When fever begins during the Yin and Mao hours (03:00–07:00), it indicates *Dou* (smallpox/variolous eruption) of the Liver Meridian. The primary manifestation is eruptions that are red with a greenish tint, distributed as six parts on the head and face, five parts on the back, four parts on the hands, three parts on the feet, and one-and-a-half parts on the abdomen; this is a mild condition.
When fever begins during the Si and Wu hours (09:00–13:00), it indicates *Dou* of the Heart Meridian. The primary manifestation is eruptions with red bases and pointed tops, distributed as four parts on the face, three parts on the back, two parts each on the hands and feet, and half a part on the abdomen; this is an auspiciously excellent prognosis.
When fever begins during the Chen, Xu, Chou, or Wei hours (07:00–09:00, 19:00–21:00, 01:00–03:00, 13:00–15:00), it indicates *Dou* of the Spleen Meridian. There are only a few eruptions on the face, round, full, yellow, and moist; only about ten on the back; seven or eight on each hand and foot; and none on the abdomen or epigastrium; this is an extremely favorable sign.
When fever begins during the Shen and You hours (15:00–19:00), it indicates *Dou* of the Lung Meridian. The primary manifestation is grayish-white eruptions, distributed as eight or nine parts on the face, seven or eight parts on the back, seven parts on the hands and feet, and three parts on the abdomen; this is a dangerous and adverse condition.
When fever begins during the Hai and Zi hours (21:00–03:00), it indicates *Dou* of the Kidney Meridian. The primary manifestation is black, sunken eruptions, distributed as ten parts on the face, nine parts on the back, eight parts on the hands and feet, and four parts on the abdomen; this is an adverse condition.
If eruptions appear in the early morning, "Lei Ni" (Thunder Reversal) will inevitably occur first, resembling connected silkworm eggs, dense on the head, face, and limbs, but slightly sparse on the chest and abdomen. The eruptions cannot emerge properly; lips evert, face swells with toxic attack, accompanied by bleeding from the mouth, tongue, and stool; even divine medicines would be ineffective.
If eruptions appear in the late morning ("Yi Zhao"), "Yu Gong" (Rain Palace) syndrome will inevitably occur first; eruptions have scorched heads and red bases, with blood-red patches on the abdomen and flanks. Lower back pain and abdominal pain will occur first, followed by alternating delirium and restlessness, spontaneous phlegm production and wheezing, throat obstruction with hoarseness, leading to imminent death. Students who follow my treatment methods can preserve the infant's life indefinitely.
If eruptions appear when the morning is more than halfway through, there is still concern for "Feng Gong" (Wind Palace) syndrome initially; eruptions resemble five plum blossoms, arranged in a 'pin' character pattern on the dorsal hands. Three or four in a row on the feet, seven or eight clustered in the heart and chest area, two-seven toxic carbuncles on the waist, and black scrotum without pus. Do not consider this a smooth condition, nor judge it as entirely fatal; with careful attention to diet and spirit, and random application of medicines according to circumstances, success can be achieved.
If eruptions appear after two days, they will inevitably manifest in the "Yun Bu" (Cloud Region); seven parts on the head and face, five parts of pus on the hands and feet. Four parts on the back, two parts of efficacy on the chest and abdomen. Fortunately, the tips are round, moist, and lively, yet still disliked for being dry, dark, and reddish-black.
If eruptions appear after two-and-a-half days, "Fa Dian Bian Ni" (Electric Generation and Reversal) will surely occur; high fever in the body and head is observed, requiring harmonization of the middle and stabilization of the exterior to assist efficacy. Eruptions naturally appear moist and lively, sparse on the face with empty ground; it is necessary to tonify the origin and secure the interior to prevent deterioration within.
Whenever children have fever accompanied by yawning, clear nasal discharge, coughing with phlegm-saliva, red face and lips, startle movements during sleep, dry eyes with red nose tips, cold finger and toe tips, and red veins appearing behind the ears, these are true signs of eruptive disease (*Dou*).
When eruptions involve both the Heart and Spleen organs, the tips show purple-black coloration with red bases; this is a condition of medium severity; one may rest assured without worry.
When eruptions involve the Lung, Liver, and Kidney organs together, the tips are black and the bases white; this is an extremely ominous sign and an incurable condition.
If fever persists after one-and-a-half days of eruption, with eruptions turning purple-black and delirium occurring as if seeing ghosts, purgative methods to generate fluids are appropriate; delirium should be treated with Chengqi Tang (Gut-Purging Decoction). If not treated urgently, death will occur at Zi hour (23:00–01:00) between the third and fifth days.
If fever has not subsided after two days and eruption marks appear with slight purple-redness, the previous medication should be used to diffuse; if not purple-red, Shijie San (Ten-Resolution Powder) should be used to remove internal toxins, followed by Siwu Tang (Four-Substance Decoction) as the main treatment. If medication is not taken, death will occur at Wu hour (11:00–13:00) between the fifth and seventh days.
If fever has not subsided after one-and-a-half days and eruption marks appear, shaped like silkworm eggs and hidden within the skin, they must be supported and pushed outwards; if they cannot be pushed out, death will occur at Chen or Mao hours (07:00–09:00) on the seventh or ninth day.
If fever has not subsided after one-and-a-half days and eruption marks appear as bright red, they should be diffused; do not recklessly use purgatives. If untreated, death will occur at Wu hour (11:00–13:00) on the ninth or twelfth day.
If fever has not subsided after three days and eruption marks appear, and are purple-black and scorched/sunken, Wumei Shijie San (Five-Flavor Ten-Resolution Powder) should be used, or herbs such as Huangqin-Huanglian (Scutellaria-Coptis) and Zicao (Arnebiae Radix et Rhizoma). If untreated, death will occur on the thirteenth day with teeth chattering and shivering.
Eruptions appearing before the third day indicate excess heat; Chengqi Tang (Gut-Purging Decoction), Shijie San (Ten-Resolution Powder), or Chaige Tang (Bupleurum-Pueraria Decoction) may be used.
Eruption marks appearing after the third day are mostly deficiency patterns, with few excess patterns. Medication must be prescribed based on the appearance of the eruptions; if grayish-white or densely packed red spots, diffusion is appropriate.
Upon the first appearance of eruptions, one must distinguish between exterior/interior and deficiency/excess; do not recklessly use purgatives. If eruptions are purple-red and dry, drinking water tastes like honey, tea consumption is incessant, and whole-body fever does not subside, urgently administer cooling and detoxifying agents; if treatment fails to resolve it, collapse and sinking will occur, rendering it incurable.
If there is slight fever, pale-white eruptions, clear and profuse urination, and cold breath, Da Bu Tang (Great Tonification Decoction) should be administered; if eruptions appear evenly in two or three batches, it is auspicious.
If fever and eruptions appear all at once, dense internally without black sinking, or with a cloud-like patch on the forehead (Tian Ting), with distinct cheeks, filling up in one day, responding well to medication, without coughing, treat with confidence. If there are black sinks, scorched/dry eruptions, persistent fever, or lack of moisture, it is incurable.
First, inspect the lips and tongue tip. Second, inspect if eruptions are round, lively, red, and moist, with clear distribution grounds; even if numerous, it is auspicious. Third, inspect if the epigastrium and forehead are sparse; this is an auspicious sign. Fourth, inspect if eruptions are not sunken, with tips white like wax and exterior yellow/moist like oil; this is auspicious. Fifth, inspect if erupted eruptions have tips that are not scorched or purple; this is auspicious.
