For the treatment of severe Yin abscesses (Yin Ju, a type of chronic, deep-seated, cold-type abscess), characterized by massive swelling and hardness, pain extending to the bone, yet with normal skin appearance; or cases presenting only with joint soreness without palpable masses; as well as abdominal carbuncles (Du Yong), intestinal abscesses (Chang Yong), and other conditions involving hard, deep-seated masses. This formula is also effective for sprains, contusions, and bi-syndrome (Bi Zheng, obstruction syndrome caused by wind-cold-dampness) due to the invasion of wind, cold, and dampness pathogens, manifesting as limb and joint soreness/pain and limited mobility.
* Ji Xing Zi (Cardiospermum Halicacabum Seed) – 5 liang
* Da Nan Xing (Arisaema Cum Bile) – 3 liang
* Chuan Wu (Aconitum Carmichaelii Debx. Radix)
* Cao Wu (Aconitum Kusnezoffii Reichb. Radix)
* Gan Jiang (Zingiberis Rhizoma)
* Qiang Huo (Notopterygii Rhizoma et Radix)
* Du Huo (Angelicae Pubescentis Radix) – 2 liang each
Slice all the above ingredients. Boil in 15 jin of pure sesame oil. First, add stems of *Impatiens balsamina* (Feng Xian Hua Jing) and simmer for 20 minutes until the vigorous boiling subsides. Then, add Sheng Di Huang (Rehmanniae Radix). Simmer for another ten-plus minutes. Subsequently, add the remaining herbs and fry until charred. Strain the liquid thoroughly. Transfer the filtrate to a clean pot, boil over gentle fire, and add approximately 1.5 jin of sieved Guang Dan (Litharge) and finely sieved Ding Fen (Minium). Stir continuously with a willow wood stick until perfectly uniform. Test the consistency by dropping the paste into water. Once the plaster is formed according to the above method and removed from heat, mix in pre-ground fine She Xiang (Moschus) – 5 qian, Ru Xiang (Olibanum) and Mo Yao (Myrrha) (de-oiled) – 3 liang each, Shang An Gui (Cinnamomi Cortex) powder and Ding Xiang (Eugeniae Flos) powder – 2 liang each. Blend evenly, form into balls by dipping in water, and store as described previously.
Ulcers generally favor cooling treatments. However, for cases of Yuan Qi deficiency and cold, where ulcers fail to heal after a long duration, this plaster is also suitable for application. For treating pain in bones and tendons due to contusions, or bi-syndrome due to cold-dampness obstruction, Si Wen Dan (Four Warming Pellets) should be added and mixed evenly before application. Commercially available Wan Ying Gao (Ten Thousand Remedies Plaster) is inferior to this formula. It can be rolled into pills, shaped like cakes, and applied for headaches due to wind-cold. For severe Yin abscesses, Si Wen Dan should additionally be added and mixed evenly, then applied as a thick layer of plaster.
[Formula Analysis] The term "Bo Tie" (thin plaster) was already used in the Tang Dynasty, indicating that the invention of plasters has a long history. Surgical conditions are mostly due to damp-heat and are not suitable for warming therapies; conversely, for conditions of Yin-cold coagulation, cooling agents are contraindicated. However, for miscellaneous diseases affecting meridians, sinews, bones, and joints, external therapy is sometimes appropriate. Therefore, the method of warming and nourishing is indispensable. Surgeons often use Jia Wei Tai Yi Gao (Modified Tai Yi Plaster), which can treat general external sores but is not precise enough for severe Yin-cold conditions. My master's school, the Zhu family, formulated this prescription specifically for deficiency-cold and miscellaneous diseases. It can unblock collaterals and invigorate blood circulation, while also reducing swelling and softening hardness. Its indications are quite extensive. It is an essential preparation for treating surgical conditions.
