Ophthalmological Marvelous Book (Yanke Qishu)
--------------------------------------------------------------------------------
Preface
The *Ophthalmological Marvelous Book* is a secret manuscript privately collected by the Li family of Yuzhou (present-day Chongqing City) during the Qing Dynasty. The author remains unknown. The Li family treasured this book as if it were an archaic jade tablet, rarely showing it to others. Sun Fengming, from Changshou County in eastern Sichuan, had been close friends with the Li family for over ten years before he was finally able to borrow and read it in 1886 (the 12th year of Guangxu). He then ordered his sons and nephews to copy it separately. Later, when he prescribed medicines according to the book to treat eye diseases, all treatments proved effective. According to Sun Ben-duan, the nephew, the manuscript remained unprinted for a long time after being copied. It was not until 1923 that Zhou Songrong from Zhongzhou (present-day Zhong County, Sichuan Province) saw the book, praised it, and donated funds to print and distribute 1,600 copies in Yuzhou. The following year, Zhang Xiaolin donated funds to print another 1,000 copies, and Li Xuelin donated funds to print 1,500 more copies, allowing the book to circulate. An elder who called himself "Shugu" (Old Man Who Narrates Antiquity), fearing that the name *Ophthalmological Marvelous Book* might seem strange or impure, renamed it *Ophthalmological Suitable Book* (Yanke Yishu). However, due to the miraculous efficacy of its prescriptions, later generations mostly retained the original title. In 1934, An Ren, a native of Changshou, eastern Sichuan, reprinted 2,000 copies in Yuzhou, after which no further editions were published.
The content of the *Ophthalmological Marvelous Book* features four main characteristics: First, unique theory: It posits that external eye disorders are caused by cold pathogens, while internal eye disorders are caused by qi stagnation. This contradicts the common view that "eyes do not get sick unless affected by fire" and that "internal disorders are mostly due to liver and kidney deficiency." Second, unique prescriptions: For external disorders, it uses potent acrid-warm formulas such as Siwei Dafa San (Four-Ingredient Great Dispersing Powder) and Bawei Dafa San (Eight-Ingredient Great Dispersing Powder); for internal disorders, it primarily uses qi-breaking herbs like Zhike (Aurantii Fructus), Binglang (Arecae Semen), Yujin (Curcumae Radix), and Xiangfu (Cyperi Rhizoma). This differs significantly from the common cool-cold and tonifying/attacking formulas used by others. Third, heavy dosages: Herbs such as Mahuang (Ephedrae Herba), Xixin (Asari Radix et Rhizoma), Manjingzi (Viticis Semen), and Gaoben (Ligustici Rhizoma et Radix) are used in doses of one liang (approx. 30-37g), while Shengjiang (Zingiberis Rhizoma Recens) and Huangqi (Astragali Radix) have been used up to one jin (approx. 500g), which is astonishing. Fourth, miraculous efficacy: Sun Fengming stated that prescribing according to the book yielded consistent results. Modern clinical trials confirm the remarkable efficacy of these prescriptions. Although the warm nature of the formulas may be related to the author's specific era and geography, and the theoretical stance somewhat biased, it corrects prejudices such as "eyes do not get sick unless affected by fire." Overall, the book holds significant value for ophthalmological theory and clinical practice. Regarding the unusually heavy dosages, practitioners should exercise discretion and reduce the amounts appropriately.
Due to the passage of time, original printed editions are nearly extinct, making it difficult for modern readers to access the text, which is a regrettable loss. To facilitate the wide dissemination of this book, we have collated and published the 1924 lithographed edition preserved in the Henan Provincial Library, referencing several hand-copied and oil-printed versions circulating domestically. We hope this contributes to the inheritance and promotion of traditional Chinese medicine heritage and aids in revitalizing TCM ophthalmology.
Our collation methods are as follows:
1. Using the Guangya Shuju lithographed edition as the base text, we referenced hand-copied editions from the Henan University of Traditional Chinese Medicine library, oil-printed editions collated by Li Xiongfei et al. from Hengyang Hospital of Traditional Chinese Medicine (hereinafter referred to as the Li Collated Edition), the *Ophthalmological Suitable Book* hand-copied by Liu Zhenwu from the Henan University of Traditional Chinese Medicine (hereinafter referred to as the Liu Copied Edition), and the *Ophthalmological Marvelous Book* compiled by Jing Xiaochou (pronounced 'Qiu') (hereinafter referred to as the Jing Compiled Edition).
