On the General Treatment of Various Sores and Ulcers
The Heavenly Master said: "The treatment of the Middle Jiao refers to sores arising on the chest, on the breasts, on both hypochondriac regions, on both backs, and on both hands. However, sores and ulcers have their own specialized disciplines, so there is no need to elaborate further here. Since a category has been established, retaining one general treatment method can effectively manage sores in the Middle Jiao region with miraculous efficacy. The prescription consists of Lonicerae Japonicae Flos (Jinyinhua) 1 liang, Scrophulariae Radix (Yuanshen) 1 liang, Glycyrrhizae Radix et Rhizoma Preparata (Sheng Gancao) 5 qian, and Alum (Baifan) 2 qian; this embodies the principle that 'when there is disease, the medicine targets the disease.' Additionally, use Angelicae Sinensis Radix (Danggui) 1 liang, Paeoniae Radix Alba (Baishao) 1 liang, Gardeniae Fructus Praeparatus (Chao Zhizi) 3 qian, Schizonepetae Spica (Jingjie) 3 qian, Forsythiae Fructum (Lianqiao) 2 qian, and Brassicae Semen Album (Baijiezi) 2 qian. Decoct in water and take orally. [Commentary: A divine pill for dispersing pathogens and eliminating toxins.] One dose brings relief, two doses result in complete resolution, and for ulcerated cases, four doses lead to healing. For yin-type sores, remove Zhizi from the formula and add Cinnamomi Cortex (Rougui) 1 qian. This formula is effective for all types of sores in the Middle Jiao. Its brilliance lies in incorporating herbs that disperse pathogens and eliminate toxins within tonifying herbs, thereby enhancing detoxification and clearing heat. This is the method for treating the Middle Jiao."
Lord Zhang said: "The formula by Zhi Zhenren for managing sores and ulcers is exquisite, but I have an even more extraordinary prescription. Use Glycyrrhizae Radix et Rhizoma Preparata (Sheng Gancao) 1 liang, Angelicae Sinensis Radix (Danggui) 1 liang, Taraxaci Herba (Pugongying) 1 liang, Scutellariae Radix (Huangqin) 1 qian, Lonicerae Japonicae Flos (Jinyinhua) 2 liang, and Boswelliae Resina (Ruxiang) 1 qian, grind into fine powder. First, decoct the first five herbs in five bowls of water until reduced to one bowl. Mix with the Ruxiang powder and drink. It yields miraculous results and complements the efficacy of the previous formula. [Commentary: An immortal pill for dispersing toxins.] It can treat conditions throughout the entire body, serving as a general management method."
Lord Hua said: "I have nothing to add."
Sun Zhenjun said: "I also have a formula for managing various sores. Use Trichosanthis Radix (Tianhuafen) 3 qian, Glycyrrhizae Radix et Rhizoma Preparata (Sheng Gancao) 1 liang, Lonicerae Japonicae Flos (Jinyinhua) 1 liang, and Taraxaci Herba (Pugongying) 5 qian. Decoct in water and take orally. One dose alleviates symptoms, and two doses result in complete recovery. This formula has remarkable power in detoxification and can also manage various abscesses in the Lower Jiao."
On Leg Abscess, Multi-bone Abscess, Scrotal Abscess, Straddle Abscess, Crane-Knee Wind, and Crural Ulcers
The Heavenly Master said: "The treatment of the Lower Jiao refers to leg abscesses, multi-bone abscesses, scrotal abscesses, straddle abscesses, Crane-Knee Wind, crural ulcers, and foot gangrene. Among these, scrotal abscesses and straddle abscesses are the most difficult to treat. These conditions arise because young men neglect their health, frequenting brothels or indulging in sexual pleasures, suppressing ejaculation during intercourse, enduring hunger while maintaining arousal, forcibly holding back qi at the moment of ejaculation, or forcing erection after emission. Engaging in intercourse while another person is ejaculating also generates such malignant toxins. The prescription uses Lonicerae Japonicae Flos (Jinyinhua) 4 liang, Taraxaci Herba (Pugongying) 2 liang, Ginseng Radix et Rhizoma (Renshen) 1 liang, Angelicae Sinensis Radix (Danggui) 1 liang, Glycyrrhizae Radix et Rhizoma Preparata (Sheng Gancao) 1 liang, Rheii Radix et Rhizoma (Dahuang) 5 qian, and Trichosanthis Radix (Tianhuafen) 2 qian. Decoct in water and take orally. One dose reduces swelling, two doses cure it completely, and for ulcerated cases, three doses suffice. This toxin enters through deficiency; therefore, one must strongly tonify blood and assist with herbs that expel pathogens, causing the disease to vanish like loss. Otherwise, the condition lingers, and disaster follows swiftly. Rather than allowing the toxic pathogen to spread and then attempting to disperse it later, is it not better to vigorously expel it with one dose while the healthy qi is still strong? The brilliance of this formula lies in Lonicerae Japonicae Flos, with Angelicae Sinensis Radix as the sovereign herb for tonifying blood, Ginseng Radix et Rhizoma as the minister herb, and Rheii Radix et Rhizoma as the guide herb, achieving proper balance in dosage and weight.
