset, using one dose to vigorously expel the pathogen? The brilliance of the formula lies in the use of Honeysuckle Flower (Jinyinhua), with Angelicae Sinensis Radix (Danggui) as the sovereign herb for nourishing blood, Ginseng Radix (Renshen) as the minister herb, and Rheum Radix et Rhizoma (Dahuang) as the guide herb. The appropriateness of weight and dosage is key. (Milu)
Weng Kentang stated: Qima Yong is governed by the Three Yin meridians of the hand and foot. The toxin resides in the Kidney Meridian, with damp-heat flowing into the Large and Small Intestines. Hot blood accumulates to form toxicity. If treatment is delayed, it becomes a fistula (Lou Zheng), with both urine and stool exiting from here. This area must not be cut with knives or needled. If needling is used, it should only be shallow, one or two fen. Exercise utmost caution.
Xinfa states: Qima Yong, also known as Pian Ma Zhui, arises beside the scrotum, in the hollow space between the inner thigh and the root of the leg. It is formed by the stagnation of damp-fire in the Liver and Kidneys. This location is the lowermost part of extreme yin; medical treatment must not overuse cold and cool medicines.
