Mu She (Wooden Tongue) refers to the sudden swelling of the tongue, resulting in stiff movement and numbness. The tongue is the sprout of the Heart (external manifestation of the Heart), and the Heart is the root of the tongue. This condition arises from heat-toxin in the Heart meridian, or due to accumulated heat in the Zang-Fu organs, specifically heat accumulation in the Heart and Spleen, with Qi rushing upward. In severe cases, the tongue fills the mouth and becomes hard as pangolin scales. If not treated urgently, it can be fatal. One must not press it with hands, as this may damage the root of the tongue, often leading to unclear speech. If it progresses to silent crying, frequent changes in facial complexion, and convulsive pain, it is incurable. Treatment involves pricking the purple-black areas with a small knife, or needling the Jinjin (Metal Fluid) and Yuye (Jade Liquid) acupoints under the tongue to release blood and phlegm, followed by blowing Bingpeng San (Ice and Borax Powder) into the wound. Internally, Jingfang Baidu San (Atractylodes and Saposhnikovia Decoction for Expelling Toxins) is administered, with Gardeniae Fructus (Gardenia) added in large amounts during decoction, as it is a key herb for purging fire.
She Nu (Tongue Hemorrhage) refers to bleeding from the surface of the tongue that continues like a thread. This is caused by excessive fire-evil in the Heart and Stomach forcing blood to move recklessly. The primary formula is Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill) with Sophorae Flos (Sophora Flower). Alternatively, Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxins) or Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) may be used. Externally, roasted and finely powdered dry Sophorae Flos is applied.
She Gan (Tongue Ulcer) is caused by toxic fire in the Heart and Spleen. Generally, the body of the tongue belongs to the Heart, and the sides belong to the Spleen. Mental agitation generates fire, while overthinking injures the Spleen causing Qi stagnation; severe stagnation leads to this disease, which presents with the most malignant symptoms. Initially, it resembles a bean, later growing like a fungus, with a large head and small stalk, also known as She Jun (Tongue Fungus). It is painful, red, ulcerated, and lacks skin. Symptoms are lighter in the morning and heavier in the evening. Beiting Dan should be applied promptly to shrink the lesion and effect a cure. If treatment is delayed, swelling protrudes like an open lotus or resembles a rooster's comb. The tongue becomes short and retracted, unable to extend for speech, leaking foul saliva. Aggravated by anger rising upward, it may suddenly rupture with uncontrollable bleeding. Over time, rot spreads to the gums, named Ya Yan (Tooth Rock). In severe cases, submandibular swelling and nodules appear, hard and intermittently painful, with normal skin color but a dark, non-red apex. After rupture, foul fluid flows, and the tissue rots like cotton. Despite ulceration, hardness persists, termed Mian Kui (Cotton Ulcer). It may even penetrate through the tongue and cheek, causing fluids to leak, hence also called Fan Hua Yan (Turning Flower Rock). Due to the tongue's immobility and difficulty eating, food intake is insufficient, leading to an empty stomach, various deficiency syndromes, and gradual debilitation. In the initial stage of redness and swelling, Daochi San (Guide Out Red Powder) with Coptidis Rhizoma (Huanglian) is suitable. For deficiency, Guipi Tang (Restore the Spleen Decoction) is used. For loose stools, Guishao Yigong Tang (Angelica and Paeonia Yi Gong Decoction) is indicated. Although many treatments exist, survival rate is less than one in a hundred; even with medication, life is merely prolonged temporarily.
She Ding (Tongue Carbuncle) manifests as purple blisters on the tongue, bean-shaped, hard, accompanied by chills and fever, with pain radiating to the heart region. This is also due to fire-toxin in the Heart and Spleen. Chan Su Wan (Toad Venom Pill) should be held under the tongue to dissolve and swallow, resolving internal toxins. In severe cases, needling is performed, and Xijiao Dihuang Tang is administered.