"The treatment for Crane-Knee Wind differs. This condition arises from dampness combined with sexual activity while standing, where water-dampness invades the body. The prescription uses Astragali Membranacei Radix (Huangqi) 8 liang, Cinnamomi Cortex (Rougui) 3 qian, Coicis Semen (Yiren) 4 liang, Poria (Fuling) 2 liang, Atractylodis Macrocephalae Rhizoma (Baizhu) 2 liang, and Saposhnikoviae Radix (Fangfeng) 5 qian. Decoct in over ten bowls of water until reduced to two bowls. Divide into two doses: take one in the morning and one before sleep. Cover with thick blankets after taking the medicine to induce heavy sweating. Do not remove the blankets lightly; allow the sweat to dry naturally for healing. One dose is sufficient; re-administration is unnecessary. The brilliance of this formula lies in using Huangqi to tonify qi, as movement of the legs depends on qi circulation. In Crane-Knee Wind, qi deficiency prevents adequate circulation, making walking difficult. Using half a jin of Huangqi greatly strengthens qi. Rougui assists in unblocking qi, and Fangfeng disperses pathogens; though seemingly opposing, they complement each other. Baizhu and Yiren eliminate cold-dampness. Once pathogens are removed, healthy qi solidifies, leading to rapid recovery. If one fears the patient cannot tolerate it and avoids using it, harm will result.
"Multi-bone gangrene arises in the middle of the thigh, characterized by extra bone growth, caused by damp-heat. With proper treatment, it is easy to cure; otherwise, terrifying changes may occur. The prescription uses Angelicae Sinensis Radix (Danggui) 1 liang, Lonicerae Japonicae Flos (Jinyinhua) 1 liang, Paeoniae Radix Alba (Baishao) 1 liang, Bupleuri Radix (Chaihu) 1 qian, Artemisiae Scopariae Herba (Yinchen) 3 qian, Gentianae Radix (Longdancao) 2 qian, Atractylodis Macrocephalae Rhizoma (Baizhu) 3 qian, and Glycyrrhizae Radix et Rhizoma Preparata (Sheng Gancao) 3 qian. Decoct in water and take orally for immediate cure. [Commentary: Divine pill for transforming bone.] If neglected, a bone grows and inserts horizontally between the skin, causing pain; it must be extracted for healing. Remove the bone with iron forceps and apply the previous flesh-regenerating ointment externally. Two treatments lead to recovery. The brilliance lies in Baishao, which pacifies Liver Wood and activates tendons. Multi-bone gangrene is not true bone but tendon transformed into bone-like tissue. Baishao not only clears Liver fire but also disperses Liver pathogens. Once pathogens are gone, tendons relax, and the bone-like tissue dissolves. Combined with Jinyinhua's detoxifying properties, these two herbs work synergistically.
"Foot gangrene is also caused by damp-heat. The prescription uses Lonicerae Japonicae Flos (Jinyinhua) 1 liang, Taraxaci Herba (Pugongying) 1 liang, Glycyrrhizae Radix et Rhizoma Preparata (Sheng Gancao) 3 qian, Angelicae Sinensis Radix (Danggui) 1 liang, and Coicis Semen (Yiren) 2 liang. Decoct in water and take orally. One dose cures it. [Commentary: Dispelling dampness and eliminating pathogen powder.] The brilliance lies in using Yiren as the sovereign herb, as dampness typically affects the lower body, and water flows downward. Yiren removes dampness and benefits joint qi, while Jinyinhua clears fire-toxin. Assisted by Sheng Gancao, pathogens disperse easily and dampness retreats. Blood deficiency allows water-dampness to invade easily. Danggui tonifies blood; when blood is sufficient, water cannot invade, and dampness cannot enter, ensuring appropriate usage and rapid effect.