2. Missing characters, erroneous characters, and obscure words, phrases, or sentences were annotated one by one. Annotations are marked with superscript numbers corresponding to their order at the bottom of each page.
3. When An Ren reprinted the book in 1934, he included a preface titled "Preface to the Widely Circulated Ophthalmological Marvelous Book." This is an important document for studying the history and academic value of the book, so it has been added based on the Li Collated Edition.
4. The original text lacked a section titled "Explanation of the Ophthalmological Marvelous Book," which has been added from the Li Collated Edition.
5. The section "Essential Principles for Treating Eye Diseases" was originally misplaced under "Methods for Processing Prohibited Medicines for Eye Diseases." It has been extracted and given its own heading based on the Liu Copied Edition.
6. The original book had no table of contents; a new one has been compiled from the main text for ease of reference.
7. Many herb names in the original text were non-standard; they have been corrected directly without annotation.
8. The original book lacked an appendix "Hundred-Effective Formula for Treating Breast Diseases." Based on the Li and Liu editions, this has been appended to the end of the book to give readers an overview of the various versions.
9. Due to the change from vertical to horizontal layout, the character "right" (indicating position) has been changed to "upper."
Given the limited expertise of the collators, errors may exist. We respectfully invite colleagues to correct them.

Preface to the Widely Circulated Ophthalmological Marvelous Book
Pearls hidden in Hepu shine brilliantly, but without the discernment of the mermaid-like collectors, who would treasure and hide them? Jade from Kunlun Mountain is lustrous and warm, but without the carving of a jade artisan, who would take and use it? Secret manuscripts and life-saving prescriptions can yield miraculous effects, but without the transmission by predecessors, who would verify and widely spread them? The *Ophthalmological Marvelous Book*, a secret manuscript of the Li family, is like pearls and jade hidden in rough stone. They treasured it as an archaic tablet and did not show it privately. Yet, upon consulting a physician, prescriptions could be obtained immediately; upon taking them, effects were swift as drumbeats. People in the capital loved the uniqueness of its formulas but long despised the Li family for being aloof like the "drilling-core plum." Later, Mr. Sun Fengming, not sparing expense, obtained this excellent formula. Prescribing according to the book yielded divine effects, benefiting society for a long time. In the first year of the Republic of China (Ren Zi year), I finally obtained Mr. Xia Chaoxun's copy of the original. Though I am dull-witted and unworthy to understand its depths, I have occasionally grasped its subtle applications and found unexpected efficacy. Can this not be called a rare treasure? Ashamed of my shallow learning, I dared not add or subtract anything from the old version. However, the old version had many disordered bamboo slips, inverted sequences, and lacked classification. After quiet reflection, I carefully arranged the sequence and classified the content, not claiming perfect order, but merely clarifying the structure for easier reading. By autumn of the 12th year of the Republic of China (Gui Hai year), Mr. Huang Shouxing requested to publish it for public welfare, so I went to Yuzhou to print and distribute 2,000 copies. The following year (Jia Zi year), Mr. Sun Benduan went to Hanguo Heyihui, printed 3,000 copies, and distributed them to charitable organizations across provinces and counties. Thus, the book spread throughout China. If translated and promoted again, would it not spread worldwide? Is the verification and circulation of this book not like lustrous pearls and jade being recognized and used globally? However, the original dosages are too heavy; calling them contemporary formulas is incorrect; using one or two qian (suspected to be liang) per dose is also incorrect for ancient formulas. The claim of miraculous efficacy and unique formulas initially did not shock the world. This is why contemporaries, upon seeing it, gasped in doubt and set it aside. Do not be confused by its narrow circulation. This is why Old Man Shugu changed the title to *Ophthalmological Suitable Book*, which is justified. Some people overly emphasize the theories of Five Wheels and Eight Bowls, favor cool-cold attacking and tonifying formulas, and criticize the heavy dosages, failing to understand the magical changes. They slanderously denounce the book, even wishing to burn it. Such people are no different from summer insects discussing ice. They show their lack of measure. Do not be confused by the reluctance to widely promote it. Affairs of the world can be discussed with those who know, but not with those who do not. Alas, how lamentable! One might say: It is not difficult; simply change liang to qian. Starting with experimental doses is acceptable; increasing gradually upon efficacy is also fine. The subtlety of application lies in the mind; spiritual understanding depends on the individual. Do not be like those who mark the boat to find the sword, avoiding the ridicule of merely reading one's father's books. That would suffice. I specifically imitate the spirit of "narrating without creating," presenting this for the selection of enlightened experts, so its miraculous utility may circulate. May there be many enlightened experts in this age, inspiring future ones. I offer these few words at the beginning, hoping for guidance from contemporary scholars.
