Chen Shiduo said: The so-called "Guiding Treatment Method" (Yin Zhi Fa) refers to guiding the treatment upward for diseases located in the lower part, and downward for diseases located in the upper part. For instance, if a person suffers from deficiency fire (Xu Huo) boiling up in the throat, mouth, and teeth, using cold-natured medicines may bring slight relief upon ingestion, but shortly after, the condition worsens. Using more cold medicines leads to diarrhea and abdominal pain, while the heat in the upper body becomes even more intense. If one attempts to use hot-natured medicines taken cool, the patient may not believe in this method and refuse easy treatment. Thus, external guiding methods are employed to achieve a cure. The prescription consists of one Fuzi (Aconiti Lateralis Radix Praeparata), ground into powder, mixed with rice vinegar into a paste, and applied to Yongquan (KI 1). Shortly thereafter, the fire energy subsides; soon after, the heat stops and recedes, leaving the body feeling icy cold. Then, a large dose of Liuwei Dihuang Wan Decoction (Six-Ingredient Rehmannia Pill Decoction) is administered, and the fire no longer boils up. This type of fire is akin to thunder-fire; it becomes fiercer when encountering water. Have you not observed the vibration of thunder? During times of dense clouds and heavy rain, the thunder vibrates more intensely, shaking heaven and earth, adding to its momentum. However, once the sun shines brightly, the rain ceases, the sound disappears, and silence prevails. Have you not observed the world in winter? Severe cold, frost, and snow freeze the suburbs; rain and snow fall thickly, and the yin wind is harsh. This is because heavenly qi descends while earthly qi rises; thus, if the lower part is hot, the upper part naturally becomes cold. Have you not observed the world in summer? The scorching sun steams, heat accumulates and burns, scorching trees and forests, like boiling soup and water; this is because heavenly qi ascends while earthly qi descends, resulting in heat above and cold below. Human deficiency fire operates similarly. Now that the fire surges upward, the lower limbs become ice-cold. By using Fuzi, a medicine of great heat, to guide it via Yongquan (KI 1), it is because although Yongquan (KI 1) is a water point (Jing-well point of the Kidney Meridian), there is actually fire energy stored within the water. Fire nature tends to flare upward, while the acupoint itself is cold. Suddenly receiving fire energy causes the water to boil and warm up; as the water warms, the fire naturally descends. Like energies seek each other, and it must return to its original dwelling. Once the fire returns to its dwelling, how can it continue to boil in the heavens? This is the subtle miracle by which the fire in the throat and teeth suddenly vanishes without apparent cause. The ingenuity of this guiding treatment should be understood.
Master Zhang said: There is another method of guiding treatment; record it carefully. For patients suffering from Jue Ni (Reversal Cold/Extremity Inversion) who dare not take internal medication, use Wu Zhu Yu (Evodiae Fructus), one liang, ground into powder, mixed with half a liang of flour, and water to form a thick paste. Spread this onto a cloth shaped like a bell, with a paper thickness of half a fen, and apply it to Yongquan (KI 1). The hands and feet will cease to be inverted/cold. Moreover, symptoms of heat above and cold below can all be treated with this guiding method. This is also the method of guiding fire back to its source (Yin Huo Gui Yuan).
