At the junction of the five fingers, when a toxic swelling occurs: if it is located at the Hegu acupoint (LI4) between the thumb and index finger, it is named Hegu Ju (Hegu Carbuncle); those occurring in other interdigital spaces are collectively named Shou Ya Fa (Hand Interdigital Abscess), commonly known as Shou Ya Zhi or Hand Dorsal Interdigital Abscess. These conditions represent syndromes of the Three Yang Meridians of the Hand (Large Intestine, Sanjiao, Small Intestine). Those occurring in the interdigital spaces of the palm center represent syndromes of the Three Yin Meridians of the Hand (Lung, Pericardium, Heart). Initially, it resembles millet grains, gradually enlarging to the size of beans, accompanied by heat and unceasing pain. This is caused by the condensation of dampness and fire. In the early stage, dispersing, detoxifying, clearing heat, and resolving toxicity should be employed. If pus forms but fails to drain spontaneously, prepared silkworm feces or horn needles should be used to puncture and drain it. Once the pus is discharged and pathogenic factors recede, the stomach should be supported and the Ying (Nutritive) Qi harmonized to achieve recovery.
Hegu Ju, also known as Hu Ya Du (Tiger Interdigital Toxicity), is caused by the accumulation of damp-heat toxin in the Hand-Yangming Large Intestine Meridian. Initially, yellow millet-like vesicles appear, with sensations of heat, itching, and pain, and the root of the disease is deep. If red lines extend towards the axilla, it is named Hegu Ding (Hegu Furuncle). If it initially resembles a bean and gradually develops into diffuse swelling, hardness, and unbearable pain, it is named Hegu Ju. The treatment method is the same as for the preceding condition.
She Tou Ding (Snake Head Furuncle), also known as Tian She Du (Heavenly Snake Toxicity) or Tiao Ju, arises on the tip of the thumb or other fingers. Initially resembling millet, it gradually enlarges to the size of a bean or like a peach/plum fruit, becoming hard, red, swollen, and painful, with pain so severe it radiates to the heart. The color may vary between green, white, yellow, purple, or black; it may itch or feel numb without pain. Originating from deep within the tendons and bones, it has a deep root and heavy toxicity, resulting from the condensation of heat-toxin in the Hand-Taiyin Lung Meridian and Hand-Jueyin Pericardium Meridian. Severe cases involve swelling of both the dorsum and palm of the hand, with hardening of the fingertip skin. Treatment involves applying pig gallbladder externally, coating with Xionghuang San (Realgar Powder), administering Chansu Wan (Toad Venom Pill) to induce sweating and resolve toxicity, and taking Yinhua Jiedu Tang (Honeysuckle Detoxification Decoction). If suppuration occurs after four or five days with a visible yellow head that can be pricked, it is a favorable outcome (Shun). If it does not suppurate, lacks pus, and black discoloration crosses the joint, it is dangerous (Xian). Medications such as Liangge San (Cooling Chest Powder), Huanglian Jiedu Tang (Coptis Detoxification Decoction), or Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) may be administered. If the toxin attacks the Heart, manifesting as vomiting, anorexia, abdominal distension, and bleeding from the gum margins, it is a critical prognosis. Early incision before the pus matures is contraindicated; otherwise, it causes skin rupture and muscle damage, doubling the pain and delaying healing. Prolonged illness may result in the formation of sequestra (dead bone); only after their expulsion can the wound close. For deficiency preventing closure, tonification methods should be used. In worse cases, failure to discharge pus and transform fire-toxin leads to tendon necrosis and loss of a phalanx.
She Yan Ding (Snake Eye Furuncle) arises in the sharp corners beside the fingernails, resembling a bean, with a purplish-red color, partially exposed, and hard as an iron nail. It is also caused by fire-toxin. If a yellow head emerges resembling an eye, it should be punctured with a needle.
Shui She Tou (Water Snake Head) refers to bright yellow blisters on the fingertip. These should also be punctured; draining the noxious fluid leads to recovery.