December 7, 1934 (Republic of China Year 23, Jia Xu Month, Day 1 of the 11th Lunar Month), Liao Zheng, pseudonym Xuguzi Anren, from Changshou, Eastern Sichuan, recorded at the study in ancient Yuzhou.
Origin of the Ophthalmological Marvelous Book
This book was obtained from a celestial secret manuscript passed down through generations and privately collected by the Li family of ancient Yuzhou. It has been tested repeatedly with verified results and is more concise than other ophthalmological books. However, the dosages are quite heavy; practitioners should exercise discretion during diagnosis, adjusting weights with a small scale. Users must proceed with caution.
Respectfully noted by Quan Ming-shi, Changshou, Eastern Sichuan
Spring of Bing Wu Year (Guangxu Era) at the Study of Tianfu Temple, Yucheng.
Explanation of the Ophthalmological Marvelous Book
1. The original title was *Ophthalmological Marvelous Book*. Old Man Shugu feared it sounded strange and impure, so he renamed it *Ophthalmological Suitable Book*.
2. This book is structured as a prescription manual, directly stating conditions and formulas without discussing medical theory, pulse diagnosis, or medication opinions. It is not convenient for students to research; readers please understand.
3. I have tested this book for over twenty years, achieving 100% success, superior to all other ophthalmological books. Regrettably, it lacks theoretical exposition, leaving this for worthy scholars to supplement.
4. For early-stage external eye disorders, medical consultation is not always necessary. Formulas like Great Dispersing Powder, prescribed according to the book, are invariably effective.
5. For severe eye diseases,务必请明医照此书斟酌加减 (must consult a skilled physician to adjust dosages according to this book). With divine application, long-term use leads to self-healing. Do not arbitrarily prescribe.
6. This book has three characteristics: ① Prescriptions can be made directly from the book. ② Upon initial recovery, vision does not become blurred. ③ Cured completely with proper treatment, never recurring.
7. The dosages in all formulas are too heavy. Whether prescribed by oneself or a doctor, it is advisable to change liang to qian; taking them still yields good effects.
8. This book is a copy of the Li Elder's handwritten version, originally lacking classification. I initially divided it into four categories. After printing, errors were found. Now slightly corrected, it is divided into five categories, thus differing slightly from previous printed editions.
9. The end of this book includes a "Hundred-Effective Formula for Treating Breast Diseases," treating all breast conditions including mastitis, breast pain, breast cancer, breast abscess, etc., with divine efficacy. Ancients said breast cancer had no cure since ancient times. This appendix is provided to benefit the world.
Ophthalmological Marvelous Book
This book was transmitted by immortals; it is hundred-percent effective for eye diseases. Do not neglect it because it is short. Though short, it contains all 108 types of eye diseases. Though numerous, they generally fall under two main categories: Internal Disorders and External Disorders. What is an External Disorder? External disorder is Cold. What is an Internal Disorder? Internal disorder is Qi. Following this main formula, one will not miss one in a hundred.
Commentary by Old Man Li of Tianfu Temple, Chongqing
Any condition involving redness, swelling, inability to open the eyes, unbearable pain, prominent red blood vessels and fleshy growths covering the eyes, photophobia, excessive tearing, eye discharge, crab-eye (pterygium/pseudopterygium), and wind-stroke pouring into the eyes, are classified as External Disorders.
Any condition where the interior is not red or swollen, painless, without red blood vessels or fleshy growths, with clear cornea, but only blurred vision, progressing to blindness over time, is classified as an Internal Disorder.
There are also several conditions neither caused by Qi nor Cold; their formulas are listed at the end.
External Eye Disorders
For treating all men, women, and children with any external eye disorder, characterized by redness, swelling, unbearable pain, photophobia, dislike of light, prominent red blood vessels, excessive tearing, or formation of opacities. Quack doctors consider this false fire attacking upwards, using both cooling and tonifying medicines. If it is true fire eye, it heals; if it is false fire eye, taking these cooling and tonifying medicines removes the redness and swelling but causes green membranes and white clouds to cover the pupil inside the eye, obscuring vision, leading to the belief that the person should remain disabled. This is due to unclear medication principles, causing mistaken harm. For wind-stroke pouring into the eyes and crab-eye, taking cooling medicines will cause perforation with no cure, and taking tonifying medicines will lead to death.
