[Startle and Convulsion]
When convulsions occur before the eruption of smallpox/rubella spots, Qingjie San (Clearing and Resolving Decoction) should be used; if convulsions persist after the spots have appeared, Daochi San (Coptis Decoction for Guiding Heat Out of the Heart) is appropriate; if deficiency-type convulsions occur after the scabs fall off, Ningshen Tang (Spirit-Calming Decoction) is indicated; if convulsions are triggered by food stagnation fever, Dakang An Wan (Great Peace Pill) [i.e., Muxiang Da'an Wan] should be administered.
[Annotation] "Jing Dou" (Smallpox/Rubella with Startle) is mostly caused by the fire of the toxin disturbing the Shen (Spirit), transferring heat to the Liver, where Liver Wind battles with Heart Fire, resulting in this syndrome. The treatment method must distinguish between the beginning and the end stages: If convulsions appear first before the rash erupts, one must not solely use cold-natured herbs that might obstruct and close up the toxin; instead, only use Qingjie San to disperse exterior pathogens, and once the rash erupts, the convulsions will stop naturally. If the rash has already taken shape but convulsions continue unceasingly, this indicates toxin-fire lurking within the Heart Meridian, and it is suitable to treat with Daochi San added with Huanglian (Coptidis Rhizoma). If twitching occurs after the scabs fall off, this is due to deficiency of True Qi and internal attack by pathogenic fire, for which Ningshen Tang is the primary prescription. If convulsions are triggered by food stagnation fever after the disease, this is due to Spleen and Stomach weakness, inevitably accompanied by sallow complexion, tidal fever, and sour-smelling stool; Muxiang Da'an Wan is suitable for treatment.
