Those from wealthy families who live in excessive comfort often suffer from stagnation and obstruction of Qi and Blood, which frequently prevents the fetus from turning. Therefore, before delivery, one must engage in regular exercise to ensure the smooth flow of Qi and Blood, facilitating fetal movement and making childbirth easier. This is a matter that should be attended to in advance.
In the month preceding delivery, parturients should not consult fortune-tellers or deities. If shamans or priests deceive and frighten them for profit, falsely claiming ominous signs and praying for divine protection, hearing such words will inevitably cause doubt and fear. Doubt and fear in the heart lead to Qi stagnation and Blood stasis, resulting in difficult labor; this must be avoided.
Normal delivery (Zhengchan) occurs when, at the time of impending birth, the midwife assesses the parturient's condition based on observation and careful examination. If the infant has reached the birth canal, abdominal pain will be urgent, and lower back pain will be severe. The chest feels sunken, and the pubic symphysis separates. There is an urgent need to urinate and defecate simultaneously, golden flowers flash before the eyes, and amniotic fluid or blood flows down. At this moment, the child has exited the womb, and if the mother exerts effort once, the child will be born immediately, avoiding mishaps. If these symptoms have not yet appeared, even if half a day or a day passes without delivery, it is harmless. One must absolutely avoid panic among family members of all ages, nor should they pray to gods or make vows, as seeing or hearing this will inevitably cause anxiety in the parturient. Anxiety leads to timidity and weakness, making it difficult to eat or drink. One should not, out of pity for the mother's pain and eagerness for her relief, force her to exert effort. Exerting effort too early affects the lives of both mother and child; how can one not revere and caution against this?
For young women experiencing their first delivery where the pubic symphysis does not open, possibly due to premature labor or forced pushing causing the fetus to lie transversely in the abdomen with ineffective medications, Shen Zuo Yin (Divine Oak Drink) is suitable (also treats all types of dystocia). For cases where the membranes have ruptured for a long time, amniotic fluid is exhausted, wind enters the birth canal, and the birth passage is dry and slippery leading to dystocia, Shen Ying San (Divine Response Powder) is suitable. For middle-aged women who have given birth too many times, suffering from Qi and Blood deficiency leading to dystocia, Tuo Hua Jian (Flower-Removing Decoction) is suitable. For cases where bleeding precedes, or amniotic fluid breaks first, with the fetus reversing and rushing upward, Da Shun Tang (Great Smoothness Decoction) is suitable, or simply using 70 seeds of Abelmoschus manihot (Huangkui Zi) (stir-fried, ground into powder) taken with water. For formulas effective in managing all types of dystocia, such as Jia Wei Xiong Gui Tang (Modified Ligusticum and Angelica Decoction), Fo Shou San (Buddha's Hand Powder), You Mi Jian (Oil and Honey Decoction), E Jiao Tang (Donkey-Hide Gelatin Decoction), Sheng Jin Dan (Victory Golden Pill), and Dang Gui Huang Qi Tang (Angelica and Astragalus Decoction) can be selected. For cases where amniotic fluid breaks first but delivery does not follow, extending to two, three, four, or five days, 5 qian of Fish Maw (Yu Jiao) (processed to retain nature, ground) mixed with wine is suitable, or 3 qian of Malva verticillata seeds (Dong Kui Zi) (stir-fried) decocted and taken orally.