For all men, women, and children with external eye disorders, use Siwei Dafa San (Four-Ingredient Great Dispersing Powder) or Bawei Dafa San (Eight-Ingredient Great Dispersing Powder), adjusting based on symptoms. Taking one or two doses at the onset disperses chronic cold completely, leading to full recovery and preventing recurrence. If cooling or tonifying medicines are taken, green membranes and white clouds will form on the eyeball. At this stage, it is appropriate to take Great Dispersing Powder for four to five doses, followed by Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) for one or two doses to consolidate righteous qi.
Take Great Dispersing Powder for four to five doses again, then Buzhong Yiqi Tang for one or two doses. Alternating these internally, while applying external eye drops, once chronic cold is dispersed, white clouds and green membranes naturally dissipate. Why worry about the eye not regaining clarity?
For wind-stroke pouring into the eyes, where the head feels like being beaten with a club and the eyeball feels like being pierced by needles, unbearable pain. Internally, take Siwei Dafa San, heavily adding Beixixin (Northern Asari Radix et Rhizoma) two liang or three liang, and add Tianma (Gastrodiae Rhizoma) two liang (stir-fried with ginger juice). Externally, apply the Head-Wind Wrapping Medicine. Internal administration and external wrapping must occur simultaneously. One dose of each internally and externally leads to cure.
For crab-eye formation, continue using Siwei Dafa San, adding Quan Chong (Whole Scorpion) one liang or two liang (washed with wine to remove salt), and Jiangcan (Bombyx Batryticatus) one liang or two liang (stir-fried with ginger juice). One or two doses lead to cure.
If not cured, apply leech blood to the crab-eye externally. If there is fire, the crab-eye turns black; use Yan Furong Bing (Saliva Hibiscus Cake). These two formulas must be used concurrently with oral decoctions to be effective.
Siwei Dafa San (Four-Ingredient Great Dispersing Powder)
Mahuang Rong (Ephedrae Herba processed into fluff, removing powder) one liang or two liang, Manjingzi (Viticis Semen) one liang, Gaoben (Ligustici Rhizoma et Radix) one liang, Beixin (Northern Asari Radix et Rhizoma) five qian or one liang (leaves removed completely), Laojiang (Old Ginger/Zingiberis Rhizoma Recens) one jin or eight liang (crushed with skin as guiding agent).
Bawei Dafa San (Eight-Ingredient Great Dispersing Powder)
Based on Siwei Dafa San, add Xiqianghuo (Western Notopterygii Rhizoma et Radix) one liang, Bei Fangfeng (Northern Saposhnikoviae Radix) one liang, Baizhi Shao (Angelicae Dahuricae Radix tip) two liang, Chuanxiong (Chuanxiong Rhizoma) one liang. Still use Lao Shengjiang (Old Fresh Ginger) eight liang or one jin as guiding agent. Adjust based on symptoms as follows:
If white clouds obscure the pupil, add Shed Snake Skin (Ophiopogonis/Serpentis Cutis) one or two strips (washed with wine), Chan Tui (Cicadae Perioaterium) one liang (remove wings and feet), Bai Kou Ke (White Cardamom Shell) one liang (stir-fried with ginger juice). These three herbs scatter clouds and remove opacities. Must apply external eye drops.
If tears are hot and abundant, heavily add Manjingzi one or two liang; if tears are cold, add Bei Wuwei Zi (Northern Schisandrae Fructus) two liang, crushed. Note that cold tears indicate internal eye disorders; clarify this and use these five ingredients as the main basis.
If red blood vessels on the sclera do not disappear, add Sangbaipi (Mori Cortex) a handful, scraping off the rough bark with a knife. If red vessels are cleared, this herb is not needed, as it strongly drains lung qi.
If the eyeball swells and hurts at night, add Fei Xuanshen (Pharmacoscopiae Radix) one liang, Han Fangji (Stephaniae Tetrandrae Radix) one liang; or add Chao Xiangfu (Stir-fried Cyperi Rhizoma) one liang, Xiakucao (Prunellae Spica) one liang. If fleshy growths appear, add Baizhi Shao (Angelicae Dahuricae Radix tip) one or two liang.
