There are sixteen types of internal cataracts; medical practitioners must examine them carefully. Clearly understanding the morphology of each one ensures safety without needling.
Confusing needle techniques with methods for treating visual obstructions (Yi) may damage Shen Guang (spiritual light/vision), making recovery difficult. One must first clarify cold, heat, deficiency, and excess, harmonizing the four limbs to maintain peace.
Otherwise, Qi stagnation and improper care may lead to vomiting, mental fatigue, and reversal of the opacity. Coughing or head tremors indicate unsuitability for needling; if the patient is prone to fright, Zhenjing Wan (Calming Fright Pill) should be administered first.
When seeking cooling agents like Yin Gao (Silver Paste), one must assess the condition at the time. For old opacities, use fine needles; for thick ones, thin needles; for tender ones, different approaches apply. Needle shapes must not be uniform.
For patients with deficiency, postpartum women, or those in pregnancy, handling requires great caution in care. Choose days without rain or wind, and observe a pure fast for three days before needling.
Practice medicine with a calm and focused mind; avoid noise from relatives during chanting. The patient sits cross-legged facing brightness; the practitioner adjusts their belt and steadies their heart.
The practitioner follows the conduct of sages, guided by compassion—a true good karma. If bleeding occurs, stop immediately; re-bandage as before and inspect again.
If sudden convulsions occur, the physician bears heavy responsibility; after thirty days of medication, clarity may return. After seven days, upon unbandaging, although objects may be seen, if there is a sensation of movement like flowers in water, do not speak unnecessarily.
Persistently take Huanjing Wan/San (Restore Vision Pill/Powder); after one hundred days, vision will clearly recover its original foundation.
After the golden needle treats internal cataracts, the physician's instructions must be deeply understood. Wrap black beans in cotton like a ball, place it on the eye, and secure it with cotton ties.
Ensure the head rests steadily on the pillow (some versions mention bean support); lie supine for three days without impatience. If slight pain occurs after bandaging,
do not doubt if it is caused by Brain Wind (Nao Feng). Follow classic methods for needling or cauterization; if pain is extreme, apply hot compresses.
To relieve nausea, hold and swallow juice from dried plum (Bai Mei); after vomiting, continue lying supine as instructed. Rising may cause strain, so though rare, this must be known.
For seven days, consume warm soybean porridge. Avoid clenching teeth; defecate and urinate slowly; do not rise independently or rely on support.
Shouting or excessive talking may disturb the eyeball, causing visual disturbances like flying snow. Maintain this devotion for thirty days, then gradually go outside to recognize acquaintances.
Avoid recalling yin-yang matters when the mind is restless; sleep separately from spouse for one hundred days. Do not wash the face near the eyes for one month.
As the needle site remains warm and painful, abstain from five pungent spices, alcohol, and wheat products for a year. Take mild medications to eliminate the root of the disease.
