Bingju (Malar Abscess) arises from the reckless movement of ministerial fire in the Shaoyang Sanjiao and Gallbladder meridians, compounded by Kidney Water's failure to nourish Liver Wood, or due to contracting Wind-Heat pathogens. The flesh at the hairline of the temples is tightly compacted, making it most difficult for tissue to necrose and ulcerate. Initially, there are alternating chills and fever, dizziness, pain radiating through the Taiyang meridian, and in severe cases, swelling extending from the ears and eyes to the temples. Treatment must not employ acupuncture or moxibustion recklessly; one must differentiate Yin and Yang, Exterior and Interior, Pathogenic factors and Healthy Qi, Deficiency and Excess to avoid mismanagement. If upon initial appearance of the sore, symptoms include more chills than fever, dry mouth with thirst favoring hot drinks, a pulse that is deficient, rapid, and weak in all six positions, along with hardness at the lesion site, mild pain, no ulceration or suppuration, and diffuse roots, this indicates deficiency of True Qi and excess of pathogenic factors. The treatment principle is primarily to support the interior and expel toxins, assisted by detoxification, using formulas such as Qinggan Yangxue Tang (Liver-Clearing and Blood-Nourishing Decoction) or Tuoli Xiaodu San (Interior-Supporting and Toxin-Expelling Powder). If upon initial appearance, symptoms include more heat than chills, dizziness and headache, dry mouth and tongue, desire for cold drinks, constipation and dysuria, a pulse that is deep and forceful, a sore with unopened roots, redness, swelling, and pain, systemic fever, and easy necrosis and suppuration, this indicates excess of Healthy Qi and deficiency of pathogenic factors. The treatment principle is primarily detoxification, assisted by supporting the interior, using formulas such as Zhizi Qinggan Tang (Gardenia Liver-Clearing Decoction) or Shuniu Zi Tang (Arctii Fructus Decoction). Furthermore, if the sore appears purple-black, flat and sunken, hard without pus, with toxin spreading to the ears and neck, accompanied by foul odor, lackluster complexion, mental dullness, and anorexia, these are considered incurable conditions.
Eju (Frontal Abscess) occurs between the hairline on the forehead at the Qucha acupoint (Ex-HN 5), belonging to the Foot-Taiyang Bladder Meridian. Onset typically presents with chills and fever, severe headache as if split by an axe, unbearable pain, stiffness in the neck as if pulled, and soreness in the lower back as if broken. Initially, Wanning Dan (Ten Thousand Spirits Pill) should be used to induce sweating and dissipate Wind pathogens; subsequently, clearing and supporting methods should be employed. There are also Left Eju and Right Eju, resembling peaches or plums, which result from accumulated heat in the Foot-Yangming Stomach Meridian. This area is near the Taiyang acupoint (Ex-HN 5); if exposed to Wind-Water pathogens after ulceration, it can be fatal. Even when applying medicated plasters, careful protection is required, and medications should be used to hasten wound closure. If the condition persists through winter, it may transform into a Cold Sore. Moreover, since this area near the Taiyang acupoint extends up to the frontal angle, it is a dangerous region; abscesses that fail to ulcerate or heal within the expected time frame are considered incurable.
