On the Regulation of Shanghan (Cold Damage), On the Treatment of Summerheat, On the Regulation of Zhongfeng (Wind Stroke), On the Regulation of Zhongshi (Dampness Strike), On the Regulation of Huozheng (Fire Syndrome), and on the Aftercare of Zaozheng (Dryness Syndrome).
The Heavenly Master said: "Zhongzhi" (Final Treatment) refers to the planning for aftercare once the disease has been cured; hence it is called Final Treatment. For example, how to regulate after Shanghan (Cold Damage) has healed; what decoctions to drink after Summerheat; and how to recuperate after Zhongfeng (Wind Stroke)—these are the cases in point.
When the pathogenic factors of Shanghan (Cold Damage) have completely receded, the healthy Qi naturally becomes deficient, and it is appropriate to tonify the healthy Qi. However, patients often exhibit strong Stomach Qi but weak Spleen Qi; if they consume large amounts of tonifying agents, they may be able to eat but unable to digest or absorb them. The method should employ fewer herbs that tonify the Stomach and more herbs that tonify the Spleen. It is especially inadvisable to use many Spleen-tonifying herbs while using few Kidney-tonifying herbs. This is because the Kidney can generate Earth (Spleen), and Earth can naturally generate Metal (Lung). When Metal is prosperous, Wood (Liver) will be restrained and will not excessively control Earth (Spleen). Therefore, tonifying the Kidney is precisely a way to tonify the Spleen. The prescription uses Shudi (Rehmanniae Radix Preparata) 1 liang, Maimai Dong (Ophiopogonis Radix) 3 qian, Wuweizi (Schisandrae Fructus) 5 fen, Baishao (Paeoniae Radix Alba) 3 qian, Rougui (Cinnamomi Cortex) 3 fen, Baizhu (Atractylodis Macrocephalae Rhizoma) 3 qian, Yiren (Coicis Semen) 3 qian, and Baijiezi (Sinapis Semen) 1 qian. Decoct with water and take orally. (Commentary: Pi Shen Zhi Zi Tang [Spleen-Kidney Supreme Tonification Decoction].) This prescription specifically tonifies the Kidney and Spleen meridians; without deliberately targeting every organ for tonification, all organs are effectively treated.
Summerheat injures Qi, but its treatment should not prioritize regulating Qi; instead, it should focus on nourishing Blood. This is because when Yang is injured, Yin-Blood is also consumed. The prescription uses Danggui (Angelicae Sinensis Radix) 1 liang, Baishao (Paeoniae Radix Alba) 3 qian, Chuanxiong (Chuanxiong Rhizoma) 1 qian, Shudi (Rehmanniae Radix Preparata) 1 liang, Wuweizi (Schisandrae Fructus) 1 qian, and Maimai Dong (Ophiopogonis Radix) 3 qian. Decoct with water and take orally. This prescription is essentially Siwu Tang (Four Substance Decoction). Its brilliance lies in consisting entirely of Yin-meridian herbs, supplemented by Maimai Dong (Ophiopogonis Radix) and Wuweizi (Schisandrae Fructus) to nourish Lung-Metal. Since Lung-Metal is now prosperous, it can restrain Liver-Wood from controlling Spleen-Earth. Thus, Siwu Tang (Four Substance Decoction) nourishes the Liver, allowing it to remain tranquil and untroubled.
After Zhongfeng (Wind Stroke), there is also a deficiency of Qi. Such conditions strictly require tonifying Qi and must not involve tonifying Blood. This is because Zhongfeng (Wind Stroke) occurs after Blood stasis; adding more Blood tonics would exacerbate Qi stagnation. The prescription uses Renshen (Ginseng Radix) 3 qian, Fuling (Poria) 3 qian, Yiren (Coicis Semen) 3 qian, Banxia (Pinelliae Rhizoma) 1 qian, Shenqu (Massa Medicata Fermentata) 5 fen, Baizhu (Atractylodis Macrocephalae Rhizoma) 5 qian, Gancao (Glycyrrhizae Radix et Rhizoma) 1 qian, Rougui (Cinnamomi Cortex) 1 qian, and Chenpi (Citri Reticulatae Pericarpium) 5 fen. Decoct with water and take orally. (Commentary: Qi Xue Liang Bu Dan [Qi and Blood Dual Tonification Pill].) This prescription excels at tonifying Stomach-Qi to generate Lung-Metal-Qi, and tonifying Mingmen (Gate of Life) fire to generate Spleen-Earth-Yin. With this, why fear the prosperity of Wind-Wood (Liver-Wood)? These three prescriptions are excellent methods for aftercare and can serve as principles for Final Treatment.
Master Zhang said: "This is exquisitely brilliant. What more can I say? I shall supplement each point." After Zhongshi (Dampness Strike), if dampness has been purged, the method should focus on strengthening the Spleen. However, one cannot merely strengthen the Spleen; one must tonify the fire of Mingmen (Gate of Life) to generate Spleen-Earth. The prescription uses Baizhu (Atractylodis Macrocephalae Rhizoma) 5 qian, Fuling (Poria) 3 qian, Rougui (Cinnamomi Cortex) 3 fen, Baishao (Paeoniae Radix Alba) 3 qian, Yiren (Coicis Semen) 5 qian, and Baijiezi (Sinapis Semen) 1 qian. Decoct with water and take orally. This prescription specifically tonifies the fire of the Kidney meridian without being excessively hot. Consequently, Spleen-Qi receives warmth, naturally dispelling dampness and generating Stomach-Qi.
Once the Fire Syndrome has dissipated its residual heat, the patient's breath will likely become faint, rendering them unable to sit or stand. If one一味ly (solely) clears heat, Stomach-Qi will be damaged, and the marrow exhausted; when will the Water (Kidney-Yin) regenerate? The prescription uses Shudi (Rehmanniae Radix Preparata) 1 liang, Xuanshen (Scrophulariae Radix) 5 qian, Maimai Dong (Ophiopogonis Radix) 1 liang, Niuxi (Achyranthis Bidentatae Radix) 1 qian, and Baishao (Paeoniae Radix Alba) 3 qian. Decoct with water and take orally. (Commentary: Ji Shui Tang [Water-Nourishing Decoction].) This prescription is ingenious in moistening Lung-Metal to generate Kidney-Water, while simultaneously calming the Liver. Once these three Zang organs are tranquil, Stomach-Qi will naturally regenerate. Why bother clearing residual heat further?
For Dryness Syndrome, once the pathogen is eliminated, the aftercare strategy lies solely in vigorously tonifying Kidney-Water. When Kidney-Water is sufficient, Lung-Metal will be nourished. The prescription uses Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill), supplemented with Maimai Dong (Ophiopogonis Radix) and Wuweizi (Schisandrae Fructus), can be used for treatment.
Master Hua said: "I have not transmitted these methods either, so there is nothing to discuss."