If the temples swell and hurt, add Da Ming Ma (likely Ephedra or similar) one liang (stir-fried with ginger juice), pure Gaoben one liang or two liang. If eyes itch, add Quan Chong (Whole Scorpion) one liang (washed with wine), Jiang Chong (Bombyx Batryticatus) one liang (stir-fried with ginger juice). Crab-eye also adds these two herbs.
If the outer canthus is red, it is fire in the Large Intestine; add Shu Jun (Prepared Rheum Palmatum/Rhei Radix et Rhizoma) five qian or one liang, soaked in water in a bowl to drink. Shu Jun is stir-fried with wine.
If the inner canthus is red, it is fire in the Small Intestine; add Huai Mutong (Akebiae Caulis) one liang, Cheqian Ren (Plantaginis Semen) one liang.
If the tongue has a white coating, it is cold pathogen; yellow coating indicates chronic cold. Heavily add Mahuang (Ephedrae Herba) two liang or three to four liang, Xiong Pian (Aconiti Lateralis Radix Preparata/Fuzi) one liang or three to four liang, Xingren Shuang (Apricot Seed Frost) one liang.
Head-Wind Wrapping Formula
Use one raw Fuzi (Aconiti Lateralis Radix Preparata), one liang Hu Jiao (Piperis Longi Fructus), a bunch of Huo Cong (Allium Fistulosum/var. Glaucum), four liang Lao Jiang (Old Ginger).
Mash together, put in a pot, stir-fry with wine until very hot. Scoop out into a cloth pad and wrap the affected area of the head. It will feel like burning; do not remove the medicine due to extreme heat. Remove when it is no longer very hot. Keep the medicine, stir-fry and wrap again as before. If dry, add wine and wrap again. Two or three times leads to cure. Internal and external treatments must coincide.
Leech Blood Application Formula
Apply leech blood to crab-eye. Wrap the fish body in dry cloth, leaving two to three cun of the tail exposed. Have the patient lie supine. Cut off two to three cun of the tail to let blood drip onto the crab-eye. Do not stop.
Drip four to five times daily until cured. Internal and external treatments simultaneously, divine effect.
Saliva Hibiscus Cake
Saliva Hibiscus Cake specifically treats Fire Crab-Eye. Once fire appears, the color turns black. This formula also treats Eye Dan (carbuncle of the eyelid) and eye leakage.
Take four or five Saliva Pa Chong (likely insect secretion), five qian Furong Ye (Hibisci Mutabilis Folium, baked dry), mash into paste, make into a cake. Close eyes tightly, do not let it enter the eye. Apply this cake to the leaking eyelid. One or two applications cure it; do not apply excessively. Internal and external treatments simultaneously, must be effective.
For treating all men, women, and children with any external disorder, green membranes and white clouds obscuring the pupil, regardless of duration, first use Great Dispersing Powder to completely disperse chronic cold, then use Erbai San (Two-Whites Powder).
Erbai San (Two-Whites Powder)
Jiu Baishao (Wine-processed Paeoniae Radix Alba) two liang, Sheng Baizhi (Fresh Angelicae Dahuricae Radix) two liang.
Grind into powder. Take three to five qian each time, steam with pig liver. Chicken liver or sheep liver is better. Finish this material to cure.
Another single formula adds Bai Kou Ke (White Cardamom Shell) one liang, stir-fried with ginger juice. Take until cloud-opacities dissipate. After cure, take Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) one or two doses, and Shu Yi Bajie Tang (Cooked Enhance Morinda Decoction) one or two doses to conclude treatment.

Another formula provides temporary relief. Take a few Huoxiang Zi (Agastaches Fructus), place in the mouth for about a quarter hour, then remove; effective immediately.
Formula for Pointing Old Eye and All External Opacities
Luganshi (Calamina) one liang, Huajiao (Zanthoxyli Pericarpium) two liang.
Put Calamina in a clay pot, cover with Huajiao. Burn the bottom of the pot with white charcoal until Huajiao produces no smoke. When smoke stops, remove Huajiao. Grind Calamina extremely fine, apply to opacities. If adding Fan Naoshan (Ammonium Chloride) three fen, Cigu Fen (Colocasia Esculenta Root Powder) one qian, Baixiangding (Passer Domesticus Faeces) one qian, even more miraculous.