"Crural ulcers are also caused by damp-heat, often resulting in rotting legs with foul odor. Herbal decoctions alone are ineffective. First wash with green onion soup. Then prepare: White Wax (Baila) 1 liang, Red Lead (Huandan) 2 liang, Earthworm excrement from leek fields (Qiucai Di Shang Erchong Fen) 2 liang, dried by frying to 1.5 liang, Borneolum Syntheticum (Bingpian) 5 fen, Camphora (Chaonao) 3 qian, Moschus (Shexiang) 5 fen, Daemonorops Fructum (Xuejie) 5 qian, Plumbum Carbonicum (Qianfen) 1 liang, Fried Rosin (Chao Songxiang) 3 qian, Ruxiang defatted 3 qian, Myrrha (Moyao) 3 qian, Viriditas Cupri (Tonglv) 3 fen, Calomelas (Qingfen) 1 qian, and Terminalia Chebulae Retartulum (Ercha) 3 qian. Grind each into extremely fine powder. While the wound is wet from the onion wash, sprinkle the powder onto the ulcer. It will cause unbearable itching, but do not scratch. Soon, yellow fluid resembling golden juice will flow out in several bowls. Wash again with onion soup, repeat sprinkling and flowing three times. Fluid will decrease and pain will stop. [Commentary: Divine pill for separating dampness and detoxifying.] The next day, apply the previous ointment on thick leather, adding 2 qian of this powder, and let fluid drain. If itching is extreme, stroke with crane feathers to relieve itching. Apply two patches to stop fluid and heal. Leg abscesses follow the multi-bone treatment method; no new formula is needed. For crural ulcers, take internal decoction: Lonicerae Japonicae Flos (Jinyinhua) 1 liang, Coicis Semen (Yiren) 2 liang, Poria (Fuling) 1 liang, Glycyrrhizae Radix et Rhizoma Preparata (Sheng Gancao) 5 qian, Achyranthis Bidentatae Radix (Niuxi) 5 qian, Dioscoreae Hypoglaucae Rhizoma (Bixie) 5 qian, Pinelliae Rhizoma Preparatum (Banxia) 5 qian, and Cinnamomi Cortex (Rougui) 5 fen. Decoct in water and take orally. [Commentary: Internal transformation pill for separating dampness.] Combine with external ointment application. Using this formula continuously, two doses cure it. The brilliance lies in Yiren as sovereign, Jinyinhua and Bixie as ministers, and Fuling as assistant-guide. Yiren removes dampness from feet, Fuling drains dampness from Spleen and Stomach, Sheng Gancao and Jinyinhua resolve stagnant heat-toxin, Bixie travels to the lower limbs, dispels dampness, and strengthens shins, assisted by Niuxi to support tendons. Thus, rotting damp ulcers vanish effortlessly. This is the finest aspect of Lower Jiao treatment."
Lord Zhang said: "Lower Jiao treatment ends here, but I wish to elaborate further. Multi-bone formation, though originating from damp-heat, is not solely formed by it; it often results from a preference for cold water and fruit consumption. At the onset of multi-bone gangrene, use Rheii Radix et Rhizoma (Dahuang) 1 liang, Hibisci Mutabilis Folium (Furong Ye) dried and powdered 1 liang, Moschus (Shexiang) 3 fen, Borneolum Syntheticum (Bingpian) 3 fen, Myrobalanus Chebulae Fructus (Wubeizi) 1 liang, Garcinia Hanburyi Resina (Tenghuang) 3 qian, and Alum (Sheng Fan) 3 qian. Grind into powder, mix with rice vinegar to form a thick paste. Apply around the multi-bone gangrene, enclosing the skin and flesh, leaving a bean-sized opening in the center. Frequently sweep the opening with vinegar using a goose feather. If not swept and allowed to dry enclose, it is useless. Internal resolution occurs within one day and night. [Commentary: Detoxifying powder.] Gangrene arising between the Huantiao acupoints often becomes multi-bone gangrene if this encircling medicine is not used. Therefore, once gangrene appears, regardless of bone presence, apply this medicine immediately for miraculous effect. It is equally effective for other carbuncles, sores, and boils.
Lord Hua said: "I have nothing to discuss."
Lei Gong said: "I also have a formula for treating multi-bone conditions, using internal resolution methods with remarkable efficacy. Any toxin reaching the Huantiao acupoint constitutes multi-bone gangrene. Use Ginseng Radix et Rhizoma (Renshen) 3 qian, Rheii Radix et Rhizoma (Dahuang) 5 qian, Taraxaci Herba (Pugongying) 1 liang, Lonicerae Japonicae Flos (Jinyinhua) 2 liang, Trichosanthis Radix (Tianhuafen) 3 qian, and Coicis Semen (Yiren) 3 liang. First, decoct Yiren in six bowls of water to obtain three bowls of liquid. Then decoct the remaining five herbs to obtain three bowls. Divide into two doses. Taking two doses over two days resolves the condition. It is a divine formula. If already ulcerated, use the Heavenly Master's method. [Commentary: The Heavenly Master said: The formula is very magical, surpassing mine.]
(Note: Ancient texts often used Da Fangfeng Tang for Crane-Knee Wind, combining Qi and Blood tonics, as the disease is in the Lower Jiao Yin level. This formula excludes Blood tonics, suitable for early stages. For chronic cases, ancient formulas should not be discarded. Recorded by Li Ziyong.)
