When one has just contracted malaria (Zhang disease), it is imperative to strictly observe dietary restrictions. Not only are raw, cold, and greasy foods forbidden, but especially alcohol, meat, fish, and wheat-based noodles. Rice consumption should also be suspended; only porridge may be eaten.
One must still abstain from animal products. If absolutely necessary to consume some white rice grits (Bai Shen), salted fermented soybeans (Xian Chi), or radish, these must not be eaten during an attack. They should only be consumed after the attack has subsided and some time has passed. If there is no attack, one may eat white porridge freely every day.
Do not eat until overly full. Failure to observe dietary restrictions will make the disease difficult to cure. The food consumed will all assist the pathogenic factors, rendering medication ineffective. If one eats vegetarian porridge for several days and then takes medication according to the previous methods, efficacy will be achieved. This illustrates that "dietary restriction is more important than medication."
One should only rinse the mouth daily; washing hands, face, and combing hair are prohibited. One should remain calm, sitting or lying down, without movement for several days. Through such nursing care, recovery is assured.
Once the malaria (Zhang disease) has ceased, record the number of days since the initial onset. Based on this duration, exercise extreme caution. Generally, after the cessation of attacks, hands may be washed after three days, the face washed after seven days, and hair combed lightly once or twice after half a month. Sexual intercourse must be abstained from for three months; abstinence for one hundred days is particularly excellent.
During periods without attacks, continue eating vegetarian porridge for three days. After five days, pork spleen soup (cooked thoroughly) may be consumed in small amounts, followed by soft rice. After ten days, a small amount of wine may be drunk, and meat broth eaten sparingly. However, juices derived from various bones must not be consumed. Violation of this rule will cause recurrence. For meats such as beef, mutton, and chicken, it is advisable to maintain dietary restrictions for another month. If proper nursing is neglected leading to recurrence, medication must again follow the previous protocols. The aforementioned measures refer merely to nursing care. Recurrence due to irregular eating, overexertion, or emotional instability, even if treated with medication according to previous methods, often results in a lingering condition lasting half a year before resolution. It is ultimately superior to nurse carefully to prevent recurrence.
In cases of Cold Malaria (Leng Zhang) at the initial stage, characterized by coldness and cold limbs in the waist and feet, with a deep and fine pulse, caused by hunger, cold exposure, or dietary injury, internal administration of Ginger (Sheng Jiang) and Aconite (Fu Zi) decoctions is indicated, along with external moxibustion on Zhongwan (CV12). [Located four cun above the umbilicus, ascending from Jianli (CV10); this is Zhongwan.] Qihai (CV6). [Located 1.5 cun below the umbilicus, in the depression; this is Qihai.] Zusanli (ST36). [Ascending one cun from Xianglei, at the tip of the shin muscle where pressing reveals muscular elevation; this is Zusanli.] By warming the middle jiao and the lower origin through these three acupoints, the disease will invariably heal. If the aforementioned symptoms are absent, and the patient is not generally weak, or if the disease has just begun, moxibustion should not be performed immediately. If the malaria has persisted for a long time, resulting in deficiency of qi and blood, and medication is ineffective, moxibustion on Gaohuang (BL43) is suitable. [Descending from Pohu, on the fourth interspace, above the fifth thoracic vertebra. This point is central, 3.5 cun lateral to the spine. Sit upright and curve the back to locate Gaohuang.] Or Dazhui (GV14). [Above the first thoracic vertebra; this is Dazhui.] and the fifth thoracic vertebra area (where Shendao (GV12) is located below the fifth vertebra). These two points, with moxa cones numbered according to the patient's age, are particularly effective. Adding Zusanli (ST36) will ensure the cure of chronic malaria. Heat Malaria (Re Zhang) contraindicates moxibustion.
