The treatment principles for measles are relatively straightforward. However, if complicated by *shazhang* (Sha-zhang: a condition characterized by acute abdominal pain and distension due to pathogenic Sha toxin) and not treated with scraping (*guasha*) or bloodletting, or if the contraindications of *shazhang* are violated, the measles pathogen may follow the *shazhang* trend and attack the Zang-Fu organs retroactively. Ascaris lumbricoides (roundworms) cannot survive in this environment; they will either be vomited out or passed through the stool. Otherwise, they may accumulate in the large intestine, causing pain around the umbilicus. The treatment should first employ scraping and bloodletting, followed by the administration of Mangxiao (Natrii Sulfas) and Dahuang (Rhei Radix et Rhizoma) to purge dead worms and accumulated feces, allowing the toxin to exit via the stool, thereby resolving the *shazhang*. Mild cases of measles will resolve spontaneously, while severe cases will manifest fully.
Clinical Cases:
1. A granddaughter of Liu Xiangzhong, in February, suffered from wind-strike with fever and cough. Measles appeared faintly but failed to erupt. She had hoarseness leading to aphonia, abdominal pain, and a state of mental confusion and heaviness. Previous physicians failed to cure her. Upon my consultation, her left pulse was Kou (hollow like a scallion stem), and her right pulse was Se (rough and choppy), with intermittent pauses. Using fire to illuminate the Sha veins on the back of the hand and fingers, I performed bloodletting with more than twenty needles. She was given Chenxiang Wan (Aucklandia Pill) taken with clear tea, slightly chilled before drinking. Additionally, Zipu Tang (Purple Magnolia Bark Decoction) combined with Jingjie Yinhua Tang (Schizonepeta and Honeysuckle Decoction) was administered, also slightly chilled. Subsequently, bloodletting was performed twice more on the fingertips. Following the previous prescription, Huangqin (Scutellariae Radix), Shigao (Gypsum Fibrosum), Mangxiao, and Dahuang were added, and it was taken warm. Her voice improved slightly, so Shigao was removed and Huangbai (Phellodendri Cortex) was added, taken warm. Four dead ascarids were passed, and she recovered after bowel movements normalized.
2. A maid surnamed Liu contracted measles, presenting with fever, cough, and abdominal pain. Her pulse was excessively Hongda (surging and large), indicating a complication with *shazhang*. Bloodletting was performed at five points on the popliteal fossa, releasing dark purple toxic blood flowing in streams. Another twenty-plus needle pricks were made on the fingertips. Taoren Honghua Tang (Peach Kernel and Safflower Decoction) with Sumu (Calamus Wood) removed, combined with Jingjie Tang (Schizonepeta Decoction) with Chuanxiong (Ligustici Chuanxiong Rhizoma) removed, plus Huangqin, was administered slightly chilled. Two ascarids were passed, and she recovered.
3. Guo Hongfu presented with fever, cough, vomiting, and diarrhea. Measles appeared and then disappeared again. He complained of thirst, scorched lips, epistaxis, hematochezia, a yellow tongue coating, and hard, painful tenderness around the umbilicus, accompanied by loud screaming. Medical treatment was ineffective. His brother, Guo Defu, invited me to treat him. His left pulses were all Fu (deep and hidden), and his right pulses were Hongjin (surging and tight). Bloodletting was performed on the fingers and arms with over thirty needles. Duhuo Honghua Tang (Angelicae Pubescentis and Safflower Decoction) with Shigao, Huangqin, and Mangxiao added was administered slightly chilled for two doses. This resulted in the passage of accumulated feces and six dead ascarids, allowing the measles to erupt subsequently. Then, Jingjie Yinhua Tang with Xuanshen (Scrophulariae Radix), Huangqin, and Huangbai added was taken warm for three doses, leading to recovery.
4. The second son of Fang Yuanxing suffered from Shanghan (Cold Damage) with fever, cough, restlessness, and severe abdominal pain. Measles appeared and then vanished. He had hoarseness and aphonia, and his six pulses were Xianjin (wiry and tight). Bloodletting was performed on the upper breast area, fingers, and arms with over twenty needles, but he did not improve. Shegan Douling Tang (Belamcanda and Coptis Decoction) with Ganju (Chrysanthemi Flos) and Huafen (Trichosanthis Radix) removed, plus Shigao, Taoren, Honghua, and Dahuang added, was administered for two doses, slightly chilled. This led to the passage of accumulated feces and three dead ascarids, allowing the measles to finally emerge. Bloodletting was performed again on the popliteal fossa, and the remaining symptoms, including abdominal pain, completely resolved.
